A roof bolt operator incorrectly coupled a piece of pusher steel and finisher still while attempting to drill a hole for roof bolt installation. With pressure applied, the finisher steel flew back and struck the employee around the bridge of the nose causing a laceration and a fracture of the sinus cavity.

Mostrando los 32A roof bolter's right uniform sleeve was caught by a roof bolt plate as EE was installing a roof bolt in the #8 heading. Both forearm bones were broken and EE received a cut to the right ear that required stitches.
A roof bolter operator reported at the end of EE's shift that EE's left shoulder was strained earlier in the shift while installing rope bolts. The employee performed EE's regular duties throughout the shift.

As a result of the broken pin, the canopy lowered when bolter operator pressurized the stab jack. The canopy lowered on operator's head resulting in a strained neck and shoulder.

As he was lining the pusher and finisher steels up he accidentally hit the lift lever and caught his left pointer finger between the two steels. He received medical attention and has been put on restricted duty. Days restricted will be reported when he returns to work.
