Emerald Mine No 1 Coal

The employee was manually operating a GOAB switch when an overhead porcelain insulator broke into multiple small pieces. One piece fell and knocked off the employee's hard hat. A second piece fell and made contact with the right side of EE's head, near the hairline, resulting in a laceration that required medical glue for treatment.

Employee was diagnosed with bilateral carpal tunnel, requiring surgical repair. Surgery on the right wrist has been completed. The left wrist is unscheduled at this time. **Left wrist scheduled for surgery, completed on 03/26/2021.**

An unplanned roof fall occurred in a cross cut of the slope track extension.

The Emerald #1 Elevator went down at 10:25am on Jan 8th due to a broken buffer switch. The elevator was down until aprox 6:30pm when the issue was corrected and the elevator was back in service.

The hoist car went down at 1:40am and was put back in service at 2:15am. The "E" stop was in and needed pulled out.

Installing 2' by 2' plate on raw coal screen and strained back when the plate slipped. The employee required physical therapy. This is being turned in as a medical treatment incident.

Walked through gate encountering cobwebs and large spider which alarmed employee causing him to instinctively react in swatting motion. Struck elbow on post of fence. Was later treated at hospital for blood clot

Walking slope belt and slipped. Grabbed rail to catch himself and pulled his shoulder. He required Physical Therapy (which he began attending on 10/13) and is being turned in as a medical treatment.

#9 Escape capsule was inoperative due to a bad transformer. This was found during regular scheduled electrical checks. Miners were notified and all mining was stopped until the transformer was replaced at aprox 9:15am.

#1 Elevator was taken out of service due to suspected beam wear

Employee was walking and slipped, causing him to fall against and onto NM belt. The incident resulted in a laceration needing stitches.

Emerald hoist car reported out of service at 11:18 pm. Hoist rope overlapped itself. Both agencies were notified. Back in service at 11:57 pm.

The #1 Portal Elevator was reported out of service at 1:51 AM. There was a processor fault and had to be reset. Mon Valley Integration has been contacted to re-program processor. Elevator back in service at 2:51 AM.

Employee was passenger in mantrip that was bumped by a motor. Employee had some pain in back at time of incident. Did not seek physical therapy until Sept 8th. Has not missed any days work but will be reported as a Medical Treatment for physical therapy.

Employees left pinky & ring finger were struck by a jack bar while helping another employee jack on the Rockdust tanks

Emerald #1 elevator went down at 4:03 am on 5/30/2015 due to a PLC fault. The elevator was down until 5:18 am when the PLC fault was cleared and the elevator was put back in service.

Felt pain in left knee and hip while working on locomotive. Diagnosed with moderate degenerative changes of posterior horn medial meniscus. Had surgery on left knee 4/4/16

The injured was lifting a bucket of bits for the LW shearer and felt pain in his right groin area. He was diagnosed with a hernia and scheduled for surgery. He is a lost time accident.

The injured was lifting his tool bag when he felt pain in his lower right back. The injured was diagnosed with a lower back strain. The employee continues to work but is receiving chiropractic treatment and is being reported as a Medical Treatment Accident.

The injured was cutting off a motor from the slope car. He told slope operator to pull slope car out of mine but the motor did not uncouple. When slope car pulled up it took the motor with it. The injured's left foot got caught under the frame of the motor. He suffered a bruised left heel.

The injured was removing a plug from a hydraulic cylinder on a longwall shield. When he removed the staple, the plug shot out under pressure and struck the injured in the left hand causing a laceration between his left index and middle finger which required two sutures. This is being reported as a Medical Treatment accident.

The injured was unloading a timber jack and felt pain in his lower back. This is being reported as a medical treatment because the injured received Physical Therapy.

The injured was tightening the brake adjustment on the slope hoist. The wrench he was using slipped off the nut and the injured fell back and struck his head. He received a laceration that required two sutures. This is being reported as a medical treatment accident.

The slope hoist was down from 11:00 am till 5:30 pm on 3/24/2015. The auxilary brake on the hoist gear box was sticking. This condition was repaired and the hoist put back in service.

The injured was pushing a drill steel into the roof while roof bolting and felt pain in his abdomen. The injured was diagnosed with a hernia. His first missed day was 03/04/2015.

