CARPAL TUNNEL - LEFT HAND AND LEFT ELBOW

EMPLOYEE WAS TRYING TO UNCHOKE THE ZINC 1ST CLEANER LAUNDRY DISCHARGE LINE. A WATER HOSE WAS IN THE CHOKED-UP LINE, THE EMPLOYEE WENT TO PULL THE WATER HOSE OUT OF THE LINE. AS EE WAS PULLING THE WATER HOSE OUT OF THE PIPE, THE PIPE DISLODGED OUT OF THE PIPE CONNECTOR HUGGER AND FELL HITTING THEIR LEFT ARM AND LEFT HAND.

EMPLOYEE WAS CUTTING A BOX OPEN AND HEARD A POP IN RIGHT SHOULDER. THIS BECAME LIGHT DUTY ON 04/28/2025.

EMPLOYEE TWEAKED RIGHT KNEE WHILE EXITING CAGE

EMPLOYEE OPENED SCALER DOOR AND CAUGHT FINGER BETWEEN DOOR AND HANDRAIL.

EMPLOYEE WAS OPERATING A MT85 WALKBEHIND AND PINNED SELF BETWEEN THE RIB AND THE MACHINE.

EMPLOYEE WAS CLIMBING STEPS INTO EE'S DRILL, FOOT SLIPPED, EMPLOYEE FELL AND CUT LEFT FOREARM TRYING TO CATCH SELF FROM FALLING.

EMPLOYEE WAS GETTING OUT FROM UNDER FENDER, MOVED THE HYRO RAM HOSE OUT OF WAY, RAM CAME OFF SUSPENSION ARM AND RAM HIT BACK OF RIGHT ANKLE AND RIGHT HEEL.

CARPAL TUNNEL SYNDROME - BOTH WRIST

EMPLOYEE HAD OPEN KNIFE IN HAND WHILE ROLLING UP EMULSION HOSE. WHEN THEY WENT TO WRAP THE HOSE, THE KNIFE CUT THEIR LEFT INNER FOREARM ABOVE THE WRIST.

EMPLOYEE WAS REMOVING HYDRAULIC HOSE ON LOADER, TRIED BLEEDING THE LINE OFF, DIDN'T SEEM TO HAVE MUCH PRESSURE, WHEN IT POPPED OFF IT SPRAYED HYDRAULIC ONTO EMPLOYEE'S FACE

AFTER HAND SCALING, EMPLOYEE REPORTED WRIST IS SORE

THE EMPLOYEE LEANED AGAINST THE HANDRAIL ABOVE THE ROD CHANGER AND IT GAVE OUT AND THE EMPLOYEE FELL TO THE PLATFORM BELOW.

EMPLOYEE WAS LIFTING A 3.5 HP PUMP AND FELT PAIN IN THE LOWER BACK. THE INCIDENT HAPPENED ON 08/29/2024 AND THE EMPLOYEE DECIDED EE NEEDED TO BE SEEN ON 09/10/2024 AT THE MEDICAL CLINIC.

EE WAS RIDING IN ROXOR, BRAKES WENT OUT, HIT PILLAR CAUSING EE TO HIT HEAD ON ROOF OF ROXOR. 8/8/24 Injury Happened- The EE sought medical attention - reported they were personal injuries. 8/16/24 - EE ask for medical leave for a personal injury from (8/15-9/8). 9/9/2024 - The EE reported original injury 8/8/24 as a work injury that resulted of an alleged accident at work.

THE EMPLOYEE WAS IN A CRAMPED POSITION WORKING ON A SEATBELT OF A SCALER AND PULLED SOMETHING OUT OF PLACE IN EMPLOYEE'S BACK.

EMPLOYEE WAS GETTING RODS TO FEED THE MILL AND SMASHED FINGER ON LEFT HAND

WASP STING TO LEFT BICEP EMPLOYEE WAS FINE AND DID NOT REPORT TO THE DOCTOR. EE CONTINUED WORKING.

