EMPLOYEE WAS OPERATING MECHANICAL SCALER ON 08/29/24. LATER NOTICED A KNOT BETWEEN SHOULDERS. NOTIFIED SUPERVISOR ON 09/01/24.
EE CUTTING CHAIN WITH HAND GRINDER. GRINDER BOUND UP AND SLIPPED AND EE GOT A CUT ON LEFT HAND BETWEEN THUMB AND INDEX FINDER REQUIRING 5 SUTURES.
IMPACT AND SOCKET CAME OFF BOLT WHILE RE-TORQUING BOLTS. EE FELL LANDING ON PINKY FINGER OF LEFT HAND. EE RECEIVED SUTURES IN FINGER AND PLACED ON RESTRICTED DUTY AT THAT TIME.
EE USING A CHOP SAW TO CUT HEADER TUBES FOR THE FOAM GROUT PUMP, INHALED VAPOR FROM CUTTING THE FOAM INSIDE THE TUBE. EE PLACED ON RESTRICTED DUTY ON 10/13/2022.
EE WELDING/CUTTING ON HAUL TRUCK AND GOT FOREIGN OBJECT IN LEFT EYE. EE TAKEN FOR MEDICAL TREATMENT TO REMOVE FOREIGN OBJECT AND RECEIVED PRESCRIPTION EYE MEDICATION AND RELEASED BACK TO FULL DUTY.
As a 20' section of NQ rods was being lifted up along the slide to be added to the drill string, the top of the water swivel caught the edge of a tower strut leveraging the bottom of the rods to kick up striking the miner on the left elbow and shoulder forcing the miner to the deck.
EE cutting a cylinder stem with torch, the stored energy cause the cylinder to kick back striking EE on forehead, lacerating forehead. EE received sutures to forehead and released back to full duty following that medical treatment.
EE felt pain in back during repetitive bending during the drilling process. EE went for medical treatment on 2.26.18 and was placed off work at that time.
EE hooking up bottle rack on crane, another EE bumped the crane control to go up a little bit and caught EE left index finger between crane hook and bottle rack. EE was taken to ER and received 9 sutures in left index finger. EE released to restricted duty.
EE got a foreign object in eye while welding. Nothing was imbedded in eye, however, EE was given a prescription eye medication to prevent infection.
EE using an air hose to clean and got debris in his left eye. EE was given a prescription eye drop and released back to work with no restrictions.
EE was operating a grader, when put into reverse the grader jerked and EE felt pain in neck. EE received restricted duty per company Dr.
While EE was blowing dust and debris with air hose, EE got something in left eye. Dr removed small piece of metal imbedded in left eye.
EE was exiting drill when his foot slipped, felt pain in Right shoulder. Dr prescribed pain medication.
EE got debris in eye while grinding. Doctor removed debris from eye and gave him a prescription eye drop.
EE was using a hand grinder with the guard provided when the grinder slipped off the metal he was grinding and cut his left wrist resulting in stitches.
Running 3"casing. While making a connection, employee let down winch line before connection was made smashing the employee's middle right finger with the 3"casing.
EE was washing sweeper truck hopper out with high pressure water hose, when a piece of dry concrete fell down on top of his hat. EE had pain in neck and extending to mid back. After treating with company doctor, EE was referred to a specialist and due to restrictions, he could not return to work on 3/22/10.
EE was attempting to move a boulder. He put his hand on wire rope while pushing the down button on crane control wire rope pulled his finger into the sheeve. Smashing pinky and ring finger on right hand. EE was taken to ER and later transferred to Des Peres Hospital where he had surgery on 11-17-09 making this a lost time accident.
While backing remote past himself, right rear tire shot a rock out hitting the employee in the knee. After continuous treatment employee had surgery to the right knee on 11/04/09 making this a lost time.
Employee was walking along side of crusher and got dirt in right eye. Employee was taken to company doctor on 6-30-08 and received antibiotic eye drops.
Employee was changing the motor on pump. When the torch was turned on to warm bolts to loosen, fire flashed. Employee was taken to area hospital and remains off work, making this a lost time accident.

