EE was operating a drill press in the welding bay to drill holes into a bracket, EE secured it in a vice. While standing to the right, EE held the vice with EE's left hand and operated the press with EE's right. The bit bound, causing the bracket to spin out of the vice which caused EE's left thumb to be caught between the vice and bracket.

Drilling with a jack leg drill. EE removed drill pressure and the drill dropped but the drill steel stuck in the roof. EE grabbing the top of the drill to keep it from falling when the drill steel fell from the hole, striking EE's finger. EE was taken to the ER and diagnosed to have sustained a laceration and fracture to the right index finger. It required stitches and a splint.

While cleaning out dust hopper EE removed the guard and was working on unplugging the hopper when EE dropped EE's knife into the hopper. EE had thought that EE had turned off the hydraulic power and hydraulic lever on the agitator. When reaching into the hopper to get EE's knife the agitator came on and caught EE's right arm between the agitator and dust hopper.

On August 18th at approximately 4:00pm employee was advancing a shield over the shearer in a effort to support rock that was falling out from the roof and face. They lowered the shield adjacent to the one they were operating from, when rock that was approximately 8"x8"x 12" thick struck them on the left hand causing injury to their left hand.

Working in a 2-man crew, driller opened rod clamp while chuck was not engaged with 300 ft of drill rod in the horizontal borehole. Water flow from the borehole pushed the free rods backwards into the helper behind the drill rig, striking their right side and causing bruising.

On 6/27/2025 EE was hit by a softball size piece of coal coming off shearer drum. CT scan was negative, and EE returned to work. On 7/5/2025 EE was diagnosed with pleurisy. Workers Comp investigated alleged diagnoses and could not determine if it was related or not. They accepted the claim making the incident reportable on 7/22/2025.

Employee was proceeding to advance TG shields from transition as face was advancing and a chunk of coal fell out and hit in the back causing them to accidentally fall forward causing injury to their leg.

While EE was running the miner the face was popping and a piece of coal came off the face and hit EE in the mouth causing one of their front teeth to get chipped.

While advancing shields, Shield 159 got hung up, EE went to climb over leg ram to assist Shield 159 on advancement and left sided foot slipped in between the leg ram and spill tray on pan then the pan rocked back from shield 159 hanging up and pinching foot between the ram and tray.

While EE was bolting on the right side of the bolter, they were drilling the outside 7' hole and while doing so they dropped the drill box causing the wrench to fall out. They lowered the box and leaned over to pick up the wrench, they placed their left hand on the drill box at which time the steel came out of the roof and hit their left ring finger against the drill box.

Employee held onto the end of a cap piece while jaws on can setter closed. The tip of right hand thumb was pinched.

While installing a 7' roof bolt in the first row of the graveyard shift, the operator dropped the bit insert on the ground. The operator got off the bolter and grabbed the bit then got back on the roof bolter boom and was in the process of placing the drill still in the drill chuck. While doing so, the operator closed the hydraulic jaw pinching right-hand fingers

Injured was polishing a 22" long, 2" in diameter pin on the shop lathe. EE was using a 4 foot long piece of emery cloth. When the emery cloth was worn, EE choked up on one end of the emery cloth to have better grid on the emery cloth. As EE was working the emery cloth back and forth, EE got EE's thumb too close to the bar and the rotating bar pulled EE in and broke EE's left arm.

EE was helping set shields, they spun the shield and it needed to be pumped up, so they stopped and set the milk to the shield, EE had right hand resting on the shield. When they set the milk to the shield the side seal pin popped out and crushed the ends of EE's middle and ring fingers.

EE was using a handheld cordless bandsaw to cut rib bolt into 1 foot lengths. EE was holding the bolt with the left hand and was using the saw with the right hand. When the blade contacted the bolt, it rolled up the bolt and cut EE's hand. EE was wearing cut resistant gloves. Using the gloves greatly reduced the laceration.

While installing a rib bolt, the wrench broke and hit operator over the right eyebrow causing a laceration requiring 8 stitches.

EE was installing 7' roof bolt. Bolt was pushed into hole most of the way. The bolt bent so EE adjusted drill mast to straighten bolt. When EE tried to push it in again, the wrench broke and hit EE in the cheek. This caused fractures to EE's cheek.

Miner was breaking up a 3'X3'X2' rock on the shield toe with a Jack Hammer. The tip of the Jack Hammer slipped and the Jack Hammer went forward causing the miner to pinch finger between the Jack Hammer handle and the rock.

Repairing feeder breaker on surface. EE needed to remove pin to adjust bracket on hydraulic jack mounted to tracks. The pin in bracket was bound up. When EE air-arced through pin, EE thought that the jack and tracks would drop to ground. However, the tracks came up when the pin was cut. A different bracket on tracks struck EE in welding hood causing lacerations to forehead.

EE was working in adjacent maintenance bay servicing a piece of equipment. Metal push pin from adjacent maintenance bay bounced off ground and struck EE on the lower leg.

EE could not get two sections of drill steels to separate. EE bounced the steels against the drill box, they separated and the bottom steel came back and struck EE on chin causing laceration.

EE roof bolting, when two drill steels came together, they pinched the palm of EE's left hand causing a laceration.

Employee was assisting on the roof bolter. While the right side operator was putting in a rib mesh, injured tried to grab a bundle of plates that had slid under supply tray. At the same time the operator lowered the mast and pinched the employees hand.

