Drill steel was hung in drill pot, employee engaged rotation and drill steel struck employee on left shoulder and ear causing a laceration to his ear.

The employee was pushing a rock out of the way with the stab jack on the roof bolter. The rock either broke or the stab jack slipped over top of the rock, causing the drill head to move forward quickly catching his left thumb between the drill steel and the canopy post.

The individual was operating the continuous miner and positioned outby the boom of the miner wearing safety glasses, when an object was thrown by the cutter head, striking himi in the head above the left eye. This is a medical only report.

The EE was observing the operation of the CM when a piece of rock and coal came off the head of the CM and struck him in the left eye. Some material was embedded in his eye and had to be removed. This is a medical only treatment. No lost or restricted work days.

The individual had been grinding in the shop area. When he finished he rubbed his eyes with his hands and got a sliver of metal in his right eye. He was wearing prescription safety glasses while he was grinding. The individual received prescription medication for his eye.

The individual had his back to roof bolter waiting on miner to exit #5 entry. When miner trammed out of the way, the roof bolter trammed forward with the T-bar of the roof bolter catching the toes on the right foot.

Injured was stepping across the cable to the roof bolting machine. When the cable reel of the machine was turned on, the cable caught his foot, causing him to fall and bruise his ribs.

He was welding in the surface shop and the prolonged exposure to the welding flashes resulted in flash burns to his eyes.

EMPLOYEE WAS MOVING MINER CABLE AND CO-WORKER PULLED THE CABLE TIGHT WITH A BOOM, PULLING THE VICTIM INTO THE ROOF.

EMP WAS SPOTTING A BOLT WHEN A PIECE OF SLUG ROCK FELL ONTO THE CAMOPY OF THE ROOF BOLT MACHINE AND ROLLED OFF CANOPY ONTO HIS FOOT AND LEG.

EE WAS DRILLING A HOLE FOR A ROOF BOLT AND MASHED HIS LEFT INDEX FINGER WHILE USING TWO PIECES OF STEEL.

EMPLOYEE WAS INSTALLING ROOF BOLT AND STRAINED GROIN. HE WAS PUSHING WHEN HE FELT THE PAIN.

EMPLOYEE WAS BOLTING TOP AND A PIECE OF DRILL STEEL FELL ON FINGER.

EMPLOYEE WAS PUTTING UP FIRST BOLT IN THE LAST ROLL WHEN ROCK ROLLED OFF RIB AND HIT HIM ON THE ANKLE.

EE WAS BOLTING & A SMALL SLUG OF ROCK FELL & SCRATCHED HIS HIP. FEDERAL INSPECTOR WAS ON JOB & CHECKED PLACE.

EE WAS BOLTING & A SMALL PICK OF ROCK STRUCK HIM IN BACK ON 03-31-94 AT THIS TIME IT WAS NOT A LAST TIME ACCIDENT ON 04-27-94 EE WENT TO DOC & WAS PLACED ON LIGHT OR NO DUTY DUE TO BACK PAIN I S SCHEDULED TO RETURN 05-05-94 MISSED 5 DAYS CAME BACK FULLY RELEASED

EMPLOYEE WS BOLTING TOP WHEN A SMALL ROCK FELL BETWEEN T-BAR AND CANOPY AND STRUCK HIM IN THE LOWER BACK.

EE WAS BOLTING TOP AND SAID HE PULLED A CHEST MUSCLE TAKING HIS STEEL OUT OF HOLE. CAME OUT & TOOK TO DR.

EE MASHED HIS LEFT THUMB WITH DRILL STEEL. DID NOT MISS ANY WORK. WENT BACK IT CHECKED AND CARE BACK TO WORK.
