Miner was emptying dust collector, material flushed going in both eyes under glasses. Miner sent to emergency room, both eyes flushed out, eyes drops prescribe making this a reportable injury. Miner was released to full duty on 11/7/2025 after follow up appointment.

Miner was operating skid steer, backed into hole in roadway jarring the machine, EE's right elbow struck the safety bar and right shoulder contacted the cab enclosure causing a cervical strain and shoulder sprain. Injury became reportable on 10/7/2024 when employee received therapy for injury and restrictions.

Miner was installing bolt on frame of conveyor holding head of bolt with right hand. As miner ran nut to tighten with an impact driver, the washer on the head of the bolt pinched miner's right thumb tip against the conveyor frame causing laceration requiring stitches to close.

Employee was walking in stockpile area, slipped on ice covered with dust falling and striking right shoulder on ground. After continued pain in shoulder EE was sent for evaluation on 1/31/2022, MRI revealed torn rotator cuff in right shoulder. Injury became reportable on 2/18/2022 when surgery was performed to repair torn rotator cuff in right shoulder.

Employee was removing a lump of material from an air slide gate, EE lacerated EE's right forearm on the edge of the steel gate. Required stitches to close wound. All energy control procedures were followed.

Employee was walking on dirt roadway, stepped in frozen rut in roadway and twisted right ankle spraining the ankle.

Miner was found lying on ground left side of 990H Loader, was seen minutes before on railed platform at rear of Loader. Miner was transported by ambulance to hospital, responsive at time of transport, trying to talk, attempting to sit up, remains in hospital for treatment for injuries.

Employee was operating rail car mover with cars attached, could not stop mover or cars, struck rail car parked on siding. Extensive damage to mover and injuries to employee. MSHA was notified immediately, investigation in progress.

Miner was walking on roadway at cooler, felt something go in right eye, was wearing all required PPE including safety glasses, had no issues with eye rest of work day, eye became irritated during the night, reported incident on 4/24/2018, sent to eye specialist, metal flake removed from right eye, written out of work for 4/24/2018 and 4/25/2018. Full duty 4/26/2018

Person had a seizure due to drugs in body. Medical emergency not related to mining activities.

Employee was blowing duct work down through 1 inch port, dust blew out of port and under face shield contacting skin on face causing alkali/lime reactive burns to face.

Mechanic was checking flasher for brake lights on pick up truck. When EE exited from under dashboard EE lacerated right forearm on parking brake pedal. Stitches required to close laceration.

Employee was blowing down packing machine, cement blew up under safety goggles into both eyes, treated for chemical burns to both eyes, out of work 2 days.

Supervisor was walking at ground level of kiln pier, EE slipped on unpaved surface twisting right knee. Slight tear to right ACL occurred. This accident became reportable on 05/31/2016 when employee had surgery on right knee.

Employee missed last step on ladder when exiting loader. Fell to ground landing on right elbow and shoulder, contusion of shoulder.

employee was using hand grinder, grinder slipped lacerating right index finger.

Employee closed pack house freight elevator door on left hand lacerating ring finger. Stitches required to close wound.

Employee was opening hatch on top of rail car, car moved unexpectedly and employee slipped off side. Employee was attached by harness to a retractable lanyard, employee was swung against car fracturing bone in left wrist.

Employee pulled water lance from port in tower, felt pain in right arm. Diagnosis was slight tear to bicep muscle, employee continued to fully perform his job duties until March 17 surgery, when this accident became reportable.

Employee was walking down steps on west side of cooler, he tripped on step and felt pain in his right knee, diagnosed with knee strain. Employee was able to fully perform job functions and returned to full duty. Pain continued in knee, sent for MRI. Employee was sent for surgery December 30, 2010, arthritis aggravated by trip on the step.

Employee was walking on roadway, stepped off roadway and twisted right knee. Employee was sent for evaluation, he was able to fully perform his job until 08/24/2010 when pain increased in knee and he was sent for MRI. Diagnosis was torn meniscus with no work restrictions placed on employee. This injury was deemed questionable and has now been deemed work related and reportable.

Employee walking out of control room front door, opened door and wind blew door out of hand and sharp edges on handle of door cut right hand index finger

EE reported to supervisor on 1/19/2010 that his left knee was swollen, the employee did not report any accident, but felt it was due to excessive walking and stair climbing. On 01/25/2010 the employee was sent to Healthworks for evaluation, diagnosis was left hip and thigh strain due to excessive use. Employee was placed on restricted duty on 1/25/2010.

