As EE was reassembling the silo pipes, EE attempted to use an impact tool and wrench combination to tighten a bolt. The impact torqued the wrench, and EE's little finger was pinned between the wrench and the pipe.

EE had opened the haul truck hood with a crowbar, and then EE was attempting to use the bar to prop the hood open while EE was filling the truck with antifreeze. In the process, EE shifted position and the crowbar came loose and crushed EE's hand between the hood and the engine bay.

Employee was on the lower platform of the control house, assisting in freeing a rock obstruction from the mouth of the jaw crusher. Employee had opened the top of the crusher cage so that the operator could manually free the obstruction. After obstructions were freed, the rock entered the jaw and the vibration caused the guard to fall and impact employee's left hand.

The employee was climbing down the ladder of a haul truck, missed the last few steps and fell to the ground.

The miner was walking toward the plant and noticed the front end loader coming up behind EE and stepped to the left side of the road to move out of the way. As EE stepped down, EE's left ankle rolled causing an ankle injury.

Operator was attempting to repair a leaking seal on a rotary valve. The operator placed their hand between the chain & the drive sprocket. EE's hand was pulled into the sprocket & crushed between the chain & the sprocket. The incident resulted in a broken thumb & 2 broken fingers in addition to a laceration across EE's entire hand.

EE was climbing onto loader using provided steps and his foot slipped off bottom step, causing him to hit his upper teeth onto step of ladder.

After dismounting the plant, the injured turned to walk and his knee popped and caused him to fall to the ground. Injured thought his knee would be o.k. and worked the rest of the day. That night, his knee swelled and he went to the doctor. Injury caused a strain to the employee's knee.

Operator was backing down ramp toget load of water with half of load water on. Trans converter failed truck was free wheeling using brakes lightly or feathering, and turned into high wall, water sloshed to low side turned over. Driver had seat belt on.

Employee was using wedge to clear rocks hung up in jaw. Took safety glasses off to see down in jaw. Broke rock loose and wedge went down past jaw bottom. Had installed new toggle plate. Had closed gap on jaw. Struck wedge chipped off metal and cut beside right eye.

Stopped to tire boot strings, put foot on truck bumper, leaned over achilles tendon sanpped left foot.

WHILE WALKING FROM CRUSHER, EE SLIPPED, FELL, HITTING RIGHT KNEE ON GROUND. NO LOST TIME. KNEE SWELLED THAT NIGHT, EXAMINED 9 AM NEXT MORNING BY DOCTOR. BRUISED, BONE CHIPS, BRACE ON KNEE, RES UME WORK.

STANDING IN TRUCK BED WELDING TURNED FOOT SLIPPED LANDED ON METAL WEDGE IN BED. BRUISED RIBS AND BACK ON SIDE NO LAST TIME.

EMPLOYEE WAS COVERING ROCK WITH TARPS AT THE END OF WORKDAY. GRABBED TARP WRONG WAY AND PULLED MUSCLE IN SHOULDER.

EE WENT TO GET IN END DUMP AT BEGINNING OF WORK DAY. GRABBED HANDRAIL TO PULL UP AND TWISTED LOWER BACK.

EMP WAS GRINDING METAL DOWN AND HAD SAFETY GLASSES ON AT THE TIME A PIECE OF METAL GOT IN EYE (LEFT).

2 SOUTER EMPS WERE JACKING UP JAW TO UNBLOCK WHILE EMP WAS DIGGING OUT ON OPPOSITE SIDE AND ROCKCAME DOWN FROM TOP AND STRUCK EMP ON THE HEAD.

ROCKS HUNG IN CRUSHER HURT BACK WHILE PULLING ROCKS OUT OF CRUSHER. LUMBAR STRAIN TO BACK.

REPLACING CONVEYOR BELT. STANDING ON #4 CONVEYOR BELT ON THE LEFT SIDE, SLIPPED AND FELL STRAIGHT DOWN SIDE OF CONVEYOR BELT LANDING ON LEFT ANKLE TURNING IT TO THE INSIDE OF FOOT. PULLED LIGA MENTS ON OUTSIDE OF LEFT FOOT AND ANKLE.

EE WAS OUT OF HIS LOADER AND WAS UNCOVERING ROCK PULLING BACK PLASTIC VISQUEEN 40'X100' SHEET WHEN HE FEEL INJURING RIGHT KNEE.

EMPLOYEE WAS OUTSIDE OF LAODER, IN FRONT OF WINDHIELD CLEANING WINDHIELD, WHEN HE FELL AND HIT RIGHT ELBOW ON LOADER ARM. EMPLOYEE STATES HE SLIPPED AND FELL WHILE CLEANING WINDSHIELD. NO BRO KE SKINS NO VISABLE BRUISE. EMPLOYEE WAS SENT TO DR AND ELBOW WAS X-RAYED. NO BREAKS FOUND. LEMPLOYEE WAS RELEASED TO GO BACK TO WORK.
