Miner was working to repair a crusher using parts cleaner. Parts cleaner and oil from the crusher got onto EE's pants. When EE used a grinder to cut steel sleeve, sparks ignited EE's pants and EE's knees and calves were burned.

Worker was removing nuts using and air impact wrench and his right wrist became sore. He did not report the injury to management until 05/26/2009. Went to a clinic and was placed on restricted duty. No lost time.
Worker was repairing a air-powerd greaser; the piston became stuck. When he tried to free the piston the fingers of his left hand became trapped by the piston causing a laceration to the middle finger of his left hand requiring stitches to close the wound.
EE STARTED THE AIR COMPRESSOR AND A PARTICLE CAME OFF THE EXHAUST AND FLEW IN HIS RIGHT EYE. THE CENTER OF HIS RIGHT EYE WAS SCRATCHED BY THIS MATTER.
EE WAS GRINDING IN A REAL TIGHT SPOT AND THE GRINDER (9" HAND HELD GRINDER) KICKED BACK AND HIT MY LEFT KNEE. SOME SWELLING AND HURTS WHEN HE BENDS THE KNEE BUT IF HE STANDS UP STRAIGHT DOES N OT HURT.
EMPLOYEE WAS CUTTING A PLATE WITH A TORCH AND GOT TO THE END OF THE CUT WHEN THE PLATE FELL ON HIS LEFT FOOT. EMPLOYEE WAS CARELESS AND DID NOT HAVE HIS SAFETY SHOES ON WHICH WOULD HAVE PREVEN TED THE BRUISE TO HIS LEFT ANKLE, AS AREA WOULD HAVE BEEN COVERED. EMPLOYEE WAS INSTRUCTED AT TIME OF STARTING EMPLOYMENT THAT STEEL TIPPED SHOES ARE REQUIRED TO BE WORN EVERYDAY AT WORK.
EMPLOYEES WELDING SIDE BY SIDE. EMPLOYEE USED WELDING GOGGLES/SHIELD BUT FAILED TO SHIELD HIMSELF FROM OTHER WELDER. FLASH BURNS OCCURRED INBETWEEN WELDS FROM OTHER WELDER'S ARC.
EE WAS GRINDING EDGES OF PLATE METAL, OBJECT LODGED IN RIGHT EYE, EE STATES THAT HE WAS WEARING SAFETY GLASSES AT TIME OF ACCIDENT.

WAS CARBON ARK A BUCKET, WHEN A HOT PIECE OF SLAG WEN INSIDE MY SHOE.
EE WAS GRINDING ON A BUCKET, WHEN A PIECE OF METAL FLEW IN HIS EYE (LEFT).
WHILE WELDING RECEIVED A SLAG PUNCTURE IN RT FINGER.
