Employee was cutting through a metal wall on 902 dragline, during this process the employee flinched. This caused the grinder to jump, and the cut-off wheel came into contact with employee's finger. Although gloves were in use, the injury still required sutures.

On March 7th, 2024, a dragline ground person was in the process of pushing in a ramp off the bench to access the pit floor for cleaning. During the process of pushing in the ramp, the dozer blade caught a rock or frost edge. The dozer turned sideways, overturned and then came to rest on its tracks. The employee was wearing the seatbelt at the time of the incident.

On May 31st, 2023, an employee was working to remove the crowd brake on 935 electric shovel to replace broken bolts. When the final bolt was removed, the stored energy in the crowd brake caused the brake to rotate. When the brake rotated, the employees arm came into contact with it. This contact caused lacerations to the employees' arm, which resulted in stitches.

Employee was performing a welding/grinding repair on a Dragline bucket. Later in the shift the employee's eye became irritated. The eye was flushed several times and nothing was identified in the eye. The next morning the eye was still irritated. An eye doctor identified a metal sliver in the eye. Proper PPE was being utilized when welding/grinding work was being performed.

An employee was in the process of starting to line bore. During this time the employee was shaking a can of spray lubrication to prepare to lube the cut. During this action the employees hand encountered the boring head which then pinched EE's right thumb against the bearing plate. This caused a partial amputation of the right thumb, and a broken right ring finger.

Employee was performing weld repairs in the car body of a loading shovel. Employee was wearing safety glasses & face shield, but at some point during grinding/air arcing a particle contacted the employees right eye. The eye was flushed & employee finished the shift and went on days off. On 8/28/18 the eye was bothering so employee went to eye doctor who removed a small particle.

Employee was cutting tack welds loose with a grinder. The grinder kicked back and struck employee's left leg causing a laceration.

While cutting a strap on a cable spool, the torch got caught pulling the employee's hand into the spoke causing a fracture.

Debris entered employee's eye while grinding on a beam. Employee was wearing a face shield and safety glasses.

While torqueing lug nuts, the right middle finger was pinched between the tool and wheel.

Employee was grinding out a weld area on the mast of the dragline. Employee was wearing all his personal protective equipment. Grinder trigger was not engaged but disc was still spinning when grinder slipped in employee's hand, disc contacted left index finger cutting tip. Finger required 4 stitches, employee returned to work.

Employee was grinding out an air arc cut for repair on a front end loader. Employee was wearing safety glasses and a face shield. The grinding particle found its way under both and contacted his right eye. The work area was fairly open, but had some potential for particles to deflect around. The employee was taken to eye doctor who removed the particle. Employee returned to work.

Employee lifted hood after welding in the shoe horn. He felt slag enter left eye from another employee grinding nearby.

EE was welding/grinding overhead around a shoe horn. EE later felt something in his right eye.

EE was grinding on 11/19, when a piece of slag flew into his left eye, while wearing proper eye and face protection. Eye was washed out on the job, but EE was taken to clinic on 11/22.

Unknown if foreign body entered right eye from grinding or from sweeper that came near employee's work area blowing dust particles.

A piece of slag fell into EE's left boot while welding. EE waited until weld was complete to remove boot and retrieve slag. Dime sized burn became infected and required debridement on 6/17/10.

Employee was operating a demolition saw. Employee was cutting a vertical concrete wall when the saw "kicked back", striking the employees faceshield, knocking his faceshield up, and cutting his lip and nose.

EE was trouble shooting air conditioning system on a Cat 24H motor grader. When checking air flow in an air duct, the employee did not realize the exposure to a moving fan. When retracting his hand it caught the fan, injuring two fingers. The finger tips were cut and fractured requiring medical treatment.

Employee was grinding overhead and wind blew some trash into his left eye.

While grinding a particle became imbedded in an employees left eye.

While air arcing and grinding, a particle became embedded in the employee's right eye.

Welder received a minor burn to his left eye while working on the revolving frame of a dragline.

Employee was spot facing tub with a hydraulic magnetic drill. EE was raising drill head up when shirt sleeve got hung in the drill pulling hand into drill, cutting the top of left hand. 14 stitches.

EE WAS GRINDING ON THE LEFT SIDE TOP OF THE TUB IN THE RAIL PATH AREA. THE WIND BLEW A FILING INTO THE EYE. 3

EE WAS SLIDING MATERIAL THRU THE TABLE OF THE SAW WHEN HIS LEFT FOREFINGER CAME IN CONTACT WIT6H THE SAW BLADE THAT WAS STILL MOVING. HE WAS WEARING LEATHER GLOVES.

EE'S FINGER WAS FRACTURED WHEN THE JACK HE WAS USING SLIPPED AND HIS FINGER WAS PINCHED BETWEEN THE JACK AND THE DOZER FRAME.

WHILE REMOVING DRAGLINE BUCKET TEETH, THE JACK HAMMER SLIPPED OFF THE PIN AND PINCHED EE'S THUMB. THE CUT REQUIRED STITCHES.

WHILE REMOVING A BOLT, THE SOCKET CAME OFF THE IMPACT WRENCH AND HIT HIM IN THE FOREHEAD. THE CUT REQUIRED STITCHES.