Employee was on the second step of a step platform 16 inches off the ground, he turned to lift down a section of 3 inch pipe that had been cut away from the main line. As he turned his right foot stepped on his left foot causing him to slip off the step, his left foot caught under the step twisting his left leg. He felt pain in his left shin, transported to hospital.

Employee's left hand was pinched between a vertical brace and duct.

EE struck face against an elevator support causing a small cut to his face on his right eyebrow and right cheek.

Employee was poking out a coal choke in the chute of a coal bunker, he pulled the bar he was using to remove the choke towards him and felt a sharp pain in his lower right back. Employee was transported to the hospital, he has a muscle strain and was placed on restricted duty.

Dust

Employee was walking through Packhouse Warehouse to inspect a sump pump, he stepped in a cracked portion of concrete floor and twisted his right ankle. The broken concrete was 10 inches by 12 inches and 1 inch deep. Employee sustained sprained right ankle.

Foreign body in to right eye

EE was grinding while wearing proper PPE. Dust flew in from behind face shield and into left eye.

Employee was walking through area at new kiln, when he twisted his left ankle and foot. Medical evaluation revealed a fractured bone in foot. A cast was applied and work restrictions mandated. The employee is a manager and is able to perform his job duties within the restrictions.

Cutting with a Metabo when the guard came loose and started spinning causing a laceration to left index finger.

Struck by duct work being moved. Final investigation still pending.

Employee hit right elbow against handrail and was treated with first aid. Condition did not improve. 8/28/2008 - Employee was taken to medical facility for treatment.

Employee was fueling the road sweeper, he stepped down off of platform at fuel tank and twisted right knee. Diagnosis by company doctor was stress fracture to right knee. This injury was contested by worker's comp. As of this date it has been verified as a work related injury.

Welding; unknown foreign body to right eye; nothing removed.

Employee climbed onto platform at shrink wrap machine on the Packing Line to pull jammed plastic off of machine. As he stepped down from the platform to the floor he slipped and struck the floor with his left elbow. Injury sustained was a fractured left elbow.

Stepped off crane pad, lost footing and felt pain to right knee.

Employee was holding concrete pipe, employee was splashed by the concrete presure and foreign object got in left eye.

Manlift basket became caught on a bolt. Upon releasing the basket, the basket moved quickly causing the employee to hit his left hip area against the control panel located inside the basket.

Unloading hand rails off the back of a truck; rail hit a piece of wood that was nailed to the back of the truck causing EE to twist back.

Stepped too high- stepped to an elevation of approximately 24" above ground and felt a pain in right lower leg.

Employee was pulling nail from a board, hammer slipped and hit right knee.

Employee jumped from 3' elevation to the ground and stepped on a rock and twisted right ankle.

Employee tripped on welding lead and twisted both knees, diagnosis-strain. Employee returned to work full duty, 11/26/2007 MRI showed meniscus tears in both knees. On 12/14/2007 employee had surgery on left knee. Injury had been contested as a pre-existing injury non work related. it became work related as of this date.

Employee was guiding excess chain from electric hoist around the crane drum, the weight of the chain caused excess chain to run out pulling employee's left hand into the railing. Left pinky finger was cut requiring stitches. Restricted duty only no lost time.

Employee stepped down off platform at Kiln 2 Vent Fan, twisted knee and felt pain. He reported incident but refused medical treatment on this date. Follow up showed torn left medial meniscas in left knee. Surgery on 11/02/2006 made this a reportable lost time accident as of this date.

Employee was exiting Finish Mill # 1. As he stepped down onto platform he twisted his knee. Employee who is a supervisor went to company doctor who released him for work, follow up showed torn medial meniscas in right knee. Employee had surgery on 11/02/2006 making this a reportable lost time accident

The employee was using an electric pallet mover to move empty cement bags. The pallet mover became stuck and when he pushed on the handle, he felt a sharp pain in his groin area. He was sent to the doctor and was diagnosed with a hernia. Surgery was performed on 08/10/2006.

Employee was holding steel plate in place when it slipped causing him to twist his back. Sent to hospital, released with a back strain. No lost time, wages or change of job at this time. A later MRI showed a disc problem which was operated on June 2; on this date it became a reportable and lost time accident.

Using bar to move slide on stone belt, bar slipped and employee experienced pain in right shoulder.

BLOCKING UP BIG JOE TO REPAIR WHEEL-SLIPPED OFF PINCH BAR, GOT FINGER CAUGHT BETWEEN BLOCK & FRAME.

