EE was cutting a steel beam into. The beam was lying across the forks of a forklift. When the beam was cut into, one end of the beam slid off the forks and landed on the concrete floor, then bounced and struck EE on top of his right foot.

Mostrando los 38EE was raising the drill boom of the roof bolter to check the size of a busted hydraulic hose. The hose got fouled on the frame of the roof bolter, and when he raised the drill boom and looked under it, the hose came loose and struck him above the left eye, resulting in a cut.
EE HAD DRILLED A HOLE IN THE MINE ROOF AND THE DRILL STEEL STUCK IN THE ROOF. WHEN HE LOWERED HIS DRILL POT, THE DRILL STEEL FELL FROM THE HOLE AND STRUCK HIM ON THE LEFT HAND.
EE was lowering the head of the continuous miner on some crib blocks that were being used to block the head up. When pressure was applied to the crib blocks, one crib block twisted out and struck EE on the right jaw.
EE was operating a continuous miner. He was cutting the bottom ledge of rock out from the edge of the #9 intersection, (continuous miner was in the last open crosscut between #8 and #9). The crosscut was on a downward grade, and when he start the cutter head and placed it in the bottom, the miner slew around and struck him with the conveyor boom. EE was not in the "RED Zone".
EE WAS OPERATING A CONTINUOUS MINER, CUTTING OUT THE MINE ROOF FOR AN OVERCAST, WHEN A PIECE OF ROCK FROM THE CUTTER HEAD STRUCK HIM ABOVE THE RIGHT EYE REQUIRING STITCHES.
EE WAS AT THE CONTINUOUS MINER TALKING WITH THE OPERATOR ABOUT A NOISE THE CUTTER HEAD WAS MAKING. THE CONTINUOUS MINER WAS IN THE RIGHT BREAK OF THE #4 ENTRY AND EE AND THE OPERATOR WERE STANDING IN THE HEADING. THE CONTINUOUS MINER CUTTER HEAD WAS RUNNING AND EE WAS STRUCK IN THE LEFT EYE BY A FOREIGN OBJECT.
Dislocated (L) shoulder.
EE was drilling a hole in the mine roof with a roof bolter and the drill steel broke and struck him on the right eye brow.
EE was using a cutting torch to cut a piece of metal. There was grease on the back side of the metal, and the grease popped out onto his hand, causing burn.
Pinched his finger between the pot and head of roof bolter.
Pinning top and glove got caught in bolter.
EE WAS PUTTING BLOCKING UNDER THE WHEEL OF A ROOF BOLTER THAT WAS STUCK. HE GOT HIS FINGERS FOULED UNDER A HALF HEADER AND WAS RUN OVER BY THE ROOF BOLTER TIRE, RESULTING IN TWO(2) FINGERS BEING BROKEN.
EE WAS TRAMMING THE CONTINUOUS MINER. HE HAD STOPPED THE MINER TO MOVE A SECTION OF CABLE, AND WHEN HE REACHED FOR IT HIS FOOT SLIPPED , CAUSING HIM TO ACCIDENTALLY HIT THE TRAM LEVER. THIS RESULTED IN THE MINER MOVING SUDDENLY, WITH THE BOOM STRIKING HIM ON THE LEFT SIDE OF THE BODY.
Employee was operating a roof bolter in the #3 left break on the 001-0/002-0 mmu. He was putting the drill steel in the drill pot on the Fletcher roof bolter when his left glove got caught between the chuck and drill steel. He engaged the rotation at the same time resulting in a fracture in the little on the left hand.
Rock fell off miner boom and landed on employee's foot.
EE was operating a roof bolter in the #4 heading of the 001 mmu. He was installing a 10' cable bolt. As he was pushing the bolt into the drilled hole with a cable wrench, the bolt dislodged from the wrench head and struck him on the right jaw.
EE was operating a continuous miner. He had completed cutting the coal seam and was starting to trim the top. He was located on the left rib beside the miner approx. 13" from the end of the frame. When he started to cut the top, the miner kicked sideways and caught him between the miner frame and the coal rib. Results of the injury were bruising and contusions.
EE was drilling a hole in the mine roof with a roof bolting machine and the drill steel got fouled. After removing the first piece of steel, the second piece of steel fell from hole and struck him on the right index finger.
EE was operating a roof bolter in the #3 heading of the 002-0 section. He was installing a roof bolt in the mine roof, and when he reached to align the roof bolt plate, his glove got caught on the plate, dislocating the little finger on his left hand.

Was a shuttle car parked in lower end of entry outby the intersection, looking forward to see if he was out of the miners way. had his right hand on outside of roof bolter tram & backed roof bolter into parked shuttle car, mashing his hand between roof bolter frame & shuttle car bumper.
Injured employee was removing spilled material from the shaft doors when the sinking bucket was lowered striking him in the lower right leg.
EE WAS LETTING THE CONTINUOUS MINER HEAD DOWN ON A CRIB THAT HAD BEEN BUILT FOR BLOCKING THE HEAD WHILE WORKING WAS TO BE PERFORMED. WHEN HEAD PRESSURE WAS APPLIED TO CRIB, A CRIB SQUEEZED OUT, STRIKING EE ON LEFT HAND, CAUSING INJURY TO RING FINGER & LITTLE FINGER.
