In rigging process of mechanical piece when the crane operator picked up the piece it moved to the front and hit a wood pallet and the wood pallet hit the left foot of the miner.
The supervisor went to see the pump area were one diesel pump was having problems. At the moment he was getting near to the pump, the valve at the exit of the pump got loose from the fitting and started spray diesel all over the supervisor.

Employee was cutting with an oxygen/acetylene torch a flat metal sheet in the workshop, while doing the final cut a piece of 14" X 27" X3/8" slide from the workbench falling into his left foot. Employee was wearing his PPE.
Employee was doing overhead welding under the vacuum truck tank. Suddenly he feel pain in his right shoulder when finishing the job.
Supervisor was initiating the start-up process of the Roller Mill,by overload of material in the mill table,he proceeded to rotate the table with the auxiliary motor. The auxiliary motor has a manual coupling to start-up the engine in local; the handle had a pneumatic coupling mechanism; it failed to connect properly causing the handle to bounce back hitting his right hand.
Employee operating the automatic bag filling machine and noticed malfunction of the bag applicator; he removed the guard that protects the pulley and introduced his hand to check the belts; his arm obstructed the photocell device activating the spout mechanism. His left hand got caught between the belt & pulley.
Employee was working in the removal of gear box from the spare drive shaft of E01 chain. Two hydraulic jacks with supplemental pieces were used. While the jacks were in pressure the gear box was heated to clear the interference between the couplings. As the coupling came loose one of the supplements slipped out and hit the employee on his right leg.
Workers were preparing to remove the roller bearing from the intermediate symetro gear using a tri-jaw hydraulic gear puller. After securing the puller and applying the pressure, they started warming up the roller so it would begin loosening the bearing. On a sudden release of the bearing, one of the puller jaws slung out impacting the worker's left hand and chest.
Employee was assembling an electrical control panel at the Electrical workshop workbench. To test the control circuit he energized the 120V power source. While fastening the cables with tie straps, his left hand slipped and came in contact with the thyristor cooling fan (3X3 inches), lacerating the tip of his left hand 5th finger.
Employee was making perforations to a 1" rubber conveyor belt with the arbor press at the machine shop. After completing various punch, the 3/4" hollow punch bit slip-out from the press hitting the employee right hand third finger.
EE WAS PERFORMING WATERJET CLEANING OF THE MMD- SECONDARY CRUSHER USING THE WOMA HIGH PRESSURE PUMP. AFTER FINISHING THE CLEANING HE ACCIDENTALLY SWIPED THE WATERJET OVER HIS LEFT FOOT WITH A PRESSURE OF 7,000 PSI, THE WATERJET SPRAY CUT THRU BOOT BOTH IN THE STEEL TOE SECTION AND A SMALL SECTION ABOVE THIS WHERE THERE WAS ONLY LEATHER ALLOWING A STREAM OF HIGH PRESSURE WATER
EE'S WERE ASSIGNED TO REMOVE THE BELT TENSION BY LIFTING THE COUNTERBALANCE OF CONVEYOR BELT. THEY STARTED SETTING UP THE BELT & HOOKING THE SLINGS TO THE CRANE & THE COUNTERBALANCE. THE CRANE STARTED LIFTING W/O ANY DIFFICULTIES. THEN THE BOOM TURNTABLE STARTED RIPPING APART, CAUSING THE BOOM TO FALL OVER ONTO THE CONVEYOR BELT. EE WAS PINNED BETWEEN THE HANDRAIL & BELT. FX BACK.
EE'S WERE ASSIGNED TO REMOVE TENSION BY LIFTING THE COUNTERBALANCE OF CONVEYOR BELT. THEY STARTED SETTING UP THE BELT & HOOKING THE SLINGS TO THE CRANE & COUNTERBALANCE, THE CRANE STARTED LIFT ING W/O ANY DIFFICULTY. IN A MATTER OF SECONDS THE BOOM TURNTABLE STARTED RIPPING APART, CAUSING THE BOOM TO FALL OVER ONTO CONVEYOR BELT. EE HIT BY HANDRAILS & PINNED BETWEEN BELT & RAILS.
EE WAS ASSIGNED TO REMOVE THE BELT TENSION BY LIFTING THE COUNTERBALANCE OF CONVEYOR BELT. EE POSITIONED THE CRANE EAST OF BELT. MAINTENANCE EE'S STARTED SETTING UP THE BELT & HOOKING THE SLIN GS TO THE CRANE & THE COUNTERBALANCE. THE CRANE STARTED LIFTING THE COUNTERBALANCE W/O ANY DIFFICULTY, THEN THE BOOM TURNTABLE STARTED RIPPING APART, EE JUMPED FROM THE CRANE & WAS INJURED.
