The continues miner was shearing a corner when the cutter head struck a rib bolt breaking the head off and sending it across the mining machine striking ee on his upper lip resulting in a laceration that required 9 sutures.
While welding rock guards on a new truck bed, weld splattered & burned left forearm & elbow.
The employee was drilling holes into the rib so rib bolts could be installed, when the steel struck him on the right forearm
Employee was installing roof bolt, had placed drill bit against top, started rotation when glove on right hand got caught by drill steel twisting his right hand. Employee sustained fractures to his right wrist.
Injured was operating roof bolter installing roof bolts when drill steel hit him on his left forearm. resulting in a fracture to forearm.
OPERATING BULLDOZER. DOZER ROLLED OVER WHEN EE LOST CONTROL WHILE RECLAIMING STEEP SLOPE. THE DOZER GOT ONTO A TREE TRUNK WHICH CAUSED IT TO SLIDE. EE WAS WEARING HIS SEAT BELT; HOWEVER, HE STRUCK A HARD SURFACE INSIDE CAB, CAUSING AN INJURY (LACERATIONS TO FACE).
Injured employee was supervising and assisting co-worker who was welding a cover to the end of a steel pipe. While in close proximity to welding arc, injured employee unknowingly exposed his eyes to excess radiation, thereby incurring minor eye burn.
Employee was drilling a roof bolt hole, while doing so, he lost his balance and caught his hand on rotating chuck of drill, resulting in twisting injury to ee's right forearm.
EMPLOYEE WAS HELPING TO WORK ON A DRILL, WHEN A ROCK FELL FROM THE TOP STRIKING HIM.
EE WAS INSTALLING BOLTS IN THE #4 ENTRY LEFT CROSSCUT, EE WAS OPERATING OPERATOR SIDE OF ROOF BOLTER WHEN A PIECE OF ROCK FELL FROM TOP STRIKING CANOPY, BOUNCING OFF CANOPY STRIKING HIM ON HIS LEFT ANKLE. EE COMPLETED WORK SHIFT. WENT TO DR ON 1-10-03. X-RAYS OF HIS ANKLE REVEALED FRACTURE TO HIS ANKLE (LEFT).
EE STATED WHILE IN THE PROCESS OF DRILLING OUTSIDE HOLE IN #2 LEFT CROSSCUT, WAS ADDING A PIECE OF DRILL STEEL WHEN A ROCK FELL FROM THE TOP STRIKING HIM ON HIS LEFT HAND CATCHING HIS MIDDLE FINGER BETWEEN ROCK AND DRILL POT. EE SUSTAINED AN AVULSION AND FRACTURE TO THE TIP OF HIS MIDDLE FINGER. EE WAS WEARING METACARPAL GLOVES.
EE WAS IN THE #7 ENTRY MINER HELPING WHEN A RIB ROLLED OFF STRIKING EE ON HIS RIGHT SHOULDER. EE SUSTAINED CONTUSION/BRUISES TO HIS NECK, UPPER BACK, LOWER BACK AND RIGHT SHOULDER.
AS INJURED EE HELD MACHINE BRACKET FOR TACK WELDING, A MINISCULE HOT PARTICLE STRUCK INJURED EE IN LEFT EAR.
KNEELING TO WELD WHEN MOVING TO STANDING POSITION RIGHT KNEE POPPED.
EE WAS MINER HELPER IN GOB EYE OFF #7 ENTRY WHEN A RIB ROLL STRUCK EE IN THE BACK. EE SUSTAINED FRACTURE TO THE T-12 VERTEBRA. SECTION WAS ON RETREAT MINING ("A" SECTION).
EMPLOYEE WAS BOLTING IN #1 HEADING WHEN A PIECE OF DRAW ROCK FELL FROM BETWEEN BOLTS, STRIKING HIM ON HIS LEFT SHOULDER AND LEFT HAND. EMPLOYEE SUSTAINED FRACTURES TO BACK OF LEFT HAND.