EE DIAGNOSED WITH BILATERAL CARPAL TUNNEL SYNDROME AND CUBITAL TUNNEL SYNDROME ON 4.3.2023. TREATMENT AND/OR SURGERY IS PENDING AT THIS TIME.

EE diagnosed with bilateral carpal tunnel and cubital tunnel on 01/16/2023. Treatment and/or surgery is pending at this time.

EE stepping onto first step of haul truck and foot slipped off step. EE felt pain in low back area. EE taken for medical evaluation and placed on restricted duty at that time.

EE CAUGHT BETWEEN CRANE TROLLEY AND I-BEAM. EE RELEASED BY MEDICAL PERSONNEL TO RESTRCTED DUTY. TREATMENT ONGOING AT THIS TIME.

EE was riding in a buddy seat of a haul truck and felt pain in the upper back. EE was taken for medical attention at that time. EE remains on restricted duty.

EE using a mining bar to pry a loose rock to access a drill hole in the floor and felt pain in right wrist. EE was placed on restricted duty on 10/4/2021.

Spring bolt sheared causing employees wrist to hit hand rail behind work area. Received a fracture to left forearm.

EE ADJUSTING THE FORKS ON A FORKLIFT WITH A WRENCH, WRENCH HIT EE ON THE CORNER OF THE RIGHT EYE RESULTING IN SCRATCH TO RIGHT EYE. EE RECEIVED PRESCRIPTION EYE DROP AND RELEASED TO FULL DUTY WITH NO RESTRICTIONS.

When removing metal bands from roof bolts, metal band lacerated employees right thumb requiring stitches. Employee returned to full duty after seeking medical attention.

EE DIAGNOSED WITH BILATERAL CARPAL TUNNEL SYNDROME ON 4/26/2021. TREATMENT / SURGERY IS PENDING AT THIS TIME.

EE PULLING ON A WRENCH TO BREAK LOOSE A BOLT ON A SEAT BELT AND FELT PAIN IN LEFT WRIST. EE TAKEN FOR MEDICAL EVALUATION AND WAS DIAGNOSED WITH A LEFT WRIST SPRAIN. EE PLACED ON RESTRICTED DUTY. EE FOLLOW UP ON MONDAY AND RELEASED BACK TO FULL DUTY.

EE walking in park area doing a pre-operational equipment inspection and tripped on a rock falling and injuring right elbow. EE placed on restricted duty following medical examination.

EE got out of pick-up truck to open a gate on gravel road and slipped and fell on icy roadway. EE felt pain in left knee. EE seen at emergency room and released to restricted duty at that time.

EE pulling on wrench to tighten boom roller and felt pain in right shoulder. EE was seen for medical attention on 02/01/2021 and placed on restricted duty at that time.

EE WAS REACHING TO FLIP A VALVE, HAND SLIPPED OFF VALVE AND FELT PAIN IN LEFT SHOULDER. EE PLACED ON RESTRICTED DUTY AFTER MEDICAL TREATMENT ON 01/15/2021. EE WAS RELEASED TO FULL DUTY AFTER MEDICAL FOLLOW UP ON 01/18/021.

EE STEPPING OFF BOTTOM STEP OF TRUCK AND STEPPED ON ROCK TWISTING RIGHT ANKLE. EE PLACED ON RESTRICTED DUTY FOLLOWING MEDICAL TREATMENT.

EE WELDING/CUTTING ON HAUL TRUCK AND GOT FOREIGN OBJECT IN LEFT EYE. EE TAKEN FOR MEDICAL TREATMENT TO REMOVE FOREIGN OBJECT AND RECEIVED PRESCRIPTION EYE MEDICATION AND RELEASED BACK TO FULL DUTY.

EE bent over to pick up a roll of tape off the ground and felt pain in back. EE placed on restricted duty following medical treatment.

