EE turned the small coal sieve to retrieve any magnetite from the water where coal had been washed. This sieve rests on a pivot stand and is turned weekly. EE was placing the sieve into its working position when he strained his back. He continued to work until 6/2/14 when he sought medical attention. EE was taken off work effective 6/3/14.

EE was repairing the discharge line on a Floc transfer pump. Floc was present on the floor and he slipped and struck his head on an elevated piece of concrete. EE sustained a laceration requiring 5 staples to close the wound.

EE was cleaning crusher chute when coal broke loose covering his feet, causing him to fall back hitting his head and right elbow on grating which resulted in a laceration on his head requiring 10 stitches.

Employee stepped from ladder on top of tank when the tank collapsed. He grabbed the edge the tank, cutting his fingers and resulting in three stitches.
EE was cleaning out chute, when his hand slipped on round bar and cut his finger, requiring stitches.
Employee was using a slate bar to close the door on a railroad car when the slate bar broke about ten inches from the end. It struck him on the right lower leg causing a fracture to the tibia and fibula.

Was trying to dislodge rock in wash box reject chute with a slate bar when his hand hit the edge of chute opening causing a laceration on his finger requiring stitches.

Injured employee was supervising and assisting co-worker who was welding a cover to the end of a steel pipe. While in close proximity to welding arc, injured employee unknowingly exposed his eyes to excess radiation, thereby incurring minor eye burn.

AS EMPLOYEE WAS WALKING INTO A SHALLOW RAVINE HE APPARENTLY LOST HIS BALANCE ON UNEVEN TERRAIN AND FELL.

EMPLOYEE EXITED TRUCK POSITIONING HIMSELF ON THE GROUND NEAR A PENDANT MOUNTED CONTROL STATION FOR AN ELECTRICALLY OPERATED GATE WHICH PERMITS LOADING OF COAL COARSE REFUSE FROM A 175 TON STORAGE BIN. AS EMPLOYEE OBSERVED LOADING OPERATION, A SMALL PARTICLE OF ROCK BOUNCED OFF THE TRUCK STRIKING HIM IN THE RIGHT EYE CAUSING AN ABRASION. WAS NOT WEARING SAFETY GLASSES.
