Operator was tramming a dozer to the pit and alleges he pulled a muscle in his lower back. This was not reported until the 27th.

Employee was mounting his drill, stepped onto the first step started to lift to the second step ,pulled a muscle in his lower left leg.

Operating a D-11 dozer, backed up hitting a rock that gave him a strong jolt, caused pain in lower back.

EE felt sick with back / side hurting and decided to go to the doctor. Did not inform the Foreman that he had an accident. Foreman thought he was sick.

Employee was greasing equipment when the hose of the grease gun came loose from the fitting. The end of the grease hose struck employee in left eye.

Employee was grinding metal and the grinder kicked back and cut index finger on his left hand. (Required stitches).

Employee was working highwall, backing up, backed over a rock hitting hard and causing employee to be thrown toward right hitting his head on a computer panel. NOTE: Employee did not seek medical attention or miss time until 10-31-08.

Employee was using an electric grinder when it slipped out of his hand and hit his leg. Grinding disc was still moving which caused an 1 1/2" cut on leg.

EE was operating dozer, reached for the steering clutch lever, felt pain in his left shoulder.

Drill steel fell on hand when the steel fell through an auger hole mashing his right middle finger and thumb against the deck. Received several stitches in middle finger of right hand.

Employee was attempting to grind bosses on a hydraulic control valve. He was standing under the machine, reaching through hoses and looking up at the area where he was grinding. Piece of metal dust flew into his eye.

Employee had primed water pump (6") and started engine...The primer plate blew off and hit employee in the head requiring stitches.

Employee was pushing in shot area using a D-10-R Cat dozer and backed over a hole hitting his head on left side window. Lost one front tooth, feeling pain in his head, neck and back.

EMPLOYEE WAS USING A GRINDER OVERHEAD, HE WAS WEARING A SHIELD. A PIECE OF METAL WENT INTO HIS LEFT EYE.

Employee was using an impact gun with a wobble head socket. The socket broke and a piece of metal from the socket cut the palm of his left hand where the thumb joins the hand.

EE was operating dozer on 7/28/05 when dozer hit large 5.5. rock stopping the dozer suddenly jarring ee. He felt pain in his back at that time but continued to work during varying levels of pain until 8-17-05. He was helping mechanic work on dozer, reached something to mechanic and felt severe pain in back went for medical help.

EMPLOYEE WAS OPERATING DOZER AND IT SLIPPED OFF A ROCK AND JARRED HIM. HE INDICATED THAT HIS FINGERS WERE GOING NUMB AND HIS SHOULDERS WERE HURTING.

Employee was welding overhead and got something in his eye (left).

Employee was operating a D-11 cat dozer when he backed over a rock. The dozer came down hard and jarred him. Neck was hurting and right shoulder.

Employee was operating a dozer, said dust came into the cab & got into his eyes.

Employee was cutting a hole in a battery box, finished, removed safety glasses and was putting his torches down. The rain was hitting the hot metal slag and it popped up into his face. A piece went into his right eye.

Employee was cutting a small tree with a chain saw when the saw blade kicked back and hit his upper leg.

EMPLOYEE WAS OPERATING A DOZER, PUSHING IN THE SHOT & HE SLIPPED OFF A ROCK, JARRING HIS BACK & NECK.

EE was welding on the end cap of a dozer push arm. He thought he had flashed his eyes, but it continued to get worse. He went to the doctor on Jan 7, 05, The Dr cleaned his eye and told him to stay off work on the 7th and he could return to work on the 8th of Jan. EE missed one day.

Employee was cutting bots from cutting edge on 988G, the edge dropped down hitting his left knee.

EE was pushing in the shot with a d11, a rock rolled in behind him and when he backed up he hit the rock, causing a hard jolt. He felt something pop in his neck.

EE WAS AUGERING WHEN ROCKS STARTED FALLING FROM THE TOP OF THE HIGHWALL ONTO THE AUGER. THE METAL CANOPY WAS BROKEN THRU AND THE CAB SUSTAINED BROKEN GLASS. THE EE WAS CUT ON HIS RIGHT INDEX F INGER THIS REQUIRED 6 STITCHES.

EMPLOYEE MASHED HIS FINGER WHILE HELPING A MECHANIC TORQUE WHEEL LUG NUT. HE PINNED HIS RING FINGER BETWEEN THE RIM AREA AND THE MULTIPLIER WRENCH.

WAS CUTTING BITS ON ROLLER FRAME, WHEN HOT SLAG OF METAL WENT BEHIND SAFETY GLASSES INTO RT. EYE.

EMPLOYEE WAS PUSHING REFUSE WITH A D10R DOZER AND WAS HAVING PAIN IN HIS BACK. THE PAIN GOT SO SEVERE HE HAD TO STOP OPERATING THE DOZER. HE SOUGHT MEDICAL ATTENTION. THE EE DID NOT INDICATE T HAT THIS INJURY WAS JOB RELATED UNTIL 8-4-03.

EMPLOYEE WAS WORKING ON D-10 DOZER HARD BAR, PAD THAT GOES TO THE HARD BAR CAME LOOSE HITTING EMPLOYEE IN THE HEAD. THIS CAUSED A SMALL CUT THAT REQUIRED 2 OR 3 STITCHES.

EMPLOYEE WAS OPERATING A DOZER AND SAID HE RAN OVER A ROCK CAUSING THE BACK OF THE SEAT TO SLIP, JERKING HIS NECK.

EMPLOYEE WAS SCALING THE HIGHWALL USING AN EXCAVATOR. A ROCK CAME OVER TOP OF THE BUCKET THROUGH THE WINDSHIELD AND HIT EE ON HIS LEFT KNEE.

EE WAS USING A GRINDER WHEN IT SLIPPED OFF THE LIP OF THE BUCKET CUTTING RT. KNEE. THIS LACERATION REQUIRED STITCHES.

EE OPERATING A CAT DOZER, WAS CLEANING THE WALL OF LOSE MATERIAL. HIS BLADE SLIPPED OFF A ROCK &THE SUDDEN JOLT PULLED A MUSCLE IN HIS BACK.

EE WAS OPERATING A DOZER RECLAIMING A STEEP SLOPE AND WENT OVER A LARGE ROCK WHEN HE LANDED THE JOLT JARRED HIM REAL HARD.

EE WAS OPERATING A D11R CAT DOZER IN SHOT MATERIAL AND HOOKED A ROCK WITH THE BLADE. WHEN HE TRIED TO BACK UP AND AWAY FROM THE ROCK, THE BLADE SLIPPED AWAY FROM THE ROCK AND JERKED HIS NECK.

EMPLOYEE WAS GRINDING PIECE OF METAL-DUST OR METAL FLEW UNDER HIS SAFETY GLASSES INTO RIGHT EYE.FLUSHED OUT EYE, WAS GIVEN EYE DROPS, NO LOST TIME.

EMPLOYEE WAS WELDING D11R MAIN FRAME. STOPPED WELDING AND RAISED WELDING HOOD AND THEN FELT SOMETHING IN RIGHT EYE. METAL MUST HAVE BEEN ON WELDING HOOD.