EE ATTEMPTING TO MOVE A PIECE OF EQUIPMENT INTO THE MENS CHANGE ROOM WHEN THE PIECE OF EQUIPMENT FELL STRIKING EE. EE RECEIVED FRACTURE TO LEFT FOOT AND SMALL SCRATCH BY RIGHT EYE. EE PLACED ON LIGHT DUTY WHILE UNDER MEDICAL TREATMENT FOR FRACTURED FOOT.

EE twisted left knee while splicing cable. EE placed on restricted duty after medical treatment on 3.20.2020.

EE fell from truck while cleaning lights. EE placed off work after surgery to repair broken bones in leg.

EE pulling on 30 horse pump and felt pain in low back area. EE placed on restricted duty on 1/6/2020 and released on 1/13/2020 to full duty.

Wheel came off truck causing truck to hit rib. EE placed on restricted duty after medical treatment on 8/3/2019

Vibrating screen lid broke (the eyelet holding the screen lid) and fell on employee. EE was placed on restricted duty on 7/29/2019 due to left shoulder strain.

EE USING SLEDGE HAMMER TO REMOVE A BOLT AND FELT PAIN IN RIGHT ARM AND ELBOW. EE DID NOT RECIEVE MEDICAL TREATMENT UNTIL 8/28/2019 AND PLACED ON RESTRICTED DUTY AT THAT TIME.

EE driving haul truck, hit a bump in the road, EE felt pain in low back area. EE was placed on restricted duty for strain after medical treatment the following day.

EE bending over to pick up a piece of drill steel and felt pain in low back area. EE went for medical treatment on 6/7/2019 and was placed on restricted duty at that time.

#34 HOIST WENT DOWN AT 7:20 AM. MSHA WAS CALLED AT 7:35 AM. SURFACE MAINTENANCE REPLACED 3 BUSHING IN GEAR BOX THAT ROTATES MECHANICALLY AND TELLS THE HOSITMAN THE CAGE LOCATION. HOIST WAS RUNNING, INSPECTED, AND PLACED BACK INTO SERVICE AT 08:00 PM. MSHA WAS CALLED AT 08:12 PM REPORTING THE HOIST IN OPERATION.

EE reaching to stop a piece of drill steel from rolling off steel holder and felt pain in right shoulder. EE was placed on restricted duty following medical treatment on 3/28/2019.

EE lifting and pulling on a pump and felt pain in low back area. EE was placed on restricted duty on 2.6.19 and remains on restricted duty at this time.

EE opening a gate, right thumb smashed between the gate and metal brace fracturing right thumb. EE will require surgical procedure to repair fracture and will be able to remain on full duty after surgery due to scope of job requirements.

EE diagnosed with occupational carpal tunnel syndrome on 2.22.19. Treatment and/or surgery is pending.

EE pulling on wrench to replace the sheave on the south tails pump when EE felt pain in right shoulder. EE treated at medical clinic on 10.8.2018 and paced on restricted duty at that time.

EE DIAGNOSED WITH OCCUPATIONAL CARPAL TUNNEL SYNDROME ON 7/30/2018. TREATMENT / SURGERY IS PENDING AT THIS TIME.

EE bitten/stung by insect. Took EE for medical treatment and release back to work full duty.

EE operating loader and caught the bucket on a rock jarring EE in cab. EE felt pain in shoulder. EE was placed on restricted duty on 7/17/2018 following medical treatment.

#34 service hoist went down at 6:55 am. Contacted electricians at 6:55 am. Called MSHA at 7:25 am. Electricians found a bad contact. Hoist was back up and running again at 10:20 am. Called MSHA at 10:36 to report hoist was in service.

EE felt pain in back during repetitive bending during the drilling process. EE went for medical treatment on 2.26.18 and was placed off work at that time.

EE moving a starter box when the box shifted and right hand was caught between box and handrail. EE taken to emergency room and received sutures in right hand.

