An employee mashed the end of the right middle finger when EE caught the finger between a piece of top conveyor belt structure and the conveyor belt structure rail. This resulted in breaking the tip of EE's right middle finger.
An unplanned roof fall occurred on the active 002 section. The fall was above anchorage in the intersection of the last open crosscut in the #5 entry between SS 8440 and SS 8463.
A roof fall occurred on the 8A belt flight at SS# 8660, measuring 43' long by 35' wide by 11' high. There were no injuries. The MSHA hotline was called.
A section of rib rolled off on an employee after EE exited EE's scoop and walked between the scoop and the rib in order to retrieve a mandoor. This resulted in a fractured left leg and fractured left pelvis.
The offside roof bolter operator was in the process of installing a 2nd roof bolt in the 2nd row when they noticed a piece of draw rock moving out of the corner of their eye. A piece of draw rock measuring approximately 45" by 25" by 1/2" fell striking them just below the left elbow resulting in a cut that required stitches.
The employee was in the process of installing a roof bolt. As employee raised the head of the roof bolter up to meet the roof bolt that the employee was holding in place, employee accidentally engaged the rotation on the drill pot causing the bolt to spin and fracture employee's right thumb.
The employee was underground and preparing to load used conveyor belt structure in a scoop bucket. The employee pulled the scoop up next to the conveyor structure and exited the scoop. At this point they felt pain in the middle and lower parts of their back. Medical examinations reveal extensive arthritis in the employee's back.
The injured employee and a helper were rolling a continuous miner conveyor motor on the ground in order to install the motor. The employee rolled the conveyor motor over on to left ring finger causing a broken bone on the tip of this finger.
The employee was in the process of installing roof bolts in the #6 heading. After drilling with a piece of starter steel, EE lowered the drill pot. The starter steel was stuck in the mine roof. EE raised the drill pot back up to connect to the steel. EE's right middle finger was crushed between the drill pot and the piece of steel in the mine roof.
A piece of draw rock fell on a roof bolter operator as they were walking back to drop the ATRS to move the bolter up. One row of bolts had been installed.
The employee stated that they felt a sharp pain in their lower back and legs while shoveling material from underneath #2 drive.
An employee was taking slack out of a chain that was attached to a hook that was welded on a scoop bucket. The chain was ran through a continuous miner head in an effort to pull the head out. While reconnecting the chain to the hook, the miner head shifted causing the chain to catch the employee's left index finger between the hook and the chain resulting in a broken finger.
The employee was using a disc grinder to grind on a belt rail. The grinder kicked back and cut their chin resulting in a cut that required stitches.
A piece of draw rock fell from between the last row of bolts and the 2nd from last row of bolts in the #9 heading. The rock struck the employee on the left leg resulting in a broken leg.
This report is being submitted under protest. The operator does not believe that an event occurred that led to a personal injury: The employee was operating a Fairchild scoop. Employee stated that employee ran over something in the roadway that caused the scoop to throw employee up in to the mine roof, striking employee's head.
The stab jack pin broke on the operator side drill station canopy. The roof bolter operator set the jack on the ground in order to begin bolting. As a result of the broken pin, the canopy lowered when bolter operator pressurized the stab jack. The canopy lowered on operator's head resulting in a strained neck and shoulder.
A portion of the highwall over the number #1 fan portal entry separated from the highwall and rolled to the ground. There were no miners exposed. There were no injuries. The MSHA hotline number was called.
The employee was driving a battery powered three wheeler in the primary escapeway toward the section. EE traveled under a cable that was hung up by rope hangers with a hook on each end. EE didn't see that one of the hooks had come loose and was hanging. The hanging hook caught EE's coveralls and EE was dragged up between the ride and the mine roof.
The illness is described as Grade 2-3 chondromalacia of the left knee. This illness was noticed again by the employee while crawling to roof bolter to install roof bolts. The employee heard a "pop" sound similar to what the employee had noticed in the past.
The employee stood up from repairing maintenance work on a shuttle car. When employee stood up, employee struck employee's head against the mine roof. This caused employee to stumble, lose employee's footing, and then sprain employee's left ankle.

The employee was in the process of hooking up a pull behind rock duster to the three wheeler. The attachment tongue of the duster slid and caught EE's right index finger between the attachment tongue and the ride.
The injured was sitting on the end of a shuttle car with feet on the ground when another employee, who was operating a three-wheeled personnel carrier, inadvertently struck the injured with the personnel carrier.
The employee's right thumb was cut when the thumb was caught in the knot of a chain that was being used to remove a pot from a continuous miner.

The employee was scaling draw rock in the #5 entry. When the piece of draw rock that EE was pulling came loose, the forward momentum caused the end of the bar to come down on left thigh resulting in a cut that required stitches.
A roof fall was discovered in the secondary escapeway, #4 entry of #4 Mains, 140' inby spad 1462, as the fire boss was making pre-shift examination. The fall was approximately 6' high, 20' wide and 30' long.
Employee was lifting tool box to put on a mantrip and when ee turned to put it down ee felt a pain in back resulting in a back strain.
Employee was hanging miner cable and water line using a pogo stick and rope cable hanger. As employee placed the hanger on the pogo stick, the cable slipped and dropped down and the metal hook of the rope hanger caught employee's hand between the thumb and forefinger of the left hand, resulting in a cut that required stitches to close.
Employee was drilling a bolt hole in #6 entry when a piece of draw rock fell around the drill steel and struck the employee on the head, causing a compression type injury to neck and back.
Employee was moving an oxygen tank from the supply truck to the storage area. EE felt a pop in right bicep muscle and was diagnosed as a torn bicep.

Employee was watering batteries and had the lid held up with a rope hanger. Another employee working on the other side closed the lid and the hanger came loose and the lid fell and hit EE's right hand resulting in a fracture to a bone in the top of the hand.
Employee was helping set a belt drive. He was trying to remove a jack from under the drive and while moving it back and forth, the jack bar came out and struck him in the right jaw, causing a contusion and chipping a tooth.
Employee was helping install belt structure when his left hand was caught between two pieces of belt structure and caused a cut to the wrist of the left hand, requiring 5 stitches. He was wearing gloves at the time of the incident.
While traveling up the steep grade the engine stalled out or quit. The driver shifted the transmission out of gear allowing the truck to travel backwards down a steep grade. Driver jumped from the truck which turned over and caught on fire.
Employee was installing a wheel unit on a shuttle car and as he and his helper were aligning the wheel unit, he felt pain in his back.
The employee was helping make a belt splice. While attempting to remove the belt clamps and rails, he received an avulsion to the end of his left hand index finger when it became pinched between the belt clamp rail and the belt clamp.
Employee reported he was loading six inch hard waterline. A piece of waterline rolled down from the stack and pinned his left leg. He twisted his leg in order to free himself, resulting in a left hip strain. He continued to work until January 1, 2015, after being transferred to a new mine, and started missing on 1-2-15.
