Employee cut finger while grinding resulting in a laceration that required stitches.

Technician pinched EE's thumb while installing a pin retainer plate causing a laceration that required two stitches.

Technician was using a 6" grinder to blend a weld. Grinder kicked back and the grinder wheel came into contact with the back of their left hand between index and middle finger knuckle.

Shovel faulted making contact with truck on 02-15-2023 and the employee was seen on 02-16-2023 and did not return to work.

Shovel faulted making contact with truck on 09-29-2022. The employee did not return to work.

Replacing a dragline wear liner the employee was unable to see a developing crack on the opposite side of the plate due to a welding hood. A preexisting weld failed to hold plate due to age causing the plate to fall & injure the employee. Employee was found to have a fractured toe and a laceration resulting in sutures. Employee returned to work for employee's next scheduled shift.

Employee began to feel discomfort in left eye. Employee was taken to the optometrist where a small rust ring was removed. No object was imbedded. A prescription for a steroid was given.

While performing welding duties employee received a laceration from an electrical grinder requiring stitches

Contract employee was arc gouging wear plate from a dragline bucket and as employee finished the cut, the piece of material dropped down contacting employee's lower left arm resulting in a laceration requiring stitches.

IE was performing welding repairs to a shovel car body on Wednesday and went on days off. Employee began to feel discomfort on Thursday evening then woke Friday morning to a red irritated eye. Employee was accompanied to the optometrist where a small FOB and rust ring were removed from the left cornea.

While operating a motor grader, the moldboard caught a rock jarring the operator side to side. The IE felt a pop in between his shoulder blades. The IE was transported to the hospital for evaluation, diagnosed with a back strain, released to work as tolerated, and referred to an orthopedic doctor. The IE worked until 11-8 when the discomfort would not let him finish the shift.

Employee was performing welding and grinding and at an unknown time had a piece of metal slag enter his eye.

Employee was traveling down a haul road in a rubber tired dozer, hit a bump in the road causing employee to bump his head causing a laceration.

Employee was repairing a wheel when his left index finger was pinched between rad gun extension and the wheel. The next day he went to ER and received 8 stitches.

Employee was removing bolts with hydraulic wrench, bolt broke while removing the wrench recoil hit his right hand cutting the ring finger & fracturing bone in ring finger.

Individual was cutting a metal shim with a hole saw. The saw caught metal and kicked back against individuals thumb causing lacerations.

While welding the Employee felt something in his eye, on evaluation at Jensen Eye Care, the doctor discovered a foreign body embedded in Employee's eye. At the time of incident Employee was wearing all proper PPE.

While operating a motor grader, the moldboard hit a rock and the employee began to experience back pain. Employee suffers from arthritis and pain coming from the facet joint that is behind the lumbar 4 and 5 vertebrae. Employee did not return to work.

Employee states that while being loaded by 106 shovel, a rock jarred the haul truck causing neck pain. Medical evaluation including X-rays and CAT scan revealed no injuries. The employee was not released to return to work for 3 days. Witnesses did not see this event.

While welding the injured felt something in his right eye. Upon evaluation at the Wright Clinic it was discovered the object was embedded. At the time of the incident the employee was wearing a welding hood with prescription safety glasses with side shields.

Employee was welding and a foreign body enter his left eye.

Employee got something in his eye during the work shift. Initially it didn't bother him. On December 2nd he was seen by an eye doctor due to irritation. A piece of metal was removed from the left eye, he was prescribed medication for the eye and given a full duty work release.

Employee was using a hand held grinder when a piece of the grinding stone broke off. The projectile struck the face shield he was wearing and deflected into his left cheek, the resulting laceration required 8 stitches. The investigation revealed the grinding stone was properly rated for the grinder and all guards were in place.

Employee was using a pneumatic torque wrench to tighten the U bolts on leaf springs. He was holding the wrench in his right hand and stabilizing the bolt with his left hand. He inadvertently had the gun set to loosen instead of tighten, when he pulled the trigger the gun moved in opposite direction than he anticipated pinching the palm of his left hand. Resulting in stitches.

A plant technician was dressing the ends of a 2 inch by 2 inch by 8 inch piece of angle iron on a bench grinder. His left thumb contacted the grinding wheel. The injury required minor surgery making this reportable.