EE letting clothes basket down in changeroom when deris fell off basket and landed in EE left eye. Debris was imbedded and was removed by dr. and prescribed antibiotic eye drops. EE was released back to full duty following medical care.

EE scaling down loose when EE felt pain in back. EE was placed on restricted duty on 9/26/2017.

EE diagnosed with Occupational Carpal Tunnel Syndrome on 7.25.17. EE treatment and/or surgery pending at this time. EE will remains full duty at this time. If EE requires surgery, EE will return to work with restricted duty.

EE prying with a scaling bar to unplug a boulder from a chute and felt pain in right shoulder. EE did not seek any medical treatment at that time. EE sent for medical treatment on 2.21.18 and was released with no restrictions. A follow up appointment on 3.22.18, the EE was placed on restricted duty at that time.

EE lifting 50 lb bags of chemicals and felt pain in abdomen. CT scan revealed a hernia. EE being referred for treatment. EE placed on light duty pending treatment.

EE received lacerations requiring stitches on two fingers of left hand after reaching in to adjust an engine and cut fingertips on fan blade.

EE was wiping the trunnion bearing on the ball mill when ee felt a pain in middle back. EE was placed on restricted duty pending treatment.

EE stung by wasp when reaching into back of truck. No allergic reaction. No medical treatment needed.

EE in a heading when a piece of rock fell striking EE in the head. EE received sutures and released back to full duty.

EE diagnosed with bilateral carpal tunnel syndrome on 7.25.16. Surgery dates are pending at this time. EE remains on full duty until surgical procedure.

EE cleaning rock out of plugged chute, employee lost footing when the splitter car moved causing employee to fall into a handrail, resulting in 3 fractured ribs. Employee currently on restricted duty.

EE diagnosed with Cubital Tunnel Syndrome. Surgery is pending at this time.

EE lifting a piece of steel and felt pain in left shoulder. EE placed on restricted duty pending testing on 5.11.16.

EE was walking to the park to get a haul truck to drive to shop to make repairs when EE stepped on a rock and twisted knee. EE was placed on restricted duty on 5.5.16 and will require surgery. Surgery is pending for 5.16.16.

EE stepped up on the bottom step to climb up into a loader when the step broke and a piece of metal lacerated EE's left leg requiring stitches.

EE diagnosed with Carpal Tunnel Syndrome on 4/4/2016. Surgery is pending, date unknown at this time. EE will be released with restricted duty following surgery.

EE hit a chute liner with a hammer to loosen it, EE felt something in EE's eye. Medical personnel did not find anything in eye, but was written a script for eye medication.

EE diagnosed with bilateral carpal tunnel syndrome on 2/8/16. EE remains full duty until EE has surgery. Surgery is tentatively scheduled for 3/24/2016. After surgery, EE should return to restricted duty.

EE was shutting metal doors. EE had closed the left hand door when the air pressure caught the right hand door and caught EE hand between the door. This caused a laceration on right hand requiring 4 stitches. EE released back to work all full capacity same day.

EE moving tables, dropped a table hitting EE's right hand/wrist. EE was released to full duty. EE received a splint to keep on EE's wrist to partially immobilize.

EE diagnosed with Carpal Tunnel Syndrome and recommended for surgery. EE is released full duty until surgery. Surgery dates are pending at this time. After surgery EE will be released to restricted duty.

EE diagnosed on 10/13/2015, with carpal tunnel syndrome. Dr diagnosed and recommended surgery. Surgery date is pending. EE remains on full duty until surgery.

Hoist went down due to hydraulic leak. MSHA called at 8:35. Repairs were made and hoist was inspected and released at 9:30 am. MSHA was called back at 9:30 am. No miners were endangered at any time.

EE was hand scaling and felt pain in shoulder. EE placed on restricted duty.

EE was attempting to cut a bolt out of the hitch of a portable vent fan with a handheld hacksaw, the saw slipped out the cutting groove and cut the top of his left hand. EE received 5 stitches. No restrictions.

