While attempting to start a D10 Cat Dozer, a section of the dash board burst causing glass to be blown out and causing a cut on the employee's arm that required the wound to be sutured.

The Dozer operator was tramming backward in a shot when the track slipped off of a boulder and injured his left side soft tissue in the rib area

Employee leaned over to start the fuel pump and when he pulled the engine start rope, he felt a stinging, burning sensation in his left upper chest.

Two welders were welding on the auger, the injured employee was helping the two welders hold the metal that they were welding. The flash from the welding burned the injured employee's eyes, resulting in his eye sight being limited.

Employee turned the A/C unit on and an unidentified object flew into the employees eye.

Employee was changing the chip deflector and replacing a new bit. Employee was using a pipe to unstop the drill bit and the bit blew out rock fragments into the employee arms and chest.

Employee was grinding on a piece of metal. While doing so a small piece of metal shaving flew through the side of employee's safety glasses, resulting in the metal shaving embedding in his right eye.

Employee was welding on a 777C Rock Truck Bed, and a piece of Slag flew up and struck the employee below the left eye, causing a mild burn below his eye.

Employee was tighting roller bolt on D11R dozer, Employee was lying on his back while tighting bolt and socket spun off head of bolt causing metal shavings to fly in employee eye, Employee did have safety glasses on

D9R dozer operator was traveling in reverse and struck a 777C rock truck in the rear, the rock truck was also traveling in reverse. The D9R operator states he didn't see the rock truck as he began to go in reverse, the rock truck driver also states he did not see the D9R dozer through his mirrors as he was traveling in reverse.

EE was pushing material down to 992D loader with a D9R dozer co. # M37 & the machine's blade slipped off a rock causing a sudden jerk, which resulted in EE having sudden pain in lower abdomen.

Employee was tamping dirt in drill hole of a rock approx. 4 ft in height, as He was tamping he twisted his back, causing him to pull a muscle in his back.

Employee was cutting a pine tree and when the tree fell over, the chainsaw jumped back and struck the employee in the left knee, which resulted in him getting 20 stitches in his knee. 10 inside and 10 outside.

Employee was welding beside another welder & the other welder arked his welder, causing injured EE's left eye to get severe burns.

HURT BACK WHILE OPERATING DOZER RESULTING IN BACK PAIN BETWEEN NECK AND SHOULDER BLADES.

WAS USING HAND GRINDER, THE HAND GRINDER SLIPPED FROM HAND, RESULTING IN LEFT WRIST BEING CUT.

WAS OPERATING 375 DOZER: WHEN DOZER WENT OVER BIG ROCK- RESULTING IN A WHIP-LASH MOTION HURTING LOWER BACK.

EE WAS CUTTING BLADE LINER OUT OF D-11 DOZER BLADE. LINER SPRUNG OUT HITTING HIM KNOCKING HIM TO GROUND RESULTING IN BROKEN RIB ON LEFT SIDE OF BODY #8 RIB.

EMPLOYEE WAS BACKING UP THE DOZER AND RAN OVER A ROCK.

WHILE OPERATING A KOMATSU 375 DOZER, OPERATOR BACKED OVER A ROCK. WHEN BACK OF DOZER CAME DOWN, RIPPED HIT GROUND FIRST CAUSING SUDDEN STOP.

EE CLAIMS SMOKE BEING BLOWN BACK IN FACE WHILE WELDING BUCKET ON 992C WAS INHALED & MADE HIM SICK. EE NOTIFIED CO. 4-24 THAT HE WENT TO DR. 4-22& SAID DR. WOULD NOT KNOW CAUSE UNTIL 4-28. UPON RETURN WAS TOLD TEST NOT BACK YET & WAS UNSURE OF CAUSE BUT RELEASED EE TO RETURN 5-1. CLAIMS ADJUSTED PHONE 5- & SAID HE & DR. HAD DECIDED SMOKE INHALATION. EE STILL OUT FOR UNRELATED REASON

EE WAS DIGGING OUT ROCK WHICH CAUSED DOZER TO SLIDE OVER SLOPING ENBANKMENT.

USING ELECTRIC GRINDER. GRINDER CAUGHT AND KICKED BACK CAUSING CUT OVER LEFT EYE.

EE WAS HELPING INSTALL EXHAUST SYSTEM ON DRILL. EE WAS HOLDING HAND WRENCH WHILE A SECOND EE WAS USING AN AIR WRENCH. AIR FROM AIR WRENCH CAUSED PIECE OF METAL TO GO INTO EYE.

DOZER SEAT BROKE OPERATOR HIT SOLID

WAS PUSHED BACK BY FORCE OF STREAM LEANER WAND CAUSING FEET TO SLIP & FELL OFF STRAP DRILL BACK WAS INJURED

USING GRINDER WAS WEARING SAFETY GLASSES PIECE OF METAL FLEW IN EYE...DID NOT INPAIR VISION.
