While an employee was cleaning out a 20" slurry line that was clogged with material. Water pressure and sand blew out of a ball valve that was being used to unclog the material. The water pressure and some steam that came out of the ball valve, landed on the employee causing minor 1st degree burns on his abdomen and leg areas. This also caused a contusion to his left shoulder.

As employee was shoveling material near a conveyor belt a rock flew out of the feed chute and hit the employee in the back of the left side of his head, causing a laceration.

WHILE CUTTING STEEL CABLE ON GROUND, PIECE OF CABLE CAME LOOSE AND STRUCK EE ON HIS LOWER LIP. CAUSING LACERATION TO HIS LIP.

EE WAS CUTTING HOLE IN HOPPER BRACES WITH TORCH WHEN SMALL PIECE OF SLAG HIT RIGHT EYE, ABRADING IT.

CUTTING AT O&K AND HOT SLAGE PARTICLE DROPPED INTO RIGHT EAR.

BLOWING DIRT OFF AIR CONDITIONING COMPRESSOR AND DIRT EMBEDDED IN TIGHT EYE.

DRILLING, FOREIGN BODY CAME UP STRIKING LEFT EYE. PRESCRIPTION EYE DROPS
