While cleaning the clean air side of Silo 51 air slide, an employee found the slide choked and not flowing. EE used a short air lance to clear cement and restore flow. Cement blew back into EE's face, entering EE's right eye under EE's safety glasses. EE was sent to an eye doctor; no damage was found and antibiotic eye drops were prescribed. No lost time resulted. Returned to job.

Employee was guiding excess chain from electric hoist around the crane drum, the weight of the chain caused excess chain to run out pulling employee's left hand into the railing. Left pinky finger was cut requiring stitches. Restricted duty only no lost time.
WHILE USING JACKHAMMER TO DISLODGE LARGE LUMP, THE JACKHAMMER BECAME LODGED IN LUMP. WHILE PULLING TO FREE THE JACKHAMMER, EE PULLED HIS SHOULDER, CAUSING DISCOMFORT. EE GAVE TIME TO SEE IF PAIN WOULD ALLEVIATE BY ITSELF, BUT PAIN CONTINUED. EE WENT TO DOCTORS ON 3/4/05 FOR FIRST VISIT. SURGERY SCHEDULED FOR 4/27/05.
TRYING TO EMPTY KILN 2 SETTLING TANK ON BURNER FLOOR, WHEN USING AIR HOSE, DUST SHOT OUT.
EE CAME DOWN THE STEPS TO PUT OUT A FIRE & ON THE FLOOR LEVEL WAS HOT KILN FEED, & HE STEPPED IN IT.

EE WAS STANDING TO CLOSE TO AREA WHERE CRANE WAS LIFTING GEAR TO EASE TENSION. WHEN TENSION RELEASED, A RUBBER SOFTENER, WEIGHING APPROX 40 LBS CAME LOOSE AND FELL ON EMPLOYEE'S HEAD. EE WAS W EARING A HARD HAT.