Employee was using wire wheel to clean up some parts for a tractor when a piece of wire came off and went into his left forearm. He later felt pain and reported accident on 12-06-07 and was then taken to company doctor. A specialist was not able to remove the piece of steel. He decided to leave it in his arm as it could do more damage to remove it. Employee received 6 sutures.
2 employees were welding at same time working on reclaim hopper. Apparently EE was too close & rec'd flash burns to both eyes. EE was taken to Salem ER & prescribed eye drops & an anti inflamatory prescription.
EE was rebuilding crusher. While holding up 1-1/2" impact wrench, his elbow hurt & swelled. EE taken to Co. Dr. Elbow not any better and seen by dr on 3/21/07 where he was treated with cortizone injection in elbow. It was suggested if not better in 2 weeks, he was to see a orthopedic specialist.
Compressed air line to shotcrete nozzle appeared to be clogged when air was turned on. The employee noticed air line started to come off nipple, he tried to shut the compressor off & the line came off & squirted accelerant in his eye. EE was taken to the company Dr. & prescribed an antibiotic.
EE scaled an area when a piece of loose fell across the end of boom, lifting cab, causing operator to be lifted up in the seat. Results were hitting his head on cross member of the cab, receiving a laceration to his forehead between left eye and hairline. ee was taken to emergency room where he received sutures.
EE WAS SCALLING A BROW WHEN LOOSE FELL ON BOOM OF SCALER BOUNCING OPERATOR INSIDE CAB. HE HIS HIS HEAD (HARD HAT) ON ROOF AND HARD HAD CUT RIGHT SIDE OF HEAD. HE ALSO FELT PAIN IN LOWER BACK. TAKEN TO DOCTOR ON 8/22/05. DUE TO RESTRICTIONS FROM DOCTOR, COULD NOT RETURN TO WORK ON 8/23/05 AND BEGAN LOOSING TIME
EE WAS DRIVING SCALER TO SHOP. WHEN HE CAME AROUND CURVE, SCALER HIT A ROUGH AREA IN ROAD ROCKING SCALER SIDE TO SIDE HURTING NECK. EE WAS TAKEN TO PAIN MANAGEMENT CENTER ON 10-20-05 WHERE HE RECEIVED 2 CORTIZONE SHOTS. **Per 220060200045: On 1/11/06 ee had the nerve endings in his neck seared to desensitize them to alleviate pain. Ee remains on full duty.**
EE WAS CUTTING A ROOFBOLT. WHEN THE BOLT WAS CUT INTO, THE PRESSURE ON IT CUASED IT TO SWING AROUND AND STRIKE THE EE ON THE RIGHT INDEX FINGER. EE WAS GIVEN A METAL SPLINT TO PROTECT FROM FURTHER INJURY. X-RAYS WERE NEGATIVE
Employee stuck the expanded metal into grinder and it kicked back in his hand causing a cut to left thumb. Employee received 3 sutures.
EMPLOYEE WAS DUMPING LEAD CANS INTO VOLVO LOADER. WHILE ATTEMPTING TO HOOK THE CRANE ARM ON THE CAN EAR, HIS FINGER WAS MASHED BETWEEN THE CRANE ARM AND THE CAN EAR. LACERATION TO RIGHT MIDDLE FINGER, 1ST JOINT AND LACERATION REQUIRING 11 SUTURES.
Employee states while working next to concrete grout pump, the pump line plugged off causing the pump to blow out a seal. This caused the concrete grout to be blown into face and left eye.
EMPLOYEE WAS STARTING THE PIPE CUTTER WITH HIS LEFT HAND HOLDING DOWN THE LOCKING HANDLE. WHEN PRESSURE WAS APPLIED TO PIPE THE HANDLE KICKED UPPUSHING THE HANDLE UP. BACK OF EMPLOYEE'S HAND S TRUCK SHARP EDGE OF ANGLE IRON MOUNTED ABOVE CUTTER. BACK OF HAND RECEIVED PUNCTURE WOUND, REQUIRING 5 SUTURES.
EE WAS CUTTING A 2 INCH PIPE CLAMP BOLT WITH ANACETYLENE TORCH. WHEN HEAT WAS APPLIED TO BOLT, RUST WAS BLOWN INTO EE LEFT EYE. EE WAS TAKEN TO COMPANY DR ON 8-1-02, TO REMOVE FOREIGN OBJECT. DR APPLIED AN ANTIBIOTIC CREAM ON EYE MAKING THIS A REPORTABLE.
