Die grinder sparks ignited cleaner residue during mounting hole alignment on a Hitachi EX5600 which caused hand burns.

While performing drilling operations, drill pipe fell and struck the operators right hand causing injury to the right thumb which required surgery.

Grinder slipped while in use, causing laceration to leg.

While using a handheld cutting wheel, the Technician lost grip and the cutting wheel struck their hand causing laceration to their right hand.

Mechanic received a laceration while removing a bolt.

While attempting to unplug a feeder, the jackhammer being used broke through material causing the jackhammer to drop and pinch employee's finger.

Employee was moving the core tube from the drill rods to the table. As they approached the table, the core tube released unexpectedly dropping onto their hand on the table. Equipment malfunction contributed to the core tube releasing. Employee left site to seek medical attention, and was promptly released after examination.

Employee was testing a limit function on a drill and got EE's leg pinned between the rod support and drill steel. Employees knee became swollen. EE was seen by urgent care and discharged with instructions to rest it and ice it for 3 days.

While attempting to place a bearing onto a shaft using a hydraulic press the shaft slipped causing it to make contact with the guard. The guard then struck the employee.

While attempting to make a lift a picking eye broke causing the lifting chain to strike employees arm

Technician was tightening bolts on a final drive. The technician put the 1" impact wrench on a bolt to finish tightening it. The impact wrench was in reverse, causing the bolt to back out and the technician's right pinky finger to be caught between the impact wrench and final drive.

Pull starting a diesel water pump when the motor locked on pull back.

No easily verifiable conditions contributed to the accident. Employee reported wearing all proper PPE throughout his shift.

While using a hydraulic torque wrench employee had the tip of their left thumb smashed.

While a hydraulic ram was being retracted the employee's left thumb was pinched between the ram head and its main housing.

He did not have both hands on the grinder when using it with a cut off wheel. The grinder kicked back and cut his left thumb.

As driller was pulling sample, sample became caught and stopped. Driller then continued on to next hole. Driller then coughed and felt a twinge in his back. Called shifter and was taken to ER.

Loss of power and failure of machines ability to stop, also the operators steering away from a paralell course with the incline of the slope.

While using a 4" grinder, the grinder slipped and wheel ran across left thumb

Employee's hand was caught in main line cable and pulled into winch drum, which pinched fingers of right hand.

Employee was on the staircase leading to the basement where the end of a clogged sump hose was located. Compressed air was turned on to remove the blockage. When the obstruction gave it caused the hose to whip. Employee was struck on the left elbow causing a fracture.

Ice may have built up in the hose due to extreme temperature. Although whip checks were installed on both ends, there were no guards keeping the middle portion of the hose from moving in a violent manner.

Shovel Operator was cleaning the pit floor when the bucket of the shovel hung up on a rock causing the shovel to lunge forward. The employee was jarred in the seat when this occurred and sustained a strain to the lower back. Employee was diagnosed with a LS strain and placed on modified duty.

Employee was cutting an "I" beam with a torch. The cut piece was not secured and fell pinning the employees right ring finger against another "I" beam. This caused a fracture to the finger.

Employee sustained an injury on 8/05/06 while performing crushing and pulverizing tasks. Employee was diagnosed with tedonitis and returned to work with no restrictions. On 9/17/06 employee reaggravated the injury and was placed on resricted activity.

Employee was utilizing a 4" grinder with buffing wheel to clean track frame on a shovel. While perfoming this task the employee was wearing safety glasses and a face sheild. The employee reported the incident the following day.

Employee working on impact wrench when it slipped out of hands and fell on his foot. Reportable due to restricted work day.

An employee was air arcing the underside of the LR-6 bucket, and noticed an unusual dark spot on the upper lefthand side of the lens. Checked the hood for burning or unusual smells, found nothing and resumed work. As the hood was put down the glass shattered, some pieces falling and burning the employees neck and dropping further slightly burning the employees stomach.

Cable became entrapped and broke. Hoisting plug struck employee in the face.

An employee was operating SH-4 with the cab rocking and shaking from loose bolts in the "dog house" below the cab. After a couple of hours of being jolted in the seat, his neck was feeling sore at fuel time and he shut the machine down.

