Driller was attempting to remove a 4" core barrel from the Sonic drill head when driller left the drill controls & attempted to grab it while it was rotating to finish unthreading/removing it. When driller grabbed the barrel it continued to rotate & the driller began rotating with the core barrel, causing driller to sustain a broken left tibia & fibula, & a dislocated left ankle.

The 417HT was being loaded by the 24S at the 5950 bench of Little Rock. While swinging the bed of the truck the dipper made contact rubbing against the bed of the haul truck. Employee was taken in for observation to the ER and was placed on 1 day of restrictions.

EE operating west starter machine when hanger bar came out. Pushed emergency stop before machine jammed. EE asked coworker help reverse machine. EE put right hand on sheet feed platform to watch. As machine rotation reversed sheet guide pulled EE's right middle finger amputating the tip of middle finger distal phalange from 3/4-to full fingernail up.

EE was cutting weeds when EE stepped back with left leg. EE said felt knee hyperextend and felt a pop. EE was treated on site by First Responder ice applied, but did not want further treatment. On 8/15/2019 EE reported to work and said EE wanted to be taken in.

EE got into the excavator and started it up and turned to the right to place EE's lunch behind EE. The high voltage alarm went off loudly near EE's right ear. EE was seen and released to work with restrictions.

EE was assisting in removal of gantry pin from the 24 shovel. A spacer, along with a hydraulic ram was being used. As pressure was being applied with the ram the spacer came out of place striking the EE in the right shoulder. EE was taken to GRMC x-rays negative for break. Diagnosed with Contusion to right shoulder. Placed on restrictions for 3 days; released back to work.

EE was torquing a nut when he caught his finger between the RAD gun arm and the swivels and the inner part of the wheel. Taken to ER to be checked. Fractured left index finger, 5 sutures and returned to work with restrictions.

EE was attempting to remove the pull cord/rope used to pull spool of the Grinh Winch. When the cord/rope begin to reel in catching the employees left middle finger causing it to strike the side of the winch. EE was wearing gloves. EE was taken to the ER diagnosed with a fracture and released back to work with restrictions. EE was seen on 3/22 by ortho specialist and taken off work.

EE was attempting to manually hold a ground strap that he was trying to bore strap lug to a bigger hole size with a drill press. As the bit made contact it pulled the strap along with his hand towards the side of the rotating drill bit. Removing the thumb nail. EE was taken to the ER and diagnosed with small fracture to his left thumb. EE was released with no restrictions

EE was putting bends in brackets using the chicago break machine. EE pushed the "clamp" button instead of the "inch" button smashing his left pinky. EE was taken to the ER. EE's tips of pinky bone chipped, pinky placed in splint and referred to Ortho. Taken off work.

The employee was directed by an FMI employee to drill a manifold out. The drill given to use was used for vertical drilling, the guys were asked to drill horizontal with it. EE was holding the drilling shaft to keep it stable when his glove was caught on the shaft. The glove caught on the index finger and sprained the left wrist and broke the middle finger.

EE was building up bearing blocks for the shovel. When he finished making a pass, he lifted up his welding hood, when a piece of slag popped up from where he was welding and hit his eye. EE was taken to the front gate, eye flushed and then taken to the doctor. He was given a prescription for his eye and released back to work with no restrictions.

On 10/4 EE was being loaded by the shovel when the shovel bucket made contact with the right rear corner of the bed jarring the EE. EE reported incident to supervisor but did not want to be checked out. EE came to work on 10/5 said his back was sore. EE went to clinic. EE was taken off work.

While using Grader 16M, operator backed over rock causing a back strain.

EE was pushing material in the shovel pit when he hit a rock brining the RTD to a sudden stop. EE twisted his hip when the equipment came to the stop. He was taken into the ER to be checked out. He was prescribed some medication and taken off work.

Employee was helping mechanic replace cutting edge on 966H FEL. Mechanic was inpacting off nuts. The bolt became dislodged from holding socket and started to spin freely. Employee was trying to hold the back of the bolt in socket and a burr caught glove, ripped glove off hand. Employee received 8 stitches to left hand.

Employee was working in an arroyo to do erosion control for tailings dams using a 3/4" drill. The drill bit grabbed in rubber, twisted his hand and broke it. The employee was taken to the doctor shortly after accident happened and placed off work.

Operator of the #5 D8 dozer was pushing at slope material on the 1-1x slope. While backing up to make another push, contact was made with an unseen boulder causing jarring movement in the dozer which caused neck pain.

During EE shift on 4/9 he was jarred while being loaded by the shovel. He called his supervisor to report the incident, but did not want to be taken in. On 4/10 during the shift ee reported to his supervisor that he was experiencing some tenderness and discomfort in his neck. He also statc that he wanted to be taken in to be check. He was taken to the clinic. He wa taken off work

Employee was being loaded when the dipper of the shovel came in contact with the dove tail of the bed. The bed raised and dropped causing him to be jarred. Employee was taken to ER and taken off work.

EE was checking the pump when he noticed that it was not pumping. EE shut the pump down and was getting ready to prime it when the hose came off the pump spraying the EE with hot water on his face, neck and arms. EE was transported to the ER. He was taken off work. On going.

Employee was moving a cathode sample with a jib crane when he twisted his left knee. Employee was taken to the ER & he was given a prescription and taken off work.

Operator of the 17 Scraper was fixing pot holes when he hit a pothole, straining his back. He was taken to the ER, given a prescription and taken off work. Ongoing.