EMPLOYEE WAS WELDING LINER IN SOUTH SKIP WHEN HE RECEIVED WELDING FLASH TO BOTH EYES. EYE MEDICATION WAS ADMINISTERED. EMPLOYEE RETURNED TO WORK ON SAME SCHEDULED SHIFT.
EE REPORTED WHILE GRADING AN UNDERGROUND ROADWAY WITH A GALION 850 B GRADER, THE BLADE STRUCK A SOLID LEDGE PROPELLING HIM FORWARD AND ONTO THE STEERING WHEEL. EE UNDERWENT RIGHT SHOULDER SURG ERY TO REPAIR A TORN ROTATOR CUFF ON 2/20/02.
EE WAS GETTING ON #14 GETMAN. EQUIPMENT HAS A SWING TYPE LADDER. OLADDER SUNG UNDER BASKET CAUSING EE TO HYPEREXTEND RIGHT KNEE RESULTING IN ARTHROSCOPY SURGERY ON 10-03-01.
EMPLOYEE WAS INSTALLING ROOF BOLTS FROM A GETMAN BASKET. IN THE PROCESS THE EMPLOYEE MASHED HIS LEFT INDEX FINGER BETWEEN THE BASKET TOP GUARD RAIL AND THE "STOPER DRILL". THE INJURY REQUIRED 3 STICHES.
EE WAS USING A GRINDER TO SMOOTH THE EDGES OFF A PIECE OF THE STEEL. LATER DURING SHIFT THE EE'S RIGHT EYE BEGAN TO FEEL ITTITATED WITH SOME PAIN, BUT THE EE DID NOT FEEL MEDICAL CARE WAS NECE SSARY. AT THE BEGINNING OF HIS NEXT SHIFT ON THE FOLLOWING AFTERNOON THE IRRITATION & PAIN STILL CONTINUED. HE WAS THEN TAKEN TO THE DOCTOR & A SMALL SLIVER OF STEEL WAS REMOVED FROM RIGHT EY
TWO EMPLOYEES WERE USING THE UTILITY TRUCK TO HANG VENT BAG IN 696 N/E-850. ONE EMPLOYEE RAISED THE BASKET UP WHILE HOLDING VENT BAG IN OTHER HAND. HE HEARD INJURED CALL FOR HELP, TURNED AND SAW HIM PINNED BETWEEN THE WORK BASKET RAILING &THE BACK.
EE WAS WELDING ON HAND RAIL ON VOLVO LOADER. EE STRUCK ARC BEFORE HOOD WAS FLIPPED DOWN. AS EE FLIPPED HOOD DOWN, IT HIT A PIPE NEAR HIS HEAD, AND THEREFORE DID NOT COMPLETELY CLOSE. EE HAD TO MOVE HEAD BACK TO CLOSE HOOD. THE EVENT PROBABLY TOOK 2-3 SECONDS. A FLASH BURN TO BOTH EYES RESULTED. EE RETURNED TO WORK ON HIS NEXT SCHEDULED SHIFT.
WHILE HAND SCALING IN 204 NORTH DEVELOPMENT AREA A PIECE OF LOOSE FELL FROM BACK. THIS CAUSED EE TO STRIKE HAND RAIL OF WORK BASKET.
PIECE OF LOOSE FELL FROM BACK STIRKING EE ON HEAD AND SHOULER. THIS CAUSED HIM TO STRIKE THE WORK BASKET RAILING ON THE SCALER.
WHILE REMOVING LINER BOLTS FROM FEED END OF BALL MILL LINER WITH JACK HAMMER, EE USED LEFT HAND TO AID ANOTHER EMPLOYEE TO ALIGN JACK HAMMER BITON LINER BOLT. WHEN THE JACK HAMMER WAS ACTIVATE D THE EMPLOYEE'S HAND SLID DOWN BIT. EE RECEIVEDA FRACTURE, CONTUSION TO THE MIDDLE FINGER OF LEFT HAND DUE TO THE PINCH POINT CREATED BETWEEN THE END OF THE BIT AND FEED END OF THE BALL MILL