An employee left work without reporting any problems. The supervisor did talk directly to him when he collected time cards at the end of shift, no mention of any flash exposure or eye problems.The employee called the supervisors office and talked to the lead hand, reporting that he had gone to the doctor and had been treated for flash burn.

An employee was removing paper towels from console above operators compartment and inadvertantly placed fingers up in heater fan area cutting the pad on left ring finger.

Ee was cutting a concrete wall with a concrete chainsaw. The saw kicked back cutting him on the chin.

Employee was removing a bushing out of pin bore with air hammer. Bushing broke loose and hit him in the face cutting open lip and breaking tooth.

EE WAS OPERATING A DION DOZER RIPPING A LARGE KNOLL MOUND, CONSTANTLY LOOKING BEHIND THE DOZERCAUSING HIS NECK MUSCLES TO STRAIN & SPASM

EE WAS GRINDING FRAME SEGMENTS WITH A 9" GRINDERDURING THE GRINDING A METAL SHAVING ENTERED EE RIGHT EYE. HE WAS WEARING SAFETY GLASSES AND A FULL FACE SHIELD AT THE TIME OF INCIDENT.

LANCING A PIN ON LR-1 AND HIT A GREASE POCKET, IGNITED AND FLAME HIT FACE.

WHILE SITTING IN HAUL TRUCK DURING LOADING, SHOVEL SWEPT AROUND WITH FULL BUCKET HITTING THE REAR OF HAUL TRUCK CAUSING SHOULDER AND HEAD TO HIT THE CAB OF TRUCK.

OPERATING IMPACT WRENCH THAT SLIPPED AND HIT FOREHEAD.

OPERATING DOZER (D-10), BLADE CAUGHT ROCK, EE FELT PAIN IN MIDDLE BACK.

WAS GRINDING OFF EXCESS WELD AND USED AIR TO BLOW GRINDINGS FROM WORK AREA SPACE. A SMALL METAL SHAVING DEBRIS SLIPPED AROUND FACE SHIELD AND SAFETY GLASSES. INJURY SUSTAINED IN LT. EYE FROM THE DEBRIS CAUSING AN ABRASION. EE SOUGHT MEDICAL ATTENTION TO REMOVE THE DEBRIS.

USING PORTABLE HAND GRINDER WITH GUARD, FELT SOMETHING ENTER EYE. WAS WEARING FACE SHIELD AND SAFETY GLASSES.

EE OPERATING DOZER USING RIPPER. WHILE BACKING UP HILL, DOZER SEAT WAS HITTING UPPER BACK AND LOWER NECK AREA. STARTED RESTRICTED WORK ACTIVITY 04/26/01.

EE GOT METAL SHAVING IN HIS RIGHT EYE WHILE WELDING AND GRINDING. THE OBJECT WAS IMBEDDED AND HAD TO BE TAKEN TO A PHYSICIAN TO REMOVE. HE WAS RELEASED TO WORK WITHOUT RESTRICTIONS.

EMPLOYEE WALKED INTO PIPE WHICH WAS BEING MOVED BY A CRANE.

INJURED RECIEVED A FLASH BURN TO HIS EYES WHILE WELDING. HE WAS SEEN BY A SPHSICAIN AND RECIEVED MEDICAL TREATMENT AND WAS RELEASED TO WORK WITH RESTRICTIONS.

THE BLOW DOWN VALVE BLEW EAR PROTECTION OFF AND IAR BLEW IN EAR.

EE CLAIMS WHILE REMOVING BOLT FROM CORNER BIT OF DOZER, USING AN AIR GUN, NUT SPUN AND CUT LEFT INDEX FINGER. SUTURES WERE NEEDED TO CLOSE WOUND.

D-10N CATERPILLAR DOZER BACKED DOWN 2.26:1 SLOPE OF DAM FOR TOTAL OF 136 FEET INTO POND. ( FATALITY REVIEW COMMITTEE HAS DETERMINED IT PREMATURE TO ISSUE A FINAL DECISION ON THIS CASE. WILL RE-EVALUATE IF SUFFICIENT EVIDENCE IS FOUND TO PROVE EE DIED IN ACCIDENT.)
