Employee was welding and started having trouble breathing from the smoke and fumes from welding.

Removing broken master link bolts - D10T track. Elevated portion on the service truck crane and a crib block. When lowering track to ground, engaged boom in order to elevate the track off the crib block. Boom had a delayed due to a now diagnosed missing sensor spring. The boom raised unexpectedly, caused tension on the crane cable and the cable whiplashed. Laceration to forehead.

Employee was backing up EE's run in the shot material on the Caterpillar D10T Dozer when a rock rolled out of the wind roll and the dozer backed up onto the rock and then slide off the rock jarring the employee.

Employee was operating D11T Dozer cleaning highwall and pushing shot material when EE backed up on a rock and dozer slid off rock causing it to jar EE's back and left leg. Employee refused medical attention at time of accident said was ok and didn't seek medical attention until 07/08/2024.

Employee was operating D6 Dozer cutting a road in at the toe of the valley field when employee backed up the offside track went up a rock and slid off causing it to jar their neck and shoulder area.

Employee was cutting a bracket on the highwall drill with a torch when a piece of slag popped and went through under cutting glass contacting left eye.

Employee was pulling a stump out of the ground, when the bucket slid off the stump, made seat bottom out and jerked employee's lower back. Employee did not miss any work until 12/15/2020.

Employee was cutting a piece of rebar, took the torch to blow away dust and metal shavings, when it blew back in employee's face getting into right eye.

Employee was backing up D11 Dozer when EE back over a rock and jarred EE's left shoulder.

Employee was drilling a hole for an anchor pin on the Highwall miner when the anchor pin fell striking the employee on left foot

Employee was taking D11R Dozer up on shot when the face of the shot broke causing the dozer to turn over hurting right thumb and spraining left ankle.

Removing exhaust clamp - it broke and was slung by air ratchet cutting left finger

Employee was dressing around face-up of mine area when the corner of his dozer blade caught a gas line causing the dozer to catch on fire and he jumped out and broke left leg and brunt both hands.

Employee was operating a dozer on solid rock when he got jerked causing a hurting in his lower back.

Employee was grinding on step of Volvo Truck when the grinding rock busted and a piece hit him on the left side of his face (the jaw area) cutting a gash.

Employee was cutting a bench with D9 Dozer, went over edge of bench causing dozer to turn over on it's side hurting his left hip and knee. Employee was taken to Mary Beckingridge E.R. in Hyden, Ky. where he was checked out and released.

Employee was hydro seeding on a hill and was pulling hose when he strained the lower part of his stomach.

Employee was working on road with a d-9 dozer when a rock rolled out of a berm and he backed over it jarring his lower back. Employee didn't go to doctor until 9/27/2011

EMPLOYEE WAS GRINDING A SMALL PIECE OF METAL, WHEN HIS HAND CAME IN CONTACT WITH THE GRINDER, INJURING HIS LEFT INDEX FINGER.

Employee started drill and was tramming around the hill, when he hit a bump in the road, and it jarred the door shut on the employees hand.

Employee was cutting brush on the berm when he slipped. As employee fell forward, he put his right hand on the chain, cutting his hand.

Employees was pushing spoil with a dozer. The employee went forward over the pile of dirt and when the dozer fell over the dirt the blade hit the ground, jarring the employee.

Employee was cutting a bolt off the frame of the excavator when a piece of hot metal went into his ear.

Employee was removing a portable antenna from the top of the dozer when he felt a sharp pain in his lower back.

Employee was loading a backhoe onto a trailer, the backhoe slid off the side and overturned. The employee continued to work until 06/26/2008, on this date the doctor took him off work to have surgey on his shoulder.

During over the normal use of the dozer over a period of years, employee claims that his back has become so painful that he is going to the doctor.

Employee was backing up dozer when he backed over the edge turning the dozer.

Employee was grinding metal when the grinder wheel shattered. A piece of the grinder wheel struck his left arm causing a laceration to the arm. Employee was taken to the hospital, recieved 4 stitches and returned to work.

Employee was dressing up a drill bench when he felt something like a crick in his neck, and after a short time the pain moved into his lower back. Employee stated that he had not done anything different and he didn't know what happened to cause his pain.

Employee was pushing rock with a dozer when the dozer lost traction and slipped back. When the dozer caught traction, it jerked the employee's back.

Employee was tramming dozer towards parking lot when he was blinded by the sun. The dozer dropped over a hump, hitting the blade and causing the dozer to stop abruptly. EE hit his head on cab of dozer (11 stitches to forehead.)

EMPLOYEE WAS OUT OF HIS DOZER CHECKING DRILL BENCH WHEN THE BANK THAT HE WAS CUTTING OUT FELL OUT CATCHING HIS LEG BETWEEN SOME ROCK AND THE PUSH ARM OF THE DOZER.

EMPLOYEE WAS TRAMMING DOZER WHEN THE TRACK SLIPPED. WHEN THE TRACK CAUGHT, IT JERKED THE EMPLOYEE CAUSING PAIN IN NECK AND BACK AREA.

EE WAS PUSHING FILL WITH A D11 DOZER, HE CALLED FOR THE FOREMAN TO COME GET HIM. EE STATED THAT HE COULD NOT STAND THE BUMPING OF THE DOZER. HE STATED THAT THE PAIN WAS FROM A PREVIOUS INJURY FROM A PREVIOUS EMPLOYER. EE WOULDN'T GO TO THE DOCTOR, BUT HE DID GO HOME.

Employee was cutting a tree down with a chain saw, when a limb flew back and hit him above the left eye. Four stitches to close laceration.

EE STARTED FORWARD WITH DOZER, THE DOZER SLIPPEDOUT OF GEAR, THE EE HIT THE BRAKES CAUSING HIM TO HIT HIS HEAD ON THE CAB OF THE DOZER NOTE: EE RECEIVED 9 STITCHES IN HIS HEAD.

EMPLOYEE WAS PUSHING IN A SHOT WHEN HE JARRED HIS BACK.

OPERATOR WAS RUNNING DOZER WHEN HE FELT PAIN IN HIS LOWER BACK.

EE BACKED OVER A ROCK AND WHEN THE DOZER HIT, THE EE COMPALINED OF PAIN IN NECK AND BACK.

EE WAS TRAMMING A D11 DOZER WHEN HE UNEXPECTLY RAN OVER A BIG ROCK THE JAR CAUSED PAIN IN HIS HIP AREA.

EE WAS TRAMMING GRADER UPHILL, THE GRADER LOST POWER AND STARTED BACKWARD AND COLLIDED WITH PARKED DRILL.
