Employee was preparing a rear bumper on service truck #7 to weld on by grinding with an air grinder and the grinding wheel came apart and struck employee's abdomen cutting open employee's stomach.

EE was assisting with removal of the front hubs on the 988F front end loader. While they were attempting to place the hub on shop floor with use of overhead crane, EE had placed a lifting strap around hub & to the crane hook, EE was trying to force the hub to set down level & right hand middle finger became pinched between strap & a 90 oil fitting on the hub.

While assembling a 12-inch pipe, using an excavator as a crane, the pipe moved bumping EE in the right thigh. EE was taken to the clinic and diagnosed with a bruised right hip and strain of right thigh. Physical therapy was recommended. This was treated as a first aid until 6/15/2016 when the clinic requested that the EE see a doctor for continued pain.

Employee was removing bolts from crusher top shell with air gun and caught his right thumb between the air gun handle and top shell of the crusher.

Employee was using a pneumatic chipping hammer with a chisel head to aid in the removal of an axle on a water truck. The chisel dislodged from the hammer and struck the employee on the upper lip. Sutures closed the laceration.

Employee was operating back hoe moving raw material in front of pit feeder for block wall placement when he felt tightness in his back from bouncing in the seat of the equipment while operating it.

Employee was overhead welding a cross brace in the wet screen when hot slag dripped into the gap of his long sleeve shirt where it buttons at the wrist and it gave him 2 small burns on his left elbow. The small burns developed into an infection which resulted in lost time because employee did not report the incident to his supervisor until 4/14/2009

Employee was drilling a welding lug connected to a 6" piece of cable and a quick connect. The drill press's bit caught the lug and pulled the cable into the drill as well as his hand that was holding the cable. He was correctly not wearing any gloves while operating the press. He was not using the equipment supplied clamping device.

While operating a dozer the employee felt a sharp pain in his right shoulder and shoulder blade. Initially this was first aid only. Employee terminated 11/06/07 due to work force reduction prior to being released to return to work full duty (right shoulder surgery performed 12/13/07).

While using a torch to cut off the swing die bolts, one of the bolts struck out hitting the employee above the left knee then rolling up the left arm.

Employee was cutting steel from loading box when he was struck by the 15" by 50" plate.

Cummulative trauma to tailbone from use track hoe for mud haul.

When the employee was visually inspecting a hoist he motioned to a fellow employee to engage the hoist to see if he could tell what the problem with the hoist was. When the hoist was engaged, a manual handle located nearby struck the employee on the left side of his hard hat protected head.

Adding extra steel length to drill steel - pinched inside of right thumb when coupling.

EE WAS GRINDING WELDS WITH A 7" GRINDER; GRINDING WHEEL STRUCK EE'S LEFT KNEE.

PORT SIDE RAKE MOVED TO STARBOARD SIDE, BUMPED STARBOARD SIDE RAKE & CUT EE'S ANKLE BREAKING HIS LEG

THE EE WAS ARC WELDING WHEN SLAG FELL DOWN INTO THE EE'S BOOT BURNING HIS FOOT.

WHILE WORKING IN SHOP, FOREIGN OBJECT BLEW INTO RIGHT EYE.

ELECTRICIANS WERE CHANGING MOTOR ON JAW AND AS THEY WERE LOWERING IT INTO POSITION EE WAS ASKED IF HIS HANDS WERE CLEAR, HE SAID YES. WHEN THE MOTOR WAS LOWERED DOWN IT CAUGHT THE INSIDE TIP O F HIS FINGER BETWEEN THE MOTOR AND SUPPORT.

WHILE INSTALLING A TAIL PULLEY INJURED GOT HIS RT. MIDDLE FINGER CAUGHT BETWEEN A FRAME THUS LACERATING FINGER.

EE STATES "WHILE CUTTING WITH A WELDER, A PIECE OF STEEL CAME DOWN ON MY LEFT ANKLE."

EMPLOYEE STATES WHILE ON WORK DUTY HE WAS USING A KICIPROCATING SAW CUTTING A PIECE OF PIPE THE SAW JERKED BACK CUTTING HIS PALM OF LEFT HAND

EMPLOYEE STATES"WHILE WORKING ON 305 FEEDER HE STRAINED HIS BACK. EMPLOYEE DID NOT REPORT INJURY TO SUPR. NOR DID HE MARK TIME CARD OF INJURY, CALLED IN ON 4-7-94 TO REPORT ALLEGED INJURY.

EMPLOYEE WAS CHANGING TIE ROD ON FRONT AXLE WHEN HE SLID HIS HAND DOWN TIE ROD, THE TIE ROD WAS HOIT.

WHILE ADJUSTING CR4 CRUSHER CABLE USED IN TURNING CRUSHER BROKE HITTING EMPLOYEE RIGHT HAND AND TOP OF FOREARM.

EMPL WAS OEPRATING A MOTOR GRADER WHEN THE L REAR SET OF WHEELS DROPPED ABOUT 2 FEET STOPPING EQUIPMENT ABRUPTLY THIS STOPPPING MOTION CAUSED HIS L KNEE TO HIT THE CONTROL LEVER BOX EMPL WAS N OT SEEN BY A DR UNTIL 10-13-89

EMPLOYEE WAS REMOVING CABLE FROM SAFTEY HOOK ON DREDGE. DECK CRANE WAS HOLDING WEIGHT. EMPLOYEE HAD CRANE OPERATOR LIFT UP ON CALBLE A LITTLE, THEN EMPLOYEE TRIED TO REMOVE CABLE. WHEN CABL E CAME OFF SAFTEY HOOK, IT FLIPPED BACK HITTING HIM ON THE HARD HAT AND LEFT CHEEK, ON FACE CAUSING LIP LASH TO NECK AND SMALL SCRAPE TO LEFT CHEEK.

EMPLOYEE WAS PUSHING MATERIAL ON THE SURGE PILE WITH THE DOZER.AT ONE POINT HE BACKED OVER A BERM WHICH CAUSED THE DOZER TO SLAM DOWN HARD AND HE FELT A PAIN GO UP HIS BACK,CAUSING LOWER BACK MUSCLE PAIN.

EMPL WAS CUTTING OFF RUSTED NUT WITH WELDING TORCH & HAD FREE HAND BELOIW BOLT. WHEN NUT WAS CUT FREE OF BOLT IT DROPPED INSIDE CUFF OF GLOVE CAUSING SECOND DEGREE BURN TO LEFT WRIST.

EMPL WAS CUTTING WELD LOOSE. THERE WAS TOO MUCH TENSION ON CONVEYOR WHEN WELD WAS CUT CAUSING CONVEYOR TO JUMP INTO AIR. WELDING TORCH WAS PULLED OUT OF EMPL'S HAND & ACROSS HIS ARM CAUSIN G THERMAL BURN TO RIGHT ARM.

EMPLOYEE STATES THAT HIS JOB OF DREDGE OPERATOR HAS CAUSED HIM UNDUE STRESS, HIG BLOOD PRESSURE, DIABETES, ULCERS AND CHEST PAINS.