The injured was traveling as a passenger in a jeep. As they pulled up over a hill in the D1 section a mantrip was parked which the operator of the jeep did not see in time to stop and the jeep collided with the parked mantrip. The injured suffered a back strain and is receiving physical therapy and therefore is being reported as a Medical Treatment Incident.

While hoisting the LW shearer out of the mine on a dolly the shearer shifted and came partially off the dolly 800 feet from the top of the slope. The shearer was re-secured to the dolly and hoisted the rest of the way out of the slope at 7:00 PM of the same day.

The elevator at Emerald #1 mine at #1 portal went down at 3:50 am on January 27, 2015 and was repaired and running by 6:15 am that same shift. The midnight electrician found a stuck buffer switch and repaired it.

While walking to throw a switch the injured sprained his ankle when he stepped on a wedge. The injured continued to work and has not missed work. He is receiving Physical Therapy for his injury as of 2/6/2015 and is now being reported as a Medical Treatment accident.

The Emerald #1 Elevator went down when ice which had accumulated in the shaft broke loose and damaged two buffer switches. Schindler Elevator arrived at 1:45 PM and replaced the buffer switches and the elevator was back in service at 3:15 PM.

The injured was hanging the continuous miner cable when he felt a pop in his left shoulder. The injured has continued to work but has started receiving Physical therapy on 2/6/15 and is now being reported as a Medical Treatment.

The injured was making up a roof bolt along side the continuous miner when a piece of coal (10" by 11" by 8") fell and struck him in the wrist.

The injured was carrying a dust bar and he felt pain in his left bicep. An MRI revealed a full thickness tear of the anterior portion of the supraspinatus w/associated tendinosis (tendinosis is chronic degeneration without inflammation). This accident is being reported as occupational illness do to repeated trauma.

While loading a glue pump into a jeep the back end of the pump fell on the injured' s right wrist.

The #1 elevator was removed from service at 12:35 am for a bad DBG Roller on the car. The roller was replaced and the DGB was adjusted. The elevator was tested and returned to service at 3:23 am.

While putting a scoop on charge the injured dropped the battery lid on his left thumb.

The #1 Elevator was reported out of service at 9:22 am. There was cut cable to the bottom overtravel limit switch and a broken slow down vane. Both were repaired and the elevator put back in service at 3:30 pm.

The #1 elevator was reported out of service at 7:49 pm. A buffer switch was found to be damaged by falling ice. the buffer switch was replaced and the shaft was checked for any additional ice. The elevtor was returned to service at 9:00 pm.

During a 60 day safety test on #1 The elevator did not pass the governor test and was taken out of service at 11:20 am. Repairs were made and the car was returned to service at 12:20 pm.

The injured slipped and fell while moving a rail. He caught himself with his right arm and sprained his wrist. He continued to work at his normal job. He continued to have pain in his wrist and received and injection in his wrist on 11/20/14. This is being reported as a Medical Treatment accident.

The injured was walking an air course while making a weekly examination when he began feeling pain in his left knee. The injured has continued to work but the pain continues. An x-ray showed degenerative conditions in the injured's knee from repeated trauma from years of working in the coal mine. The injured continues to work and is receiving physical therapy.

The injured was operating the continuous miner when a piece of rib measuring 3' by 3' by 3" struck him in the right shoulder/scapula area. The injured has continued to work at his normal job and continued to have pain in his shoulder. An MRI revealed some tearing in his shoulder. The injured began receiving physical therapy on 11/24/2014. This is a Medical Treatment Accident.

The individual was pulling cables for the installation of a conveyor belt storage unit when he felt pain in his side. The individual suffered a hernia. the injured continued to work till the day of his surgery, 10-31-2014.

The injured was welding on the LW shearer when a piece of rock fell on the back of his right shoulder. an MRI revealed several degenerative conditions: spurring of the AC joint, partial tear long head biceps tendon and similar degenerative conditions of the labrum. The employee continued to work till going off for surgery 5/19/2015. Reported as an occupational illness.

The #1 elevator went down at 12:45am. The retiring cam rope had broken. The rope was replaced at 1:15am. the car was tested and put back into service 3:30am.

Walking along cars in a switch on top of sloughage, coal rolled out from the rib under his feet, he slipped and jarred his back.

