While grinding on a haul truck bed a piece of metal got into employees eye. I.E. was taken to hospital where foreign particle was removed. Employee was wearing safety glasses and face shield.

Mostrando los 21While removing cutting edge on a 637 CAT scrapper, the rad gun IE was using, slipped off nut and contacted IE's mouth resulting in a broken #7 lateral incisor tooth.
IE was drilling hole to install a ladder. Drill bit became stuck causing IE's LT. wrist to twist resulting in a fractured wrist bone.
IE was bounced while operating a 24H blade and began to experience lower back pain. He was diagnosed with severe arthritis. He received an epidural injection on 03/12/2015 making this a reportable illness.
Operator was collecting tools required to start the repairs inside of the shop. When operator left his work station he turned around striking a tool with his mouth. This resulted in the laceration of his upper lip and did require sutures.
Employee was air arcing on a Letourneau Loader bucket bores. Employee flipped his welding hood up and felt something hit his eye. Employee thought that the object was not in his eye and went home. The following night the employee's eye started to hurt and informed his supervisor. Employee was taken to Eye clinic where metal chips were removed.
Blade operator backed into the water truck after pulling a rock out of the roadway. The operator could not return to his next scheduled shift after seeing an orthopedic doctor.
A shovel door latch bar fell over onto the employee's foot. Consulted an orthopedic doctor on 9-8-11 who confirmed a fracture of the second metatarsal on the right foot. The employee was given a restricted work release.
The employee was rolling up a fuel hose using an air driven reeler, when the hose (which was kinked) straightened out, twisting the end of the hose. The right hand of the employee stayed in the handle on the end of the hose, causing the wrist to twist, causing a hair line fracture to a carpel bone in the right wrist.
The employee was helping hold the drill steel in line with the guide roller on a DMM2 blasthole drill. As the carousel moved into position, the steel kicked to the side striking the left gloved hand resulting in a simple fracture of the ring and middle finger. The was seen at the hospital, the fingers were splinted and the employee returned to work.
Employee was using a small grinder to dress mushroomed metal on an R.R. machine. The grinding wheel separated and a shard from the wheel struck the employee in the neck, puncturing the skin.
Operator attached a wiggins fitting to fuel the equipment. When the nozzle was turned on, the fitting came off, spraying fuel for split-second. Some fuel was sprayed on operator with some getting in the eyes. Eyes were flushed & the EE returned to work. The next morning the EE went to see the doctor where anti-biotic medication was prescribed. Safety glasses were worn.
AFTER GRINDING WELDING SLAG EARLIER IN THE DAY, EYE STARTED TO HURT THAT EVENING AT HOME. WENT TO HOSPITAL NEXT MORNING. DIAGNOSIS - FOREIGN BODY (RUST & SLAG) ON CORNEA OF LEFT EYE.
WHILE REMOVING RUBBER FROM A PULLEY WITH A HIGH SPEED GRINDER DEBRIES WAS EVERYWHERE. RUBBER LODGED IN EE EYE. EE TRIED TO REMOVE PARTICLE WITH EYE SOLUTION. EE USED OTC DROPS FOR SEVERAL DAYS BUT EYE DIDN'T GET BETTER. SAW DR 02/15/05 TO HAVE PARTICLES REMOVED.
WHILE HOISTING THE RIGGING OUT OF THE SHOVEL HOUSE, A CLEVIS ATTACHED TO THE END OF THE SLING CAUGHT ON A BEAM ENOUGH TO "WHIP" THE SLING AND THE CLEVIS STRUCK EE ON THE LEFT EYE OF THE SAFETY GLASSES. THE IMPACT CAUSED THE GLASSES TO CUT THE SKIN ABOVE THE EYE. 4 STITCHES WERE REQUIRED.
While removing duels from a Komatsu haul truck employee got his right ring finger caught between hub and Rad gun, causing an open tuft fracture requiring 9 stitches. Employee was wearing gloves due to the cold temperature while working outside, therefore he did not notice his finger being in wrong place between the hub and Rad gun.
WHILE BEING LOADED UNDER THE SHOVEL A LARGE CHUNK DROPPED JARRING HAUL TRK. 20 MINS LATER EE REPORTED THAT HIS BACK MUSCLES WERE GETTING TIGHT. HE SAID HE HAD BEEN IN SEVERAL AUTO ACCIDENTS HIS BACK WAS BROKEN 2-3 YRS AGO & REINJURED INA HEAD ON 5/04. DR DIAGNOSED MUSCLE STRAIN & RX'D PHYSICAL THERAPY. NO VISIBLE DIFFICULTY CLIMBING DOWN LADDER OR WALKING
While torqueing the left rear wheels on a Cat 797 the rad gun slipped off the lug nut striking employee on the right side of the face. The impact caused a fracture of the lower right cheek bone.
EMPLOYEE WAS REMOVING THE LEFT REAR OUTSIDE TIRE OF UNIT 622 TO BE ABLE TO WORK ON THE INSIDE DUEL TIRE. WHEN REMOVING THE LUG NUTS HE WAS MOVING THE RAD TOOL BETWEEN THE LUG NUTS USING HIS LE FT HAND TO GUIDE THE TOOL ONTO THE NEXT NUT HE ACCIDENTALLY HIT THE TRIGGER PINCHING HIS LEFT INDEX FINGER & HAND BETWEEN THE TOOL & THE WHEEL. THE TOOL CAUSED A LACERATION OF THE LEFT FINGER
EMPLOYEE WAS WORKING WITH A CONCRETE CORE DRILL BEING CONTROLLED THROUGH A FOOT-OPERATED SWITCH. WHILE CHANGING A BIT HE STEPPED INTO THE FOOT SWITCH. THE BIT CAUGHT HIS GLOVE, TWISTED HIS WRI ST, CAUSING A SPRAINED WRIST. A SPLINT IS NOW BEING WORN.
EE WAS OPERATING A D-11 DOZER WHEN A PIECE OF DIRT GOT INTO HIS EYE; HE TRIED TO FLUSH IT OUT WITH VISINE FOR TWO HOURS BEFORE REPORTING TO HIS SUPERVISOR. AFTER FLUSHING EYE AT THE FIRST AID ROOM, IT WAS DECIDED TO TAKE HIM TO THE E.R. TO GET IT CHECKED OUT.