EE WAS WORKING IN THE REPLACEMENT OF DRAG CHAINS OF THE CLINKER ELEVATOR USING AN HYDRAULIC PRESS TO CLAMP CHAIN BUCKETS; WHILE USING THE PRESS ON ONE OF THE BUCKETS A METAL FRAGMENT SNATCHED FROM BUCKET PIN IMPACTING THE EE IN HIS LT. SIDERIB AREA, CAUSING A SUPERFICIAL PUNCTURE CUT. EEWAS STANDING AT A DISTANCE OF 5-6 FEET WEARING PERSONAL PROTECTIVE EQUIPMENT.
CONTRACTOR WORKER WAS DOING ROUTINE MAINTENACE IN THE GREEN AREAS NEXT TO THE ADMINISTRATION AND SCALE BUILDINGS. THE WORKER WAS OPERATING THE LAWN MOWER TRACTOR, A PIECE OF TREE TRUNK GOT ST UCK UNDER THE MOWER BLADES, AT THAT MOMENT THE WORKER STEP OUT OF THE TRUCK WITH THE EQUIPMENT RUNNING, THE TRACTOR WAS IN A SLOPE, HIS RIGHT LEG GOT UNDER THE MOWER BLADE.
EE WAS WORKING IN THE SCALE WEIGHT AJDSTING BALANCING THE CHOCK BOTTLES, IN THE PROCESS OF LOWERING THE SUPPORTING BAR OF THE SCALE, HIS LEFT HAND WAS CAUGHT BETWEEN THE SUPPORTING BAR OF THE SCALE & THE SHOCK BOTTLE. EE SUFFERED A FINGERTIP FRACTURE OF HIS LEFT HAND 3RD FINGER
SUPERVISOR WAS VERIFING THE DUST COLLECTOR OF THE POLDOS WEIGH SYSTEM TO CHECK FOR POSSIBLE MATERIAL OBSTRUCTION. HE INTRODUCE HIS RIGHT HAND THRU AN INSPECTION PLATE WITH THE EQUIPMENT IN IOP ERATION AND THE BLADES OF THE ROTARY AIR LOCK VALVE CAUGHT HIS HAND EE SUFFER INJURY IN THE 2,3RD & 4TH FINGERS, WITH PARTIAL AMPUTATION OF THE 3RD FINGERTIP
EE WAS OPERATING THE AUTOMATIC BAG FILLING MACHINE AND NOTICE A MALFUNCTION OF THE BAG APPLICATOR, IN THE COURSE OF POINTING OUT WHAT WAS WRONG WITH THE AUTOMATIC APPLICATOR, HE ACTIVATED THE MACHINE AND HIS LEFT HAND INDEX FINGER GOT CAUGHT BETWEEN THE BAG APPLICATOR ROLLERS.
EE TRIED TO MANUALLY PUSH A LOADED CEMENT BAGS PALLET WITH THE MACHINE RUNNING. HIS LEFT HAND GOT CAUGHT BETWEEN THE WOOD PALLET & THE MOVING ROLLER.
EE WAS CHECKING THE AIR SLIDE GATE OF THE CEMENT PACKING MACHINE AND A SUDDEN BLOW OF CEMENT DUST IMPACTED HIS FACE AND EYES CAUSING SEVERE EYE IRRITATION.
EMPLOYEE ALLEGED FEELING CAMPS IN THE NECK AND UPPER ARM AREAS. THE EMPLOYEE REQUESTED MEDICAL ATTENTION ALLEGING CONDITION WAS WORK RELATED.
EE WAS WORKING IN THE SHOP HANDLING A PIECE OF EQUIPMENT WHICH WAS BEING LOWERED BY AN ELECTRIC CRANE. A SUDDEN MOVEMENT (SLIP?) BY THE BAR CAUSED A CUT TO HIS RIGHT THUMB.
EMPLOYEE TRIED TO PUSH MATERIAL ACCUMULATED IN FRONT OF DOOR OF BY-PASS OF DUST COLELCTOR. THE AIR LOCK WAS RUNNING AND HE CAUGHT-IN HIS HEART AND INDEX FINGERS. LOST 1/3 OF HEART FINGER.