EE had worked normal shift and went home. Mill had mechanical problem and EE was called out to help with needed repairs. EE was treated for heat exhaustion.

#34 Service Hoist went down on 6/7/15 at 4:40 am due to an electrical issue. Electricians were called as well as MSHA. Electrician's repairs were complete; hoist was inspected and placed back into service at 10:05 am. MSHA was called back to update status at 10:08 am.

#34 Service Hoist was taken out of service at 9:30 am on 12.26.15 for the normal 7 day inspection. Hoist inspection was completed at 12:00 pm with problems found during the inspection that did not allow the release of the cage. MSHA was called as well as maintenance personnel to make needed repairs. Repairs were made and hoist was re-inspected and released at 12:00 pm on 5.27.15

EE slipped while climbing down ladder. EE was treated with first aid for scrape on thigh. EE now complains of back pain. Injury became reportable on 8.4.15 when doctor placed EE on restricted duty.

EE diagnosed with cubital tunnel syndrome (occupational illness) and specialist is recommending surgery. Surgery date has not been set up yet.

At 2:15 pm on 3.28.15 the electrical power wire fell down the shaft. The electricians and utility personnel were called to make needed repairs. The shaft was inspected and cleared for use at 1:55 pm on 3.30.15.

EE getting off the cage underground fell hitting elbow on ground. Fracture of bone in elbow.

EE felt sharp pain in back area under ribs while hand scaling. Dr diagnosed a fracture to one rib. EE released to restricted duty.

EE caught the heel of his boot on a rock causing him to fall and land on his right arm, hand, and wrist. EE placed on restricted duty.

EE was lifting the access door to the vibrating feeder when he felt pain in lower back. Dr. put EE on restricted duty.

EE was wrapping a pallet with shrink wrap and felt pain right arm, elbow, and wrist. Initial ER visit released back to work full duty. Follow up visit on 11.13.14 put her on restricted duty.

EE stepped on water hose and twisted right ankle. EE was placed on restricted duty.

EE was exiting boulder breaker, his foot slipped off the step and he caught himself with his arms, he felt pain in his back. EE was placed on restricted duty.

EE was walking to mill when something blew in his eye. EE was taken to eye dr. and prescribed eye drop to prevent infection to scratch on eye. Right eye.

EE diagnosed with Carpal Tunnel Syndrome on 12.08.14. EE will remain on full duty until after surgery. Surgery has not been scheduled at this time.

EE alleged carpal tunnel. EE treated with conservative care. Dr recommended surgery on cubital tunnel syndrome with ular nerve. Surgery is scheduled for 2.6.15, EE will return on restrictive duty following surgery.

Imbedded debris in left eye. EE was under a truck removing bolts when debris fell into eye. EE was taken to eye dr and a piece of metal was removed from eye.

EE was walking out of lunchroom when he lost his balance and twisted his right knee.

Carpal Tunnel Syndrome - EE had surgery on one hand on 06/04/14, other surgery to follow in approx 7 weeks.

EE was breaking bolts loose on a transmission when he felt pain in right elbow. EE has been on full duty since that date. Dr has determined surgery is needed and will is scheduled on 4/2/2015. EE should return on 4/6/15 on restricted duty.

EE was exiting drill when his foot slipped, felt pain in Right shoulder. Dr prescribed pain medication.

Carpal Tunnel Syndrome in both hands.

EE was assisting in installing an explosive box, as the forklift guided the box down EE got left middle finger pinched between box and truck. Laceration to left middle finger requiring stitches. Restricted duty until x-ray results are known.

Employees were using an Elliot Aerial Platform to perform scaling work when the man basket they were in fell to the ground.

Alleged carpal tunnel. Employee having pain beginning in left hand and wrist, has advanced to pain in hands, wrists, and right arm.

EE was tightening up bolts on haul truck when he felt pain in his stomach. He was diagnosed on March 15th with a hernia and had surgery to repair the hernia the same day. The employee's next scheduled worked day is March 21st.