Employee was walking around the continuous miner in the #7 entry of the 002-0 mmu, preparing to energize the cutter head breaker, when a piece of draw rock approx. 3'x2'x4" fell, striking his lower back. He continued to work regular duties until 7/15/04 when he went to his family dr. First day of lost time is 7/15/04.
EE WAS OPERATING A CONTINUOUS MINER ON THE 001-0 SECTION WHEN A PIECE OF DRAW ROCK FELL FROM BETWEEN BOLTS WHERE HE WAS STANDING AND STRUCK HIM ON THE TOP OF HIS RIGHT FOOT. EE WAS CUTTING COAL IN THE #3 HEADING. DRAW ROCK THAT FELL WAS APPROX. 2' X 3' X 4".
EE WAS BRINGING A CONTINUOUS MINER ACROSS THE SECTION. HE WAS BETWEEN #4 & #5 ENTRIES AND STOPPED TO REMOVE A PULL ROPE FROM MINER BOOM. WHEN HE BENT OVER TO REMOVE ROPE, HIS SUSPENDERS CAUGHT A TRAM LEVER AND CAUSED THE MINER TO MOVE AWAY FROM HIM. HE WAS CAUGHT BY EXTRA ROPES AND CABLE ATTACHED TO BOOM, RESULTING IN HIM BEING TOSSED AGAINST BOOM, STRIKING HIS BACK ON BOOM.
Ee was operating a continuous miner when a piece of draw rock approx 2' wide and 6' long and 3" thick fell from between bolts and struck him on the left arm and leg causing injury.
EMPLOYEE WAS OPERATING A CONTINUOUS MINER IN THE LEFT BREAK OF THE #2 ENTRY ON THE 001-0 MMU WHEN A PIECE OF DRAW ROCK FELL FROM BETWEEN BOLTS AND STUCK HIM ON THE HEAD, CAUSING NECK PAIN.
EE WAS OPERATING A CONT MINER IN THE #8 ENTRY OFTHE 004 SECTION WHEN A PIECE OF DRAW ROCK APPROX3'LONG, 2' WIDE, & 4"THICK FELL FROM BETWEEN BOLTS & RIB STRIKING HIM ON THE LEFT SHOULDER AN D THEN SLIDING DOWN HIS BACK
EE WAS OPERATING A SHUTTLE CAR, GETTING A LOAD OF COAL WHEN COAL RIB ROLLED OFF STRIKING HIM ON RIGHT SHOULDER. TRANSPORTED TO PIKEVILLE METHODIST ER. X-RAYS WERE NEGATIVE. EE HAD LARGE CONTUS ION ON RIGHT ARM & SHOULDER.
EE WAS OPERATING A CONTINUOUS MINER, LOADING A SHUTTLE CAR, WHEN RIB BEHIND HIM ROLLED OFF AND SHOVED HIM INTO SHUTTLE CAR CAUSING CUTS TO HEAD AND FACE AND BRUISES TO CHEST. TRANSPORTED TO PI KEVILLE ER. PRELIMINARY FINDINGS, CUTS TO FACE & HEAD. FRACTURED LEFT CHEEK BONE.
EE WAS OPERATING A ROOF BOLTER IN THE #3 ENTRY OF THE OO1 MMU. HE RAISED THE STAB JACK TO RELOCATE THE MACHINE AND THE CANOPY, HAVING ALREADY BEEN LOWERED STRUCK HIM ON TOP OF HEAD, CAUSING NE CK PAIN. HE REPORTED ACCIDENT TO SECTION FOREMAN. HE WAS TAKEN OUTSIDE AND TRANSPORTED TO E.R., X-RAYS REVEALED NO NECK DAMAGE & DR. SAID HE WOULD HAVE A SORE NECK FOR 1-2 DAYS.
WHILE OPERATING A ROOF BOLTER, OPERATOR LET DRILL POT DOWN ON LEFT KNEE.
PIECE OF DRAW ROCK FELL STRIKING INJURED ABOVE LEFT KNEE. EE DID NOT LOOSE ANY WAGES UNTIL 9-24-01.
EMPLOYEE WAS OPERATING MINER WHEN RIB FELL STRIKING EMPLOYEE, CAUSING BRUISES AND A FRACTURE TO RIGHT LEG.
EE WAS MOVING 3 WHEELER-STRUCK RIB CAUSING BACK PAIN.
WHILE HE WAS OPERATING A ROOF BOLTER A ROCK ~3' X 1' X 4" THICK FELL STRIKING THE TOP OF HIS LEFT FOOT, FRACTURING IT.
EE WAS USING A HAND HELD DISC GRINDER IT BECAME TANGLED IN HIS CLOTHING CAUSING IT TO STRIKE HIM ON THE RIGHT SIDE OF HIS HEAD AND EAR CAUSING LACERATIONS TO BOTH.