9/23/14 ee reported that he was experiencing pain and limited range of movement in his right shoulder as he performed his job. An MRI revealed several degenerative conditions along with tears of the bicep and rotator cuff. This condition is being reported as an occupational illness due to repeated trauma over a 38 year career work in the coal mine. EE worked till surgery on 9/27/14

The injured was recovering conveyor belt drive components when he tripped over an anchor bolt and he felt a pop in his knee. The injured tore his right knee meniscus. He continued to work until his surgery on 11/03/2014

A rock that measured 34" in length and 20" in width with a thickness of 4" fell from between two roof support straps striking employee on the head and left shoulder.

Employee was working at the outside shop installing a drive motor shaft. He was stepping down from the motor to a wooden 4' step ladder to get a pry bar. The wooden ladder lost balance and kicked out from under him. He fell striking his right knee/shin on the motor and striking his left shoulder on the breather exhaust.

Struck elbow on mast when pulling stuck steel out causing a bruise.

The injured was putting his boots on when he felt pain in his left great toe. The injured worked till 8/21/2014 when he went to the hospital and was diagnosed with Cellulitis (a bacterial infection) and was taken off work. This is being reported as an Occupational Illness.

Employee was helping left side bolter on CM make up bolts when a rock (3'9" l, 2' w, 10" thick)struck him in the head, neck, right shoulder and forearm, and left hand causing two fractured fingers, several lacerations to the fingers, and severed the tip of his left thumb. The rock fell out between straps. The roof support was installed according to plan.

Debris struck him in the eye. Moving pogo sticks.

As injured was dropping the drill mast the steel came out of the chuck and sprung back toward his right hand. Injured apparently reacted by jerking his hand out of the way and inadvertently placed his hand on the frame of the drill mast and he dropped the drill clamp on his hand as he lowered the drill mast.

The #1 elevator was taken out of service at 7:40 am. A bolt was broken on the upper door switch. Schindler was notified. EE repaired the bolt. Schindler arrived, tested the elevator, and returned the elevator back to service at 10:00am.

Caught his index and middle finger between a ram jack and the shearer while loading jack to be transported causing a fracture to his index and middle fingers on his right hand and 14 sutures.

Emerald Coal Resources LP was provided documentation claiming an employee of 13 years of experience sustained an occupational induced hearing loss while employed at the Emerald Mine. This claim is being evaluated by the mine operator.

Stepped on a rock and rolled his left ankle causing a sprain/strain.

Received 8 stitches on the left side of his cheek area when he struck against a short strap while roof bolting

When getting out of mantrip employee tripped and caught his right finger in tensor when he braced for the fall.

While pulling a pin from mechanical splice, employee heard a loud pop and felt a dislocation to his right shoulder. Also diagnosed with a bicep tear.

Felt a burning in his left hip when he was stuck in the mud and twisted his body.

The #1 elevator was stuck at the top landing at 3:50pm. Found a broken retiring cam cable. Replaced the cable, tested the car, and returned the elevator to service at 5:40pm.

The outside top landing door GIB came out of the door track as the car approached the top landing the outside door contacted the closing linkage of the car door which resulted in the linkage breaking. Repairs were made and the elevator was returned to service 6:39pm

putting plastic pipe down air-shaft, pipe swung around & hit under ribs. Bruised ribs & liver.

Employee received a concussion when he fell onto a low boy dolly out of a manhole.

Employee experienced back spasms throughout the day. Was diagnosed with muscle spasms and taken off work.

The anchor bolt pulled from the wall causing slack in the come-along chain which free wheeled the distribution box that came down on to Employee's right foot causing a fracture.

The #1 elevator was removed from service because it would not land properly at the bottom landing. The brushes of the generator regulator were not properly seated. The brushes were seated and the car was tested. The car was put back into service at 12:00.

While loading rollers, a roller fell from a stack and struck his left hand causing one fractured pinky finger and 14 stitches in his hand.

Tripped and fell over cable handler of loader machine and felt a strain in his left shoulder.

While bolting on the continuous miner in the D2 section in #1 entry at 9-10 crosscut, a loose piece of rock fell when employee was helping hang the miner cable. The rock bounced off of the ventilation tube and struck him in the lower lip on the left side of his mouth/face.

Moving debris and a piece of wire hit his eye. A cable hit his glasses.

The employee was riding on the inby end of a diesel mantrip when it came into contact with another mantrip. The end result of this accident was a cervical strain/sprain and thoracic strain.

The employee was walking past a trip on the haulage when he stepped on rock in the walkway spraining his right ankle.

Changing 2 pre-cleaner blades at slope.

