During repair to a water pipe located under concrete. A jack hammer was used & connected to the welder/generator for power. The generator was turned off. When they went to use jack hammer again the generator would not start. A forklift and jumper cables were used to crank the welding machine. EE went to remove the jumper cables & EE's hand got caught in the fan.

Employee did not realize the mill had not been flowing and the media had heated up the material in the attritor due to friction, EE then removed a hose without closing the valve which allowed the hot material to escape which caused burns to both hands.

Employee smashed finger in plunger that holds a bag in place.

Using grinder and piece of material went under safety glasses into eye.

Using grinder. Grinding wheel broke. Grinder caught shirt and caused minor cut to stomach.

EE HELPING MAINTANCE PERSON TO REWELD CHUTE EE BURNED BOTH EYES DID NOT FEEL ANY PAIN UNTIL AFTER SHIFT. EE WENT TO ER THAT NIGHT BECAUSE OFPAIN

EESTEPPED BETWEEN TWO 5" HOSES. HOSES DISCHARGED MATERIAL FROM BLOW POT AT 35 PSI, PINCHED TOGETHER ON EMPLOYEE'S RIGHT FOOT AND ANKLE. EMPLOYEE MUST KEEP IT ELEVATED FOR 24 HRS TO REDUCE SWEL LING.

HOSE CAME OFF OF BAGGING MACHINE FILLED AIR WITH DUST. EMPLOYEE HIT HEAD ON FORKLIFT. INJURY WAS TO EYE AREA, BLACKED EYE. EE WENT TO DR., WAS TOLD NOT TO WORRY ABOUT IT, COME BACK IN TWO WEEK S.
