EE SAID HE WAS SHOVELING ON OFFSIDE #4 BELT, JUST INBY #4 BELT DRIVE. A PIECE OF ROCK FELL, HITTING HIM ON LEFT SHOULDER AND LEFT ARM. ROCK CUSE TO BREAK HIS ARM.

EE SAID HE WAS REACHING ACROSS PINNER TO GET GREASE GUN AND WHEN HE BROUGHT HIS ARM BACK WITH GREASE GUN, HE HIT HIS ELBOW AGAINST A LID.
EMPLOYEE SAID HE WAS USING RAILROAD JACK AND SLATE BAR TO LIFT LONG JOHN STRUCTURE UP. SLATE BAR KICKED OUT JACK AND HIT HIM IN SIDE OF HEAD.
EMPLOYEE SAID HE WAS PULLING WATER LINE AND A PUMP OUT OF WATER HOLE, WHEN HE FELT SHARP PAIN IN CHEST AND BACK AREA.
EE WAS GOING TO CROSS BELT LINE ON #5 TAIL PIECE AT #6 DRIVE. THE BELT WAS OFF DOWN LINE SOMEWHERE ELSE. HE STARTED TO CROSS BELT, PUT HIS FOOT DOWN ON BELT WHEN IT STARTED RUNNING. IT PULLED HIS FEET OUT FROM UNDER HIM. HE HIT TAIL PIECE.
EE SAID HE HAD A SLATE BAR & WAS PRYING LONG JOHN OVER, AS THEY WERE PUTTING LONG JOHN PIECES TOGETHER, WHEN HE [...] A SHARP PAIN IN LOWER BACK.

EE SAID HE WAS SHOVELING BELT HE PUSHED THE SHOVEL INTO SOME SOLID COAL UNDER BELT, HE STRAINED HIS LEFT WRIST. HE SAID HE TRYED TO WORK 10-11-96 AND SAID HE HAD TO COME OUTSIDE EARLY TO GO SE E A DR.
EE SAID HE WAS TRAMMING MBC-MOBILE BRIDGE, WHEN IT TILTED OVER A LITTLE HILL RAISING DECK UP TOWARD TOP, EE HIT HIS HEAD INTO MINE ROOF. EE SAID HE HAD PAIN IN NECK & SHOULDERS
EE SAID HE PICKED UP MINER CABLE TO THROW OUT OF MIDDLE OF HAULWAY, HE SAID HIS LEFT WRIST STARTED HURTING.
EE SAID HE WAS CHINGING OUT SOME WATER LINE THATRUNS UP ON HILL & AROUND TO THE WATER BARREL. HE FELL DOWN ON HIS RIGHT KNEE.
EMPLOYEE WAS BOLTING LEFT BREAK IN #3 ENTRY. KETTLE BOTTOM FELL ON LEFT LEG. BROKE LEFT ANKLE.
EE SAID HE WAS REACHING FOR A TIMBER OFF OF A PILE OF TIMBER ON RIB AND THE RIB ROLLED OFF, HITTING HIM ON RIGHT ARM & WRIST.
EE SAID HE WAS POSITIONING HIMSELF TO LIFT UP CABLE, HAD THE CABLE OVER HIS BACK & SHOULDER, ANOTHER EE PULLED CABLE DOWN ON HIM.

EMPLOYEE SAID HE WAS TRAMMING LONG AIRDOX BRIDGE SYSTEM, WHEN A PIECE OF DAW ROCK FALL OUT BETWEEN BOLTS, HITTING HIM IN SHOULDER AND NECK.
EMPLOYEE WAS ARRANGING A DE-ENERGIZED 6/3 INSULATED CABLE FOR REPLACEMENT TO BE DONE BETWEEN SHIFTS, WHEN THE INSULATED LEADS FEEDING A 100 AMP DE-ENERGIZED BREAKER WERE APPARENTLY MOVED, CAUS ING AN ARC BETWEEN TWO PHASES ON THE TOP SIDE OF THE BREAKER. EMPLOYEE RECEIVED A FLASHBURN TO FACE AND RIGHT HAND.

INJURED WAS BACKING DOWN THE #1 ENTRY WHEN THE BOOM OF THE MINER STRUCK A ROOF BOLT SHEARING THE BOLT HEAD & DISLODGING A SECTION OF THE MINE ROOF NOT KNOWING THIS AT THE TIME HE CONTINUED TO BACK UP THIS PORTION OF THE MINE ROOF FELL STRIKING HIM & THE MINER
FAN ENTRY FELL IN FROM THE SURFACE INSIDE APPROX. 30 FT IN LENGTH 15 FT WIDE AND 20 FT HIGH FALL WAS CAUSED BY MOONFAIR BREAK IN TOP.