While walking, employee slipped on a cable and fell at the stationary loader. When he fell, he caught himself with his right hand causing an over extension of his right wrist and a simple fracture to the right hand.

The #1 elevator was removed from service at noon for scheduled maintenance to replace the car floor. A new floor was installed. The car was returned to service at 7:58am on 04/06/2014.

Employee was helping load up High Voltage cable onto a supply car when he caught his right pinky finger between the high voltage cable and the steel pipe side support (standard) on the supply car. This caused a fractured right pinky finger, his fingernail was removed, and he received two sutures to the area of his right pinky finger.

Employee was stepping onto a motor and jammed his head into the roof causing him to fall backwards landing hard on his back. Neck and lower back pain.

The injured stepped off a supply motor and twisted his ankle. the injured worked till 08/08/2014 the day of his surgery. Due to poor communication we did not realize that he went off work for his surgery. that is why we are late reporting this incident.

Employee felt a strain in his lower back while handling hi-tech pump able bags.

This miner has participated in a medical examination authorized by Federal Mine safety Health Act of 1977. This examination indicates the miner has a sufficient degree of black lung to be eligible for rights afforded under Part 90.

Employee slipped and lost his balance falling backwards and landing on his self rescuer as well as a bit bucket. The injured stated the bit bucket struck his lower back. Injured had complaints of back/tailbone pain and tingling down his right leg.

The #1 elevator would not leave the top floor. the car was out of service at 12:10am. At 12:30am the door lock was found to be the problem. The lock was cleaned and lubes and tested. The elevator returned to service at 1:10am

The #1 elevator would not leave the top landing. The car cable and the top landing level of the k limit switch were replaced. the car was out of service from 3:30pm and returned to service at 6:45pm.

Employee was exiting his vehicle and slipped on the ice in the parking lot. The employee caught himself with his left hand and received a laceration on the knuckle of his little finger that required stitches.

The wheel of a loaded refuge chamber struck employee and pinned his ankle against the rib. He was diagnosed with a fracture to the right ankle.

Employee was pulled through a high air pressure regulator while doing an examination. After pain continued, employee was checked on 02/04/2014. X-rays revealed fractures to his #8 and #9 ribs

A triangular piece of sandstone fell striking employee on the left hip. After an MRI revealed the employee has a possible stress fracture to the hip, he was taken off work.

Left thumb was pinched between SCSR and slate bar while adjusting forks on fork scoop causing a simple fracture to his left thumb

Employee tripped on uneven bottom on beltline striking right index finger on roller and cradle

The elevator car stopped 85 feet from the top landing. The car was moved to the top landing within 15 minutes. The actuator for the slack rope switch was bent.The actuator was straightened, sheaves and ropes tested, and inspected the car. the elevator was returned to service at 12:15pm

While doing a weekly fireboss examination employee felt a strain in his left knee. Employee had previous degeneration and multiple operations on this knee.

The employee's left foot got stuck in the mud while closing the Kennedy Doors and fell causing him to strain his left ankle.

While stepping out of a truck, at the Electrician's Shop, the employee slipped on ice and felt a strain in his groin. The employee's first day off due to this injury was 03/10/2014 for hernia surgery.

The #8 elevator was out of service for a period of time exceeding 30 minutes due to a bottom door lock fault. The elevator was tested after repairs were made prior to returning to service at 8:05am.

At 7:10 pm an ignition occurred at the face of #3 entry between 76 and 77 crosscuts in the D1 section where the 14ED25 miner was cutting coal. An orange flame approx. 2 feet long appeared in the upper right hand corner of the face where there was some sand stone above the binder. The flame was extinguished using the wash down hoses on both sides of the miner afet approx.7 seconds.

At 10:43 pm an ignition occurred at the face of #3 entry between 76 and 77 crosscuts in the D1 section where the 14ED25 miner was cutting coal. The flame was extinguished using the wash down hoses on both sides of the miner after approx. 6 seconds.

Removed the #8 elevator from service at 10:45 am. Replaced the govenor rope. Tested and returned the car to service at 1:25pm.

A roof fall occurred in the D Bleeder section at #88 crosscut 3 left entry at 2 room. The fall was 10 feet above the anchorage point where it appeared that a slip occurred with laminated shale.

While passing through a man door, employee felt a pop in his knee.

While operating a scoop, employee felt strain in back/neck when his back was jarred while traveling over crib.

The employee was bolting on the miner and a piece of rock fell from the top striking him on the nose.