The old metal weigh hopper floor was attached to the skip hoist and was being pulled up out of the mine when the metal floor got wedged inside the shaft preventing the 7 employees at that current time to exit the crusher. MSHA was notified at 5:00 p.m. and the employees were hoisted out of the crusher at 8:50 p.m.

EE alleged pain in his hand and wrists were becoming numb. He was taken to the company doctor and was diagnosed with carpal tunnel.

EE was struck by a liner when the new chocker broke that was holding the liner. The ee was placed on restricted duty on 12/14/11.

EE was hitting a wrench with a hammer and hit his left hand resulting in stitches.

Employee was changing out a sway bar on a 740 Cat Truck with the assistance of another employee when the bar slipped out of their grip smashing the employee's left ring and pinky fingers. The employee x-ray results were positive for a fracture on the tip of the pinky finger making this reportable.

EE was conducting a presift exam on his mechanical scaler when he was walking around the scaler his foot rolled on a rock and he heard his right knee pop. EE worked until 5/6/11, when he had his knee surgery.

Employee was hand scaling when a piece of rock struck the scaling bar smashing the EE's hand between the bar and the basket of the getman. EE received seven stitches to left index finger making this reportable.

Employee was placing banding on counterweight spool of #1 pump station. Wrench slipped allowing banding to slip- laceration between left thumb and index finger and right thumb. Employee required three stitches making this reportable.

EE was placing tie block under loader and smashed thumb in between block and frame of loader. Laceration to left thumb requiring surgery.

EE was shoveling rock out of shaft bottom, felt left knee pop. EE was taken to company doctor and was not released to come back to work.

EE was cutting a v-belt with a knife when he cut his left forearm. He sustained a laceration resulting in stitches to his left forearm.

EE was washing sweeper truck hopper out with high pressure water hose, when a piece of dry concrete fell down on top of his hat. EE had pain in neck and extending to mid back. After treating with company doctor, EE was referred to a specialist and due to restrictions, he could not return to work on 3/22/10.

EE was remoivng a rusty bolt under the shop truck, when a piece of rust/dirt fell onto her face and under safety galsses, into her right eye. EE was taken to ER where her eye was washed out and she was prescribed antibiotice eye drops, making this reportable.

EE was using hammer and chisel to split bearing, when a piece broke off and went into left finger. EE was taken to company doctor and when removing the piece of steel it required 3 sutures, making this reportable.

EE was moving pipe from the metal chop saw when it fell hitting him in the right index finger. EE was taken to company doctor and was diagnosed with a fracture to his right index finger.

While backing remote past himself, right rear tire shot a rock out hitting the employee in the knee. After continuous treatment employee had surgery to the right knee on 11/04/09 making this a lost time.

EE was installing center pins in getman. While prying with mining bar he felt pain in right groin. EE was taken to company doctor on 8/21/09 and diagnosed with a hernia and not to return to work until he has surgery.

Employee was loosening a hydraulic hose when wrench slipped and hit him in the mouth chipping his tooth. Employee was seen by a dentist on 08/07/09.

Employee was removing fuel tank and as he climbed into operators compartment he jammed his neck when he hit his head on bottom of seat. After continuous treatment with company physician, he was diagnosed with a fracture in the neck. He was taken off work effective 08/14/2009.

Employee was digging lifters out with potato hoe and hand came into contact between handle and rock. Employee was taken to Parkland Emergency Room and received 5 sutures to left pinky 2nd knuckle.

EE was preparing to weld new steps. He was standing in door opening and air caught door and swung closed. Door hit right elbow/forearm and knocked him down on left wrist. After continuous treatment with doctor and specialist, he was taken off work on 7/31/09 until evaluated by specialist.

Employee was climbing the steps to cab when his foot slipped and hit his left knuckles/hand on step. Employee went to emergency room on 9-20-08 and x-ray revealed a fracture to left side of hand.