WHILE REMOVING BOLTS ON A DOZER, THE IMPACT WRENCH SLIPPED AND CUT ROBERT'S FINGER.

EE WAS UTILIZING A MAGNETIC BASE DRILL IN A VERTICAL POSITION. THE DRILL BASE WAS NOT SECURED AGAINST FALLING. AS THE DRILL FELL THE DRILL BIT CAUGHT HIS LEFT THUMB AND CUT IT. 4 STITCHES WERE REQUIRED.

WHILE TIGHTENING A BOLT WITH AN IMPACT WRENCH, EMP DROPPED WRENCH. WRENCH LANDED ON LEFT HAND, FRACTURING INDEX FINGER.

REMOVING BEADSEAT BAND OFF THE RT FRONT TIRE ON 994 CAT LOADER, USING 1800 TOLL (BEAD BREAKER) TO SEPARATE THE BEADSEAT BAND FROM THE BEAD OF THE TIRE. WHEN THE BEADSEAT BAND CAME APART FROM T HE TIRE THE 1800 TOOL POPPED OFF AND STRUCK EE IN THE NOSE.

WHILE USING A GRINDING WHEEL, A METAL PARTICLE ENTERED EE'S EYE. THE EMBEDDED PARTICLE WAS REMOVED BY A PHYSICIAN.

EMPLOYEE WAS GRINDING A PIECE OF METAL ON A BENCH GRINDER WHEN THE WHEEL GRABBED THE METAL AND PULLED HIS HAND AGAINST THE WHEEL. THE CUT REQUIRED 5 STITCHES.

WHILE TORQUEING A BOLT ON 901 DRAGLINE THE WEDGE USED TO SUPPORT THE REACTION ARM OF THE HYDRAULIC TORQUE WRENCH POPPED OUT & HIT EE ON THE FOREHEAD THE CUT REQUIRED STITCHES

EE'S FINGER WAS PINCHED WHEN THE BOOM TRUCK OPERATOR LIFTED THE BOOM AS INJURED WAS UNHOOKING THE RIGGING . THE CUT REQUIRED 6 STITCHES.

EE WAS REMOVING BOLTS FROM SWING RACK ON 3570-W DRAGLINE WITH A JACKHAMMER. HE LEANED OVER TO CHECK THE TOOL THAT WAS INSTALLED IN THE JACKHAMMER. WHEN DOING THIS HE BRUSHED AGAINST THE TRIGGE R ON THE JACKHAMMER. IT ACTIVATED & HIT THE END OF HIS LEFT INDEX FINGER FRACTURING THE BONE.

WAS CLEANING BEARING AND RETAINER UNTI WAS STANDING UP RIGHT AND BLOCKED UNIT HAS A FLAT SPOT MACHED ON ONE SIDE OF THE RETAINER EDGE FLOOR HAD SLIGHT SLOPE. VIBRATION OF FLOOR DUE TO EQU8PMEN T MOVING IN SHOP MAY HAVE CRISED BEARING TO MOVEFORWARD AND ROLL ONTO FLAT SURFACE ON SIDE OF BEARING RETAINER AND FALL ONTO BIG TOE RIGHT FOOT AND FRACTURE TOE.

WHILE LOADING DRAGING ROPES ONTO A SKID, THE LIFTING DEVICE SLIPPED & PINCHED HIS FINGERS AGAINST THE END COLLAR ON THE SHAFT. HIS RIGHT MIDDLE FINGER WAS FRACTURED.

WHILE CUTTING AND WELDING A PARTICLE BECAME EMBEDDED IN EE S LEFT EYE AND WAS REMOVED BY A DR ON 5-3-94.

GARRETT EXPERIENCED NECK PAIN AFTER THE OVERBURDEN TRUCK HE WAS OPERATING MOVED AHEAD ABOUT ONE FOOT WHEN HIT BY THE LOADING SHOVEL BUCKET. THISBECAME AN LTA ON 2-9-94.

WHILE WASHING A SCRAPER WITH THE HOTSY PRESSURE WASHER, THE WATER STREAM WENT ACROSS HIS BOOT RESULTING IN SECOND DEGREE BURNS TO HIS RIGHT FOOT.

EE EXPERIENCED BACK PAIN AFTER WORKING IN THE SHOP IN AN AWKWARD POSITION

WHILE WASHING A PICKUP WITH THE HOTSY STEAM CLEANER, A HOSE FITTING PARTIALLY BLEW OUT SPRAYING HOT WATER ON HIS LOWER LEFT ABDOMEN CAUSING A SECOND DEGREE BURN.

WHILE CUTTING AND GRINDING ON A COAL HAULER, A PARTICLE ENTERED THE EMPLOYEE'S EYE. THE EYE WAS FLUSHED. WHEN THE IRRITATION PERSISTED THE EMPLOYEE SAW A PHYSICIAN AND OBTAINED A PRESCRIPTION.

WHILE WORKING IN DRAGLINE,MAG DRILL FELL AND SMASHED RIGHT INDEX FINGER.3 STITCHES REQ,NO BONES BROKEN,

I,NAME IS NOT SURE EXACTLY WHERE THE PARTICLE CAME FROM-WELDING OR GRINDING. HOWEVER HE DID NOT HAVE A PARTICLE REMOVED FROM HIS EYE ON 12-1-88.