TWISTED RIGHT KNEE WHILE GOING UNDER KILN 1 TO CHECK OIL BEARINGS.

WAS HOOKING DRAIN LINE AND FINGER GOT PINCHED BETWEEN PIPE AND HANDRAIL

WHILE USING JACKHAMMER TO DISLODGE LARGE LUMP, THE JACKHAMMER BECAME LODGED IN LUMP. WHILE PULLING TO FREE THE JACKHAMMER, EE PULLED HIS SHOULDER, CAUSING DISCOMFORT. EE GAVE TIME TO SEE IF PAIN WOULD ALLEVIATE BY ITSELF, BUT PAIN CONTINUED. EE WENT TO DOCTORS ON 3/4/05 FOR FIRST VISIT. SURGERY SCHEDULED FOR 4/27/05.

EE WAS LIFTING A HYDRAULIC CYLINDER INTO COOLER AND MOUNTING IT TO UNDERGRATE. HE WAS HOPING THE PAIN WOULD SUBSIDE. HE FINALLY WENT TO THE DOCTOR ON 4/25/05. SURGERY SCHEDULED FOR 6/16/05.

While lifting a suspension bar to install in the hammer mill, ee felt something pull, had gone to Healthworks 11-17-04, returned 12-02 to Dr, determined surgery was needed. Surgery scheduled for 12-8-04.

TRYING TO EMPTY KILN 2 SETTLING TANK ON BURNER FLOOR, WHEN USING AIR HOSE, DUST SHOT OUT.

WHILE LETTING CHAIN DOWN FROM SHOULDER, EXTRA LOOP OF CHAIN SLIPPED OFF AND CAUGHT RIGHT THUMB, BENDING IT OUTWARD. EE FELT HE COULD TAKE CARE OF THUMB. WENT TO DOCTOR 5/27. SURGERY ON 7/17/04.

HE WAS CUTTING A LID OFF A PLASTIC 5 GALLON BUCKET. HIS HAND SLIPPED. THE LEFT PALM WAS CUT WITH THE UTILITY KNIFE.

EE WAS TAKING A BEARING RACE OFF, AND THE RACE BROKE INTO PIECES. A PIECE OF RACE FLEW AND HIT HIM IN THE CHEEK. THE PIECE WAS LODGED IN HIS LEFT CHEEK.

WAS ON TOP OF COAL BELT FRAME WORKING ON POKING OUT COAL TANK CHOKE- WHEN DONE HE CAME DOWN & HIT KNEE ON FRAMEWORK. THIS WAS NOT REPORTED IMMEDIATELY, SOUGHT MEDICAL ATTN. ON JUNE 3, 2004.

EE WAS DUMPING VINSOL INTO FEED CHUTE OF MILL. MATERIAL WAS BLOWN OUT OF CHUTE AND COVERED HIS ARMS AND FACE.

WAS STEPPING OVER THE HWF CONTAINMENT WALL ON # 2 BURNER FLLOR. RIGHT FOOT CAUGHT THE LIP ON THE12" STEEL WALL CAUSING ME TO TRIP. WHEN I FELL FLAT ON MY PALMS FRACTURING BOTH ELBOWS.

AT 3:34 PM ON AUGUST 25 A UTILITY POWER INTERUPTION CAUSED THE COAL MILL TO SHUT DOWN. AT 3:45 PM THE MILL WAS RESTARTED. AT 4:03 PM THE PRESSURE RELIEF VALVE BLEW OFF THE MILL DAMAGING TWO THERMOCOUPLES. VALVE AND THE THERMOCOUPLES WERE REPLACED AND THE MILL INSPECTED AND RESTARTED WITHOUT INCIDENT.

WAS HITTING ON DISCHARGE DUST COLLECTOR PIPE USING A SLEDGEHAMMER. THIS WAS NOT REPORTED DUE TO MEDICAL TREATMENT DID NOT OCCUR UNTIL 8/28/03.

REMOVING SCRAPER AND PLOWS WITH SLUGGING WRENCH AND HAMMER GLANCED OFF AND HIT THUMB.

HE DROPPED COVER LID FROM #2 KILN COOLER DRAG ONTO FOOT.

EE WAS CLIMBING INTO THE QUARRY HAUL TRUCK WHEN HIS FOOT SLIPPED OFF THE LADDER, DUE TO MUDDY CONDITIONS, AND HE INJURED HIS ANKLE.