Employee was replacing north hoist rope. He was putting floor plate back on cage and as he lifted it he pulled muscle in his right arm. Employee was taken to company doctor and later he was referred to orthopedic doctor. He was given a cortisone injection on 09-30-08.

Employee was standing in basket holding the emulshion hose when he caught the on/off valve with mine belt. When pump came on he lost hold of hose and it sprayed him in the right eye. He was taken to eye doctor and released to work.

EE was walking on uneven concrete floor, when she turned her left ankle. EE was taken to ER on 8/25/08 and on 8/27/08. She was seen by company physician and diagnosed with a fracture of the foot. She began loosing time on 8/27/08.

Employee pulled himself up on jumbo hood to work on slip rings and stated he pulled his back. He was seen by company dr. on 5/21/08 and again on 5/27/08. Dr took off work pending x-ray and wants to keep him off work until next evaluation.

Alleged carpal tunnel from repetitive motion of normal job duties. Employee treated with specialist and had surgery for one wrist on 12-10-08 and the other wrist on 12-29-08.

EE hooked right toe on concrete and fell into crusher motor, hitting right ear, cheek and right shoulder. EE received 5 sutures for the laceration on the ear.

Ratchet slipped off of nut causing socket to hit employee in mouth. Employee has broken/chipped teeth and laceration/swollen lips. Employee was taken to company doctor and dentist.

Employee was changing wheel on toro loader. When he pulled on wrench to take sheel lugs loose he felt burning pain in right elbow. He was taken to dr on 8/8/07 with a follow-up visit on 8/20/07. He was prescribed physical therapy which he began 9/4/07.

EE was moving a 55 gallon barrel of grease & felt back pop. EE was taken to co. Dr , and later referred to specialist. EE rec'd cortizone shot in back 5/25/07 making this reportable.

EE was walking down a muck pile when a rock rolled & hit the release button on prosthetic leg. EE rolled to bottom of muck pile resulting in a cut that required 9 stitches. EE went to Salem ER and was treated.

Employee was walking across slick floor when he fell down and hurt his back. EE has had continuous treatment with company Dr. and specialist. Employee had an epidural injection on 11/3/06 and will began physicial therapy 11/14/06.

Employee was dumping wet pb over rail into pb high grade sump. too much weight in basket causing it to tip forward into sump. He was taken to co. dr. and after continuous treatment he has surgery on august 28, 2006 making this a lost time incident.

Employee was removing worn gear from ZN filter when the sharp edge of the gear cut his right thumb. He was taken to company dr. where he received 7 sutures.

Employee was hand mining when the tip broke on the mining bar and had a sudden jar to right side. Employee has been treating with company doctor and then was referred to an orthopedic specialist. He was given a cortisone shot on 06-20-06 and was also prescribed physical therapy.(ee shoulder did not get better, surgery on 3/27/07 to make reportable)

EMPLOYEE WAS HAULING 2 INCH PLASTIC PIPE TIED WITH A ROPE APPROXIMATELY 3 FEET FROM THE END. PIPE WAS IN A BIND AGAINST SIDE OF PETTIBONE, WHILE HE WAS HOLDING PIPE WITH RIGHT HAND LOWER THAN WHERE PIPE WAS TIED TO KEEP PIPE FROM SPRINGING BACK TOWARD FACE OR BODY. KNIFE CUT THROUGH ROPE EASIER THAN EXPECTED AND WENT DOWNWARD INTO THE RIGHT HAND REQUIRING 4 STITCHES.

EMPLOYEES WERE GROUTING 30' CABLE BOLTS, PUMPING GROUT TO THE TOP OF THE HOLES USING 7/8 X 3/4 POLY TUBE AND GROUT PUMP. SECTION OF TUBE OUTSIDE OF BOLT HOLE FAILED (RUPTURED) CAUSING GROUT TO SPRAY INTO INJURED EMPLOYEE'S EYES. HE WAS WEARING SAFETY GLASSES AT THE TIME.