Employee was using a volt meter to test the alternator voltage on a 16 G motor grader. His foot slipped causing him to lose balance. As he tried to catch himself his left hand contacted the fan, resulting in multiple fractures and lacerations of his hand and fingers.

Employee was using a Hytorc hydraulic wrench to torque bolts on a D10 dozer. A seal within the tool failed injecting hydraulic fluid into the palm of his hand.

The shovel was loading a haul truck when it faulted out causing the bucket to contact the bed of the haul truck. The operator of the haul truck suffered neck pain and has been monitored by a physician while continuing to work. The employee underwent a series of tests on 05/21/09 and was diagnosed with 2 ruptured discs.

While loading at the shovel a truck driver was jarred by a rock that fell from the bucket resulting in a back strain.

Employee's haul truck was jarred while loading at the shovel. He complained of back pain. Physician diagnosed Spondylosis, a degenerative illness. He saw an Orthopedic Specialist on 11/11/08 who prescribed physical therapy.

While using overhead crane to lift side frame, load shifted and struck employee resulting in bruising and restricted duty.

Employee was cutting 2-1 1/4 inch poly pipe-when cut was made the other end of the pipe nipped up and cut his on the lip.

Employee was positioning a guard during a move, backhoe bucket crushed his hand between the bucket & the guard. Two middle fingers on his left hand were amputated and the left index finger sustained a severe crush injury.

Employee was using a grinder on a pipe. The grinder hit a burr on the pipe and bounced back hitting him in the forehead.

While lifting a drill pipe with an overhead shop crane and web sling, individual pinched fingers between sling and drill pipe resulting in tuft fractures of the three small fingers on the individual's left hand.

Employee was removing the torque converter from a rubber tire dozer using an overhead crane. An eye bolt broke causing the sling to fly up and hit the employee in the face around the left eye resulting in stitches.

Employee was drilling a hole when the bit locked in the channel. The handle being held with his left hand broke causing the drill to rotate striking & breaking a bone in his right hand. Employee did not seek medical attention until 3 days later. He is working modified duty & was given prescription medication.

At the time of the incident the technician was removing tire lug nuts from a CAT 793 off-highway truck. As the technician was lifting a 3/4" impact wrench into position above his head he heard a "pop" and felt pain in his right shoulder.

EE had to use larger grinding wheel on grinder. Guard wouldn't fit over it. Grinder wheel blew up and a piece hit his middle finger of left hand, causing the laceration.

At the time of the incident employee was washing the differential on a CAT 793 off-highway truck. When he engaged the high pressure wash wand the ladder tipped. This caused him to fall to the ground, landing on his right arm.

A maintenance employee was removing a transmission pump from a D11 dozer. Another maintenance employee was running the overhead crane to assist with the removal. While hoisting the pump, it became temporarily jammed, causing the pump to shift, pinching the employee's right hand, resulting in 14 stitches.

On 07/07/06 employee felt eye irritation while grinding on a dozer. Employee saw a physician on 07/11/06 and was diagnosed with an embedded particle which was removed. Employee was wearing safety glasses and a face shield.

Technician was using a Camtac 13" Metal Lathe to machine a retainer for a forklift. The employee was resting his hand on the tool lock when a metal shaving struck his right middle finger resulting in a laceration that required stitches.

Gas leak in Revolving Frame was ignited, causing flash fire.

While being loaded, the shovel came in contact with haul truck bed, jarring the truck. EE was seen in ER initially, then saw Dr. on 2/21 with diagnosis of neck strain.

EMPLOYEE WAS USING AN AIR POWERED DIE GRINDER TO GRIND OUT A RACK IN A TORQUE TUBE. THE BIT PIERCED THE INNER WALL OF THE TUBE CAUSING BACK PRESSURE. THE PRESSURE BLEW THE DEBRIS UP UNDER THE EE'S HOOD & SOME METAL WENT INTO HIS LEFT EYE.

Employees were pulling a 250'-14" PVC Water pipe. They were pulling the pipe between a stationary pipe and another length of pipe using the rear bucket of a backhoe. As the pipe compressed between the stationary stand it popped out and struck the injured employee below his knee.