EMPLOYEE WAS GOING FOR KEYS TO OPERATOR THE SERRPER. WHEN HE PLANTED HIS FOOT AND TURNED HIS KNEE POPPED AND THE KNEE SWELLED. HE ICED IT THE DAY IT HAPPENED, BUT WENT TO HEALTHWORK ON 10/10/0 2 FOR X-RAY.

EE WAS HAVING A HARD TIME WITH THE GEAR SHIFT IN THE TRUCK SHE DRIVES. A REPAIR ORDER WAS ISSUED TO GET IT FIXED BUT THE LEASING CO WAS ONLY ABLE TO FIX HALF THE PROBLEM & ORDERED THE PART FOR THE OTHER PART. THE LEASING CO NEVER CALLED THAT THE PART CAME IN & IN THE MEANTIME SHE HAD AN ACCIDENT WITH THE TRUCK (NOT HER FAULT) UPON CALLING THE LEASING CO THEY TOOK THE TRUCK IN AN

EMPLOYEE WAS CHANGING A SCREEN. THE SCREEN DROPPED AND CAUGHT HIS HAND BETWEEN THE SCREEN & THE CATWALK.

EMPLOYEE WAS WALKING IN TRANSFER TUNNEL TO TRANSFER CEMENT AND TWISTED HIS ANKLE ON GRATING.

EE WAS STEPPING OVER PIPE ON TOP OF TAMKER AT BULK LOADING STATION TO DO A COLOR CHECK WHEN HE SLIPPED ON THE PIPE & TWISTED HIS KNEE.

EMPLOYEE WAS PULLING UP CHAIN ON TABLE FOR PACKING MACHINE. CHAIN WAS OILED FROM FACTORY AND SLIPPED OUT OF HIS HAND CATCHING FINGER BETWEEN CHAIN AND SPROCKET. SEVERED FINGER AT FIRST JOINT.

EE LOST HIS FOOTING WHILE DESCENDING A LADDER AND HELD BODY WEIGHT WITH ARMS ONLY. RIGHT SHOULDER TOOK THE MOST WEIGHT.

EMPLOYEE WAS PULLING A HOSE FULL OF DUST AND HEAVY, TRYING TO BLOW IT OUT. HIS BACK WAS SORE AND A WEEK LATER HE FELT PAIN AND COULDN'T WALK STRAIGHT.

EE CAME DOWN THE STEPS TO PUT OUT A FIRE & ON THE FLOOR LEVEL WAS HOT KILN FEED, & HE STEPPED IN IT.

SUPERVISOR CAME DOWN THE STEPS TO PUT OUT A FIRE & ON THE FLOOR LEVEL WAS HOT KILN FEED, & HE STEPPED IN IT.

EE WAS REMOVING BOLTS ON N-S SPEED REDUCER WHEN BOLT BROKE AND HIS HAND SLIPPED, CAUSING HIS KNUCKLE'S TO GET SKINNED.

EE WAS INSTALLING KILN HOOD. WHILE PUSHING HOOD IN HE STRAINED HIS LEFT SHOULDER.

SUPERVISOR WAS DESCENDING LADDER ON A LOADER, HELOST HIS FOOTING ON THE BOTTOM STEP AND FELL BACKWARDS USING HIS ARM TO BREAK THE FALL.

EMPLOYEE STEPPED ON MACHINERY TO CHECK IT OUT; WHEN STEPPING DOWN HE INVERTED HIS ANKLE AND FELL TO THE GROUND.

EMPLOYEE WAS REPAIRING AND REPLACING GRATE SUPPORT. WHEN LIFTING GRATE SUPPORT HE FELT PAIN IN LEFT SHOULDER AREA.

EMPLOYEE WAS WALKING UNDER FLEX CONVEYER, WITHOUT HELMET, STOOD UP TOO SOON AND CUT HEAD.

EE HAD AN AUDIOGRAM AND EVALUATION SONE BY THE SAFETY SUPERVISOR AND THE COMPANY DOCTOR. BOTHED HEARING LOSS DUE TO WORK RELATED CONDITIONS. EE IS BEING REFERRED TO AN AUDIOLOGIST.

WHILE INSPECTING THE JOB SITE FOR INSTALLATION PROBLEMS, EE SLIPPED ON LOOSE GRAVEL AND JAMBED HIS RIGHT THUMB.

EE WAS GOING UP DUST TANK LADDER AND SLIPPED, OVERTREADING HIS ANKLE. THEN COMING DOWN HE STEPPED ON ROCK AND HURT HIS ANKLE AGAIN.