Employee was cutting electric ties with pocket knife, knife slipped causing a laceration to left forearm. Employee was taken to company doctor where he received 21 sutures.

EE WAS TRIMMING DOOR PANEL OF TORO LOADER WITH UTILITY KNIFE, WHEN KNIFE PULLED THROUGH THE PANEL CUTTING HIS LEFT HAND BELOW LEFT THUMB. HE WAS TAKEN TO DOCTOR AND RECEIVED 4 SUTURES

Employee was cutting plastic with pocket knife. He thought he heard someone call his name, he looked up and knife slipped and cut left forearm. He was taken to Dr. where he received 3 sutures.

EE WAS DRIVING SCALER TO SHOP. WHEN HE CAME AROUND CURVE, SCALER HIT A ROUGH AREA IN ROAD ROCKING SCALER SIDE TO SIDE HURTING NECK. EE WAS TAKEN TO PAIN MANAGEMENT CENTER ON 10-20-05 WHERE HE RECEIVED 2 CORTIZONE SHOTS. **Per 220060200045: On 1/11/06 ee had the nerve endings in his neck seared to desensitize them to alleviate pain. Ee remains on full duty.**

EMPLOYEE WAS CHECKING THE CU & ZN CIRCUITS WHEN SHE STEPPED ONTO THE FLOOR GRATING. THE GRATING GAVE WAY, FALLING INTO THE BASEMENT & THE EE FELL. EMPLOYEE WENT TO THE DR. ON 4/18/05 & HAD A BONE SCAN ON 5/12/05. WEEK OF 5/16/05 WE WERE NOTIFIED THAT EE HAD TWO UNDERLINE FRACTURES OF #4 & #6 RIBS.

EE WAS CUTTING A ROOFBOLT. WHEN THE BOLT WAS CUT INTO, THE PRESSURE ON IT CUASED IT TO SWING AROUND AND STRIKE THE EE ON THE RIGHT INDEX FINGER. EE WAS GIVEN A METAL SPLINT TO PROTECT FROM FURTHER INJURY. X-RAYS WERE NEGATIVE

EE was in northwest hopper changing a door liner when he slipped and twisted his left ankle. EE was taken to co dr. X-rays were negative but an air splint was given that he may wear to help cushion his walking for a few days.

EE was driving axle pin into John Deere tractor with ball peen hammer. A small piece of steel broke off of hammer and became embedded in right thumb. Piece of steel was surgically removed, received 3 sutures and also an antibiotic was prescribed. EE was released to return to work the same day.

EMPLOYEE WAS WORKING ON ZINK FEED PUMP LINE INSTALLING 3/4" BOLTS USING 1 1/8 WRENCH & SOCKET WITH BREAKER BAR. WHILE TIGHTENING BOLTS, HIS WRIST POPPED AND FINGERS ON RIGHT HAND BECAME NUMB. HE WAS SEEN BY A DOCTOR ON 10/7/04 WHERE HE WAS GIVEN AN INJECTION AND AN ELASTIC WRIST BAND. HE HAD SURGERY ON 11/29/04 FOR THE RIGHT WRIST AND STARTED MISSING TIME.

Employee has been treated for carpal tunnel and received an injection to the wrist on 9/9/04. He has also been wearing a wrist splint. Employee will have surgery on 11/3/04 for left wrist.

Employee was replacing eccentric support plate gaskets under the crusher. He slipped and fell hitting his right shoulder blade and sprained his left wrist.

EE WAS LOOKING UP AT BACK, NOT WATCHING WHERE HE WAS GOING, HIT A MUD PUDDLE AND MUD SPLASHED AROUND HIS GLASSES INTO HIS LEFT EYE. EE WENT TO EMERGENCY ROOM AND WAS PRESCRIBED ANIT-BACTERIAL OINTMENT.
