EE was welding/torching when piece of hot slag/steel fell into tongue of boot and burned through boot, sock and onto skin. First Aid applied - ee sought medical treatment on 8/7 due to increasing pain. wound was infected resulting in prescribed medication and time off work.

EE was welding - grabbed wrong shield - received arc flash/burn to eyes

Employee changed head/body position resulting in sensor on welding hood becoming blocked by a beam causing the shield to clear while employee was welding resulting in arc/welders flash.

Injured was operating dozer when EE backed over windrow which jarred EE's back. Injured requested medical treatment on 7/13 and was given work restrictions.

Great Lakes Trader employee was standing near mooring wire preparing to move wire off of mooring bit. When a second Trader employee started the winch, the mooring wire struck injured in the stomach, knocking them to the ground. When they fell to the ground, hard hat fell of and they struck head on the ground.

Employee was cutting a section of channel from conveyor. Piece fell sooner than anticipated and caught finger between channel and conveyor pan.

Employee was making an initial pass with the horizontal boring machine to smooth the inner surface of a poured lead Babbitt. Employee tried to pull excess gasket material from mold when the cutting edge of the bore caught left ring finger, just above the knuckle causing severe tissue trauma.

While backing a dozer down a stockpile, EE went over a lip in the pile which shook the dozer. This in turn caused some pain in EE's neck. After continued discomfort, EE went for MRI on 9/28/2017. EE was given restrictions at that time.

Employee was operating a dozer on stockpile, pushing material to load a boat. Toward the end of the shift, employee reported that their back was hurting. Employee took Saturday off and went to the doctor on Monday.

Employee was digging in a trench. Loose piece of dirt became dislodged and struck employee knocking ee against trench wall. Impact caused injury to torso causing bruising and possible hernia.

Using an air chisel to remove broken bolts. Chisel became stuck. Used another chisel to remove stuck chisel. Piece of metal struck arm resulting in medical treatment.

EE was operating drill rig and wanted to check the level of heat coming out of vent system. The cab heater was not blowing warm air. He reached up to feel the air coming out of the heater duct and his fingers contacted the squirrel type fan. The fan cut three of his fingers on his left hand near the ends. His index finger was nicked and the other two required 12 stitches.

Dozer operator was cleaing around tunnel on large stone, and also later loaded a boat from the same pile. This required alot of backing and postioning of the dozer and the dozer does not provide a "smooth" ride. Employee felt lower back pain when he got home that day.

On 7/1 employee welded a pin keeper on a truck when a piece of slag went down his shirt sleeve and burned his elbow in area about the size of a pencil end. The burn became infected. Notified management on 7/11. Missed work as of 7/9; returned 7/15. Use of short-cuff leather gloves instead of long welding gloves in a overhead position allowed slag to enter the shirt.

Employee was lifting a steel screen with a hoist when the screen dropped on top of their foot.

Employee wearing gloves, safety glasses with side shields and face shield was cutting 3" angle iron with a 4.5" hand held grinder equipped with a 4.5" "Razorblade 27" cutting tool. It bound up and kicked back striking employee in the face (Right cheek area).

While cutting heads off of bolts with plasma cutter, EE was using left hand to secure bolt, then plasma cutter came across left hand.

EE was grinding overhead wearing a face shield, when he finished, he removed the shield and was attempting to "Dust-off" the loose debree and got some dust in his right eye.

While working on 200 bonnet with co-worker, EE was burned by co-worker. Both employees were kneeling down in close proximity, when co-worker turned with a lit torch and passed near EE's left hand and wrist, causing a burn.

The repairman was cutting steel with a torch when the steel plate fell off one end of the stand and the other end of the plate raised striking him in the head. The injured received a small cut on the side of his head requiring 2 sutures to repair.

Employee was using a water hose to clean under a conveyor belt. As he was hosing he hit some hard-packed material, which caused the water and mud to spray back into his face. EE was wearing saftey glasses at the time of the accident.

The repairman was lifting a conveyor idler with an electric chainfall when the idler struck his left hand finger, causing a fracture.

The mill repairman was rebuilding screens on the mill 4th floor when he burned off a bolt causing the lower screen deck to fall six inches onto his left foot. His foot was badly bruised and this incident resulted in lost time.

Employee was using a hog burner to clear a frozen chunk from a chute when the chunk broke free and pinned the employees hand against the rail.

Employee was drilling a hole in a 2" x 2" x 3/8" x 10' piece of angle iron. The end of the angle iron fell off the stand it was resting on and pinched employee's finger between the angle iron and the drill press base.

As the operator drove his dozer off the stockpile, the dozer jarred his back as it fell off a three foot high ledge. The dozer operator went to the doctor on 11/09/04 and started therapy on 11/11/04.

The machinist apprentice was using a grinder when he got his finger caught between the grinding wheel and the guard.

THE REPAIRMAN WAS WORKING WITH A MILLWRIGHT INSTALLING A SKIRTBOARD. AFTER THE MILLWRIGHT FINISHED CUTTING A STEEL ANGLE WITH A CUTTING TORCH, HE TURNED WITH THE LIT TORCH AND PASSED IT OVER THE REPAIRMAN'S ARM, CAUSING A BURN.

WHILE USING THE BIG SPLINE DRIVE IMPACT TO TAKE CUTTING EDGE BOLTS OUT OF 105 LOADER EE ATTEMPTED TO HOLD HEAD OF BOLT INTO POCKET. BOLT SPUN WHEN IMPACTED CAUSING INJURY TO FINGER.

THE EE WAS GRINDING THE BEVEL STRAP THAT WAS TO BE WELDED ON THE 615 CRANE MOTOR MOUNT. WHILE GRINDING, A PARTICLE BECAME LODGED IN HIS LEFT EYE. A DOCTOR REMOVED THE PARTICLE AND PRESCRIBED P RESCRIPTION MEDICATION.

THE INJURED WENT INTO THE MILL BUILDING TO CHECKTHE VSI CRUSHER MOTOR BECAUSE IT WAS SMOKING. HE DID NOT SEE FIRE AND THE POWER WAS OFF SO HE LEFT THE BUILDING TO LET THE SMOKE CLEAR. HE LATER FELT SICK AND WENT TO THE DOCTOR FOR SMOKE INHALATION.

THE INJURED WAS BURNING AND GRINDING STEEL IN THE MILL WHEN HE FELT AN EYE IRRITATION. ON 3/5/01HE WENT TO THE DR. WHO FOUND A SCRATCH ON HIS RIGHT EYE.

WHILE AT WELDING SCHOOL AT ALPENA COMMUNITY COLLEGE, A SMALL PIECE OF STEEL FLOATING IN THE AIR GOT IN MY RIGHT EYE.

INJURED WAS GRINDING STEEL AND GOT A SPARK IN HIS RIGHT EYE.

GRINDING ON ANGLE IRON, OTHERS NEARBY WELDING, FELT SENSATION IN EYE, THOUGHT IT WAS WELDING FLASH, BUT NEXT DAY STILL HURT, WENT TO DR AND HAD STEEL REMOVED.

FLASH BURN AND METAL PIECES IN EYES FROM WIND BLOWING, WELDING INSIDE JAW CRUSHER.

WHILE BURNING A HOLE W/TORCH, HOT SLAG WENT IN EAR.

BENDING DOWN TO WELD AND TURNED TO GET INTO ANOTHER POSITION AND BACK VENT OUT.