Was working on putting a shuttle car conveyor chain back together. Co-worker was tightening come-a-long. The chain attached to the flight (not the come-a-long chain) broke loose striking employee in the hard hat.

The employee was trimming the top when a piece of rock came out of the top from the head to the last strap. The piece of rock rolled back and struck the miner causing a smaller piece of rock to break off and continue to roll back contacting the employee's right ankle/foot.

The employee was assisting in removing a pan wall when one panel became stuck due to being wedged. As he went to unwedge it, the panel released catching his hand between two panels causing a laceration. At the time of the injury he was wearing the Meta Miner gloves and the panel cut through the gloves.

The #1 Elevator was stopped at the top of the landing and could not be called to the bottom. The computer room operator was contacted at 12:25 AM. The computer room operator then contacted EE around 1:00 AM and he found that the dynamic brake had a PLC Fault. EE then reset the fault, tested the car and returned the car to service at 1:24 AM.

The employee was walking to refill a rock duster when he stepped on a rock twisting his right ankle.

While installing truss bolts the injured was pushing a drill steel in a hole when he felt discomfort in his right shoulder. After a doctor's exam and a MRI it was determined that the injured has degenerative conditions that require surgery. This incident is being reported as an occupational illness's. The injured worked toll 9/11/13 when he went off for surgery.

The employee was attempting to disengage the toggle switch on the miner that controls the hydraulic oil suction function. As he reached out he accidentally scraped his right hand across a strap plate; cutting him on the top of his right hand around the knuckle area.

The employee was putting a drill steel in when a piece of top fell out and struck his right thumb.

The employee stepped out of the mantrip and felt a discomfort in his right knee. As he put pressure on it he felt the discomfort extend from his right knee down to his right foot.

The employee was shoveling debris out of a manhole on Northeast Corridor at 3 Room. While he was shoveling, he felt a strain/pain in his lower back.

The #1 elevator lost control power at 10:55 p.m. EE, an electrician was dispatched to the #1 elevator and troubleshoot the issue. He did not find any defective equipment or components during his troubleshooting phase. Power was reapplied and the elevator was tested, it was returned to service at 12:45 a.m. The elevator was down for one hour and fifty minutes.

The employee had just finished scaling down loose rib when a piece of rock fell from the rib striking his left arm/wrist causing a laceration and bruising.

The injured was dragging a rockdust hose when he slipped on a wet walkway. As he fell he reached out with his right hand to brace himself. When he hit the ground his right hand contacted an old roof strap that was laying in the c.c. causing a laceration to the base of his right hand requiring 5 sutures.

A roof fall occurred in the C4 Longwall section in the # 7 crosscut 2-3 entry. The fall measured 12 to 14 feet above the anchorage point.

A fire on the Load center in the E Mains section was reported at 5:40 am by a fireboss. The fire was extinguished using fire hose on a beltline fire tap, a fire car, and smothering with rock dust.

Slipped and fell causing a contusion to lumbar back and abrasion to right flank

EE was bolting on the miner. The drill steel came around as it rotated and slapped him in the right hand pinching it between the drill steel and the miner causing a laceration that required 6 sutures.

Employee was walking to hang a cable and stepped on a rock causing him to roll his right foot. Employee received a simple fracture of the fifth toe on the right foot.

Employee stepped in a hole with slick bottom. He lost his balance and when he fell heard a pop in his right knee. He felt some immediate pain.

Employee was struck by a rock measuring 2' x 2' x 4" thick fell from the top hitting him on the back of his left leg between the calf and foot causing a compound fracture to the left leg.

Employee received a tear/rupture to his right bicep dragging a ventilation tube to the face.

Employee was carrying pump parts back and forth in back bleeder when a 6" air line broke striking him and knocking him into roof support pumpable pier. Received pain to his head, neck, and back when he struck pier.

Employee was struck on his left foot by a rock (approx. 18" x 25" x 3 1/2" thick) weighing approx 50 lbs. from the roof. He received a fracture of the 4th toe and 7 sutures to the 4th toe.

At 8:05am the #1 elevator car had left the bottom landing and traveled about a foot. Nobody was on the car. There was a problem with the DB contactor. The defective component was replaced, the car was tested, and returned to service at 11:50am.

Employee received a compound fracture and 8 stitches to his right middle finger when he caught his hand between the bolter head an the mast as it dropped.