EMPLOYER WAS DISMOUNTING A LOADER WHEN HE STEPPED INTO A RUT AND OVERTREAD HIS ANKLE.

EMPLOYEE WAS CUTTING BELTING FOR SKIRTING ON C-3.

EE WAS CLIMBING OUT OF HATCH DOOR & PULLED A MUSCLE IN LEFT SHOULDER.

EE WAS TIGHTENING BOLT ON COAL MILL ROLL WHEN THE WRENCH SLIPPED AND HIS ELBOW STRUCK METAL GRATE.

EMPLOYEE WAS WORKING ON THE NEW MILL CLINKER CHUTE, WHILE LIFTING THE CHUTE HE FELT PAIN IN HIS RIGHT SHOULDER.

EMPLOYEE WAS REPLACING DRIVE MOTOR, WHILE LIFTING MOTOR WITH ANOTHER EMPLOYEE, INJURED FELT PAIN IN GROIN AREA.

EE WAS STANDING ON CONE CRUSHER & STRETCHED OVERTO FRAME WORK OF BUILDING WHILE STEPPING UPWARD HE FELT A POP IN HIS UPPER THIGH OF RIGHT LEG

CONTRACTOR INSPECTION OF CLINKER COOLER, ENTERING THE MANHOLE RIGHT FOOT AND BODY INSIDE, LEFT FOOT CAUGHT EDGE OF MANHOLE WHEN FOOT WAS FREED, THE BOTTOM WAS FURTHER ANTICIPATED TO THE FLOOR. RIGHT FOOT SLIPPED BEFRORE LEFT FOOT GOT THROUGHHOLE. MAN WAS CAUGHT BEFORE HE FELL, HE TWISTED HIS BODY.

EMPLOYEE COMPLAINED OF RINGING IN HIS EARS. HE WAS SENT TO THE COMPANY AUDIOLOGIST FOR ELEVATION. T WAS DETERMINED HE HAD HIGH FREQUENCY HEARING LOSS.

EE WAS USING A WRECH TO TIGHTEN BOLT WHEN THE WRENCH SLIPPED OFF BOLT AND HIS FINGER GOT CUT ON A METAL FRAME.

EE OVER TREAD HIS RIGHT ANKLE BY PARTIALLY STEPPING ON A 2" EDGE THAT HE DIDN'T SEE.

EMPLOYEE LOS THIS GRIP CLIMBING THE LADDER INTO THE TRUCK AND FELL TO THE GROUND ON HIS FEET. THEN TWISTED INTO THE TIRE OF TRUCK.

EMPLOYEE WAS REMOVING STEEL LINING IN THE MILLROOM AND IT HIT HIS RIGHT ANKLE.

EMPLOYEE WAS WRENCHING BOLTS LOOSE IN THE NEW FINISH MILL WHEN HE FELT PAIN IN HIS LEFT BICEP AREA.

EE WAS STANDING TO CLOSE TO AREA WHERE CRANE WAS LIFTING GEAR TO EASE TENSION. WHEN TENSION RELEASED, A RUBBER SOFTENER, WEIGHING APPROX 40 LBS CAME LOOSE AND FELL ON EMPLOYEE'S HEAD. EE WAS W EARING A HARD HAT.

EE WAS REPAIRING BROKEN E-BAR ON CRANE BUCKET AND GRABBED HOLD OF THE BUCKET ARM TO BRACE HIMSELF AND SOMETHING PULLED IN HIS NECK AND SHOULDER.

EE WAS CARRYING PARTS FROM #1 KILN COOLER AREA AND STEPPED IN A SMALL HOLE AND OVER TREADED HIS LEFT ANKLE.

EE WAS DOING HIS NORMAL JOB OF LIFTING PALLETS AND HE FELT PAIN.

EE WAS UNCHOKING CYCLONE HOPPER WHEN DUST OVERFLOWED FROM THE CONVWYOR CAUSING PARTICLES TO BE AIRBORNE AND BELIEVED TO BE THE CAUSE OF THE RASH.

SUPERVISOR WAS GETTING OUT OF PICKUP TRUCK AND SLIPPED ON ICE THAT WAS SNOW COVERED.

SUPERVISOR WAS EXAMINING STONE STORAGE AREA WHEN THE PLASTIC DUST CONTAINMENT STRIPS HIT HIM IN THE FACE DUE TO THE WINDY WEATHER CONDITIONS.
