The injured was pushing a drill steel into the roof while roof bolting and felt pain in his abdomen. The injured was diagnosed with a hernia. His first missed day was 03/04/2015.

Struck elbow on mast when pulling stuck steel out causing a bruise.

As injured was dropping the drill mast the steel came out of the chuck and sprung back toward his right hand. Injured apparently reacted by jerking his hand out of the way and inadvertently placed his hand on the frame of the drill mast and he dropped the drill clamp on his hand as he lowered the drill mast.

While installing truss bolts the injured was pushing a drill steel in a hole when he felt discomfort in his right shoulder. After a doctor's exam and a MRI it was determined that the injured has degenerative conditions that require surgery. This incident is being reported as an occupational illness's. The injured worked toll 9/11/13 when he went off for surgery.

EE was bolting on the miner. The drill steel came around as it rotated and slapped him in the right hand pinching it between the drill steel and the miner causing a laceration that required 6 sutures.

Employee received a compound fracture and 8 stitches to his right middle finger when he caught his hand between the bolter head an the mast as it dropped.

Employee was running continuous miner and loading coal onto a pile when a piece of rock approx. 1' x 1 1/2' and 8" thick struck his right foot just above the metatarsal when the rock rolled off of the pile.

Cut stump off tree and footing slipped and strained lower back.

Employee caught ring finger on right hand between drill steels while bolting on the right side of a continuous miner. Employee lacerated the finger.

Shearer operator was trimming the top and a small piece of sandstone flew from the cutter head, requiring five (5) sutures. Employee was wearing gloves.

He was advancing the slider tube when the tube fell, striking a strap on the strap jacks, the end of the strap struck him above the left eye causing a laceration requiring 4 sutures.

While backing up the continuous miner, the right side of a strap resting on the strap jacks, struck the rib, causing the left side of the strap to fly and hit him on top of the left hand causing a laceration requiring 3 sutures.

While bolting on the right side of the continuous miner the drill steel fell out of the roof and struck him on the right foot causing a fracture to the third toe.

He was operating the shearer at the headgate area when a piece of rock came off the drum and struck him on the left ring finger causing a laceration requiring 3 sutures.

He was using a grinder on the frame of a rockduster when the grinder kicked back and the guard for the grinder struck him in the upper lip causing a laceration requiring 9 sutures.

He was leaning against the rib with his left foot against the rib and his left knee at the same height. As the continuous mining machine was trammed forward his left knee was caught by the dust box causing him to twist his left ankle. He sustained a fracture to the left ankle.

He was drilling a hole with a Cobra Drill, as he was attempting to finish the hole the steel fell out of the roof and struck him on the left hand above the little finger. He was removed for surgery to repair a fracture on 12/11/2010.

EE was standing back from the CM machine when a piece of material flew back and struck him on the left side of his neck causing a laceration requiring 8 sutures.

He was assisting the center bolter while installing 16' bolts. He was retrieving the drill steels, when the bolter operator dropped the drill head it caught his left foot under the drill head, causing a contusion to the left foot.

He was grinding a pipe nipple on a floor pedestal grinder, when the nipple caught the tool rest. A piece of the tool rest broke off and caused a laceration to the left middle finger first knuckle requiring 1 suture.

Employee was using a down gun to drill holes in the bottom, when he raised the drill gun out of the hole he felt a sharp pain in abdomen.

The continuous miner was backing up when straps got wedged between the roof and the machine. The machine was trammed forward, the strap came off the mining machine and struck his safety glasses which cut the bridge of his nose causing a laceration requiring 6 sutures.

He was backing up the continuous miner when a strap became wedged against the roof, buckled, then popped up and struck him on the left side of the face causing a laceration requiring 13 sutures.

He was attempting to recover a drill steel from the roof when the steel suddenly fell striking him on the back of the right hand causing a contusion to the back of the right hand.

EE was mixing concrete sluray when mix got into his eyes causing burning sensation.

He was attempting to swing the boom on 3018 bolter. When he activated the lever, the boom swung and caught his thumb on his right hand between the base lift jack and the boom causing a fracture to the right thumb.

He was using a grinder on a D-9 dozer, when the grinder kicked back and struck him on the left hand, causing a lacertion that required 4 sutures.