GMS employee was retrieving a bag of rockdust near the rear of the bolting machine and the rear of the bolting machine was lowered causing injury to the employees foot.

EE was installing cable bolts using two-piece steel. EE had removed the driver portion of the steel combination and the finishing 6' portion of the steel fell from the hole striking EE's right hand against the drill extension wrench. This resulted in a fractured right ring finger.

EE was helping load roof bolting supplies on the back of a roof bolter. The roof bolter operator was positioning the machine and lowered the back end of the roof bolter on EE's left foot pinching the foot between the machine and the floor.

The roof bolter operator was removing his drill wrench from the chuck when he inadvertently got his hand too close to the auger clamp. As he pulled the lever it caught his right finger between the auger clamp.

EE installing a rib bolt through a piece of rib mesh when the screen came down and pinched his right middle finger against the drill mast.

Injured was turning off the water sprays on longwall shearer when a rush of air pushed him down causing him to strike his knee on a shield toe. He was taken to an emergency room where x-rays proved negative.

Injured employee was helping install roof bolts in a coal face during longwall recovery operations. His left index finger became pinched between the head of a roof bolt and a bearing plate causing the finger to be severed at the end near the first knuckle.

While operating Hilti Fastener Gun, employee placed his hand over barrel and gun fired.

Employee was installing the primary roof support with a jack leg drill. The air hose came loose striking him in the right hand.

Injured was assisting bolter operator. While waiting for operator, injured rested his arm on rack. When operator retracted boom the injured's finger was in pinch-point

Injured inadvertently activated jaw on roof bolter and caught his thumb in jaws

Section mechanic positioned himself on the right side of the roof bolter to begin repairs on a bantam rockduster. The right side bolter operator finished installing two rib mesh bolts and swung drill mast in and back in order to move roof bolter. Left side bolter operator found mechanic caught between bolter frame and right side drill mast that was swung back in place.

Cutting coal on the longwall face when a small piece of rock flipped up and struck his lip.

EE was repairing the CM, a cover measuring 30"L x 16"W x 1/2" thick had been lifted into a vertical position.EE was on top of the machine and needed to get additional parts for the repair when he jumped off of the miner and his right hand was pinched between the cover and the frame when the cover fell.

The miner operator was rockdusting the face with a machine mounted hydraulic Bantam duster. The dusted became plugged with damp rock dust, so the operator removed the guard to clear the plug with his hand. In the process he accidentally leaned against the toggle switch on his remote control, activating the auger.

In process of loading shield on to shield hauler. Sling was already hooked into shield toes. Injured person was holding on to sling ring with left hand and then grabbed winch hook to hook to sling. Hook twisted under pressure and crushed fingers.

Installing a rib pin and was in the process of lowering his canopy and swinging his boom in. When the canopy lowered the collar of the canopy caught the wrench protruding from the tool tray. The wrench flipped up and struck his lip.

Injured was removing a leaf spring from a pickup truck. He was using an impact wrench on the bolt head & an end wrench on the nut to keep it from spinning. There was a keeper tab associated with the nut that keeps the nut from spinning. The keeper tab spun as the impact wrench was activated and the keeper cut through leather gloves lacerating the fingers.

He was in the process of installing an upper leg keeper pin on shield No. 141. As he was attempting to insert the pin the shield took weight and the pin blew out and struck two fingers on his left hand.

Injured had just finished installing a resin roof bolt and placed the drill steel in the drill box to drill the next hole. He then released his grip on the drill steel and activated the steel clamp lever to close the clamp. The clamp closed very quickly. The injured EE's left index and middle finger were closed inside clamp. 3 sutures to close index finger.

He was using a rock drill to bolt down the 7th East head roller steel. He had his left hand on top of the steel and the drill fell over and hit him on the left finger.

Inj EE was preparing to instll a cable roof bolt into the roof. As injured employee reached to remove drill steel from drill box, he activated the wrong hydraulic lever & this action closed the drill steel clamp on his left thumb.

He was in the process of installing a belt hanger with a Fletcher Roof Bolter. He was guiding the bolt wrench onto the head of the roof bolt when his hand slid and his little finger was pinched between the belt hanger and the auger clamp.

Employee had been training on the bolter for 2 days. He had drilled a hole and was preparing to put the resin bolt into the hole. Employee stated that he thought the sleeve on his rain coat caught on the top of the drill mast and that brought his left hand in line with the steel clamp at the same time he activated the lever which closed the steel clamp. Sutures required.

Drilling roof bolt holes in face of 4LPE longwall recovery room. The first portion of the hole had been drilled and a 6' finishing steel was inserted into the hole to complete the drilling phase. The drill was being positioned to line up with finishing steel when 6' steel fell from the hole and struck him in the right index finger, pinching it.

Roof Bolter Operator was installing a rib pin. With the wrench in the chuck, he reached out to put the head of the bolt in the wrench as he was activating the feed lever. He pinched his finger between the clamps and the rib pin plate.

EE WAS INSTALLING ROOF BOLTS IN RIB TO SECURE WIRE MESH. BOLT WOULD NOT TIGHTEN AND EE BEGAN TO BRING THE BOLT OUT OF THE HOLE. THE BOLT HEAD WAS STUCK IN THE DRILL WRINCH. AS EE REACHED FOR THE WRENCH, EE INADVERTENTLY ACTIVATED THE DRILL ROTATION AND THE BEARING PLATE STRUCK EE IN THE FOREHEAD