Employee complained of muscle spasm in his back while operating a 637 Cat scraper in tandem with another operator. Employee was transported to the ER where he was checked out and taken off work. Ongoing.

Employee was placing pipe in fusing machine to get ready to cut. The valve that clamps the cylinder on pipe clamped prematurely causing his hand to move the other valve which pinched his right hand.

EMPLOYEE WAS LOADING A SCRAPER IN A BARROW PIT, HE CALLED THE D8 REQUESTING A PUSH. HE STOPPED DUE TO THE HARD SPOTS & THE DOZER HIT HIM, CAUSING HIS HARD HAT TO KISS THE STEERING WHEEL. HE WAS TAKEN IN TO THE E.R. & GIVEN TWO DAYS OFF WORK.

Making connection with drill rig, operator raised pipe with the winch top of pipe made contact with the guill rod and kicked bottom of pipe out. injured ee had his right hand on the bottom of pipe when it kicked out and smashed fingers between pipe ont he truck and the pipe being added.

IN THE PROCESS OF PULLING OUT THE EM-3 DEEP WELL PUMP, SEVERAL EE HAD TWO ELEVATORS(PIPE CHOKERS) TO REMOVE 40' SECTION OF PIPE. EE REACHED UP TO HOLD THE ELEVATOR TO GUIDE IT DOWN TO DETACH IT AND ATTACH TO OTHER ELEVATOR. COLLAR OF THE ELEVATOR BROKE FREE SMASHING EE FINGER. INITIAL DIAGNOSIS BROKEN LEFT 5TH PHALANX DISTAL. RTN TO DR 03/18 AND 03/23, WHERE TAKEN OFF WORK

TRUCK WAS BEING LOADED; SHOVEL OPERATOR NOTICED A LARGE BOULDER AND PLACED SOFT MATERIAL IN BOTTOM BED. WHEN HE LOADED THE BOULDER, THE ANGLE OF THE DIPPER CHANGED, DROPPING THE BOULDER ON THE LEFT REAR CORNER OF THE BED, JARRING THE TRUCK. TRUCK DRIVER WAS DIAGNOSED AT THE ER WITH NECK STRAIN, GIVEN MEDICATIONS, 3 DAYS OFF WORK AND TOLD TO SEE HIS PERSONAL PHYSICIAN FOR A RELEASE.

Employee was operating a dozer and had just finished pushing on the left side of the dump, and felt pain in his neck, and blacked out momentarily. He was taken to the E.R, and Dr stated that it was probably a muscle strain that resulted in nerve impingement. EE was given one day off, and prescription medication.

ASSIGNED TO REPAIR THE DOVE TAIL ON THE 410 HAUL TRUCK, HE SUGGESTED THE BED SHOULD BE TAKEN OFF, BUT IT WAS DECIDED THAT THE WORK COULD BE DONE WITHOUT REMOVING THE BED. HE SPENT THE NEXT 3 WEEKS REPAIRING THE DOVE TAIL. 80% OF THIS TIME WAS SPENT IN A MAN BASKET PERFORMING OVERHEAD CUTTING AND WELDING. HE REPORTED THAT HIS NECK AND RIGHT SHOULDER WERE BOTHERING HIM, STATING

WHILE POPPING STARTER SHEETS ON THE EAST MACHINE, A HANGER BAR HUNG UP. WHEN HE MOVED THE STARTER SHEET TO STOP THE MACHINE WITH AN EMERGENCY STOP, THE CORNER OF THE STARTER SHEET CAUGHT THE E ND OF HIS NOSE, LACERATING IT.

EE WAS CLEANING ROCKS OFF THE ROAD ON THE UPHILLSIDE OF A RAMP WITH A RTD WHEN HE BACKED INTO A TRUCK PASSING BEHIND HIM TRAVELING 5 MPH UP THE RAMP. WHEN THE RTD HIT THE TRUCK, EE WAS JARRED AND FELT PAIN IN HIS LOWER BACK. HE WAS TREATED AND RELEASE BACK TO WORK. HIS IS REQUIRING ONGOING MEDICAL TREATMENT AND PHYSICAL THERAPY.

EE WAS CLEANING THE DASH OF A HUAL TRUCK FROM THE DRIVER'S SEAT AREA. HE REACHED AROUND THE COMPUTER DISPATCH PANEL AND HIS RIGHT HAND WENT INT HE FAR RIGHT AIR VENT. THE VENT WAS MISSING THE COVER AND AS HIS FINGERS WENT INTO THE VENT, HISRIGHT MIDDLE FINGER CONTACTED THE A/C FAN MOTOR BLADE AND HE RECEIVED A LACERATION TO THE FINGER.

WHILE CLEANING ROCKS OFF A HAUL ROAD WITH A RUBBER TIRED DOZER, EMPLOYEE HIT A ROCK BURIED IN THE ROAD. EMPLOYEE WAS TREATED FOR A MUSCLE STRAIN AND RELEASED BACK TO WORK. EMPLOYEE CONTINUED W ORKING AND ON 5-24-01 STARTED LOOSING TIME AS A RESULT OF THIS INJURY.

EE CLAIMS THAT WHILE BEING LOADED BY THE P & H 4100 SHOVEL THAT A PARTICLE OF DUST CAME THROUGH THE OPENDED WINDOW OF THE HAUL TRUCK AND LANDED IN HIS RIGHT EYE.

EMPLOYEE WAS OPERATING THE #4 D-9 DOZER WHEN THE BLADE FELL WHILE BEING LOWERED. HE COMPLAINED OF PAIN IN HIS SHOULDER AREA.