He was operating the longwall shearer, when a piece of rock fell deflecting water from the machine sprays going into his left eye, causing a traumatic cataract of the left eye. He continued to work at his regular duties until 4/22/09, when he was taken off work to schedule surgery to repair the traumatic cataract.

Employee was operating the longwall sheraer when he placed his right hand on the hand rail a piece of rock flew from the drum and struck his right ring finger causing a fracture.

He was assisting the center bolter while installing bolts. He reached across the bolter head to grab a stuck steel, his left foot was under the mast when lowered, causing a crushing injury to the left foot.

He was drilling a bolt hole, steels became jammed in roof, he put his right hand on top of the drill head to check the suction, a drill steel came out of the roof and struck him on the right hand causing a fracture and laceration to the right middle finger.

He was standing near the suction line of the C Mains centrifugal pump while the line was being filled. The line came apart at a coupler and struck him on the right side of the head causing a laceration to the left side of the head, concussion and skull fracture.

While applying torque to a bolt, the drill chuck and motor fell off and hit him in the left knee, causing contusions to the left knee and left rib cage. He continued to work at his regular duties and eventually went to a physician. He was removed from work on 9/10/2008.

He was operating the longwall shearer when a piece of rock fell from between the #12 shield, struck the tailgate drum, flew from the tailgate drum and struck him on the left hand, causing contusions to the hand and amputation of the tip of the small finger.-per call-loss of finger to back end of fingernail-per call W. Dennison.

He was troubleshooting a longwall shield when the shield pulled in on his left foot causing a crushing injury to the left foot.

He was advancing the #3 longwall shield when a chain became caught in the relay bar. A piece of chain broke off and struck him in the right chin causing a laceration requiring 3 sutures.

He was standing beside the continuous miner when a strap extension fell from the strap jacks and struck him on the left forearm causing a laceration requiring 5 sutures.

He crossed in front of the #23 shield as it was activated to move. The shield moved forward and caught his right leg below the knee between the shield pontoon and the conveyor causing a crushing injury to the right leg. The lower part of his leg was eventually amputated.

EE was bolting on the tube side of the miner. He was attempting to raise the tube for clearance when he felt a pop in his right shoulder. He was diagnosed with a right shoulder strain and possible rotator cuff damage.

He was pushing an 8 foot combination bolt into the hole, as he was pushing the bolt, the bolt caught a separation in the roof and he felt pain in his back.

Employee had a drill steel resting against the mast, when the mast was lowered the mast struck the drill steel. This caused the steel to strike him on the right foot causing two broken bones in his right foot.

Employee's right leg was caught between crane and coupler.

Foreign body entered right eye while grinding.

He had his left foot under the boom of the center bolter, the mast mistakenly was put down on his foot causing a fracture to the left big toe.

HE WAS HELPING INSTALL RIB PINS ALONG #1 ENTRY, THE DRILL STEEL AND HEAD WERE NOT ALIGNED. THE DRILL OPERATOR MOVED THE HEAD AND HIS LEFT RING FINGER WAS CAUGHT BETWEEN THE STEEL AND THE DRILL MAST CAUSING THE AMPUTATION OF THE TIP OF HIS LEFT RING FINGER.

EMPLOYEE WAS OPERATING A DOZER ON THE CLEAN COAL PILE TO ELIMINATE A KNOWN VOID. THE COAL GAVE WAY UNDER ONE CAT, CAUSING THE DOZER TO SLIDE SIDEWAYS INTO A HOLE. HE STRUCK HIS RT. KNEE AND RT. ELBOW AGAINST THE DOOR GLASS, CAUSING A CONTUSION TO HIS RT. KNEE & ELBOW. HE WAS LATER DIAGNOSED WITH A RT. TORN MENISCUS.

EE WAS COUPLING TWO DRILL STEELS TOGETHER WHEN THE STEEL BENT AND CAUGHT HIS RIGHT THUMB BETWEEN THE STEEL AND THE TRS JACK, CAUSING AN AVULSION OF THE LOWER PART OF HIS RIGHT THUMB.

HE WAS PUSHING A BOLT AND RESIN INTO A HOLE AND FELT PAIN IN HIS BACK.

WHILE ROCKDUSTING THE WORKING PLACE EE WAS CAUGHT BETWEEN THE RIB AND THE LOADING MACHINE, CAUSING A POSSIBLE FRACTURE TO RIGHT PELVIS AREA, LEFT RIB AND FRACTURE TO RIGHT WRIST