The # 1 elevator was taken out of service at 10:15am for scheduled maintenance. The govenor rope was changed and the car was returned to service at 1:15pm.

The #8 Elevator stopped within 12 inches of the top landing with one passenger on the car. The car showed overspeed fault. The rope gripper set and locked the car out on fault. The passenger was stuck for 5 minutes. The car was reset and tested. The agencies were notified. The elevator returned to service at 3:45am

EE was setting up his work area to begin pumping grout. He had to join two pieces of pipe together. He was walking to where the poly was. There was a water pipe about 2 1/2' above the ground. EE leaned over and walked under the pipe. This is the point he said he injured his back.

The #8 Elevator was removed from service at 2:45 PM. The bottom door lock failed. The bottom door lock was replace and then the car was tested. It returned to service at 4:45 PM.

A roof fall occurred at the intersection of E. Corridor #1 entry and C-Mains #2 entry at 60 x-cut of E. Corridor. The fall was approx. 10'-12' high across the intersection. There were no injuries. The area was dangered off and posts were set on all 4 sides of the intersection. Ventilation was not effected. The alt escapeway for the C district was re-routed around the fall area.

Removed #1 Elevator from service at 2:00 PM. The counter weight buffer was found to be leaking. Schindler was contacted and removed/repaired the buffer. The car was tested and returned to service at 8:00 PM on 6/15/13.

Removed #1 elevator from service at 10:45am to conduct scheduled maintenance on the governor. Completed the preventive maintenance. Tested the governor and returned to service at 1:00pm

Took elevator out of service. Stopping by itself on up from bottom. Four door tracks had pushed up due to rust. Corrected problem and put back into service at 5:05PM.

Employee received a laceration to his left hand/finger when lifting a transfer rail off the track jeep. The rail slipped and caught his hand/finger between the rail and jeep.

Employee received a contusion/abrasion when he was struck by a rock that fell from in between two roof support straps on his lower back area.

On 5/20/13 EE reported feeling pain in hid rt. knee. He did not have and incident to cause this other than everyday activity. The injured had a MRI that showed degenerative condition from years of exposure to rough walking conditions in and about the coal mine. The injured cont'd to work till going off for surgery on 8/8/13. This is being reported as an OCC Illness.

Employee received a broken collarbone and a puncture wound to his collar area when a piece of rock material fell from between two roof support straps striking employee.

Employee received a laceration when he caught his index finger in the bolter drill clamp. He received 2 stitches to his right finger.

Employee was picking up a rock dust bag to put into section ventilation fan. While hoisting the bag up to his shoulder, he felt pain in his right arm/bicep.

At 3:38 am an ignition occurred at the face of #3 Entry between 46 and 47 crosscuts in D 1 Section where the 14ED25 miner was cutting coal. The flames were from top to bottom the entire width of the face and were blue to orange in color. The fire lasted approximately 10 to 15 minutes. There was a visible clay vein making its way across the race from right to left.

Employee was reaching behind seat on a mantrip and received a laceration from a loose razor blade on his right thumb.

Employee received a 3-4 inch laceration to the top of his head when a piece of rock fell from the roof between two roof support straps. Employee was roof bolting on continuous miner. Rock was approx. the size of a softball.

The No. 1 Elevator was removed from service at 11:30 am. Schindler was performing a scheduled maintenance on the car safeties. The maintenance was completed, the car was tested, and returned to service at 3:45pm.

Employee was running continuous miner and loading coal onto a pile when a piece of rock approx. 1' x 1 1/2' and 8" thick struck his right foot just above the metatarsal when the rock rolled off of the pile.

While installing 10" pipe, employee was pushing pipe together and his foot slipped on a wet surfece causing him to twist his right knee.

Felt Pain in shoulder when lifting jack to Duster. An MRI dated 03/14/2013 showed recurrent full-thickness tear of his rotator cuff with thinning of the supraspinatus tendon. Employee had surgery on 05/20/13 performed by Dr.This is an O/I

Cut stump off tree and footing slipped and strained lower back.

Employee was pounding a wedge in on the side of a wall when the hammer missed the wedge and struck his left thumb. The employees left thumb was against a cement block when he struck it.

The DEP inspector found a problem with the stop for the car safeties on the top of the car. the safety was bent. The car was removed from service about 1:00pm. The was part was straightened and reinforced then installed back on the car. The car was tested and returned to service at 3:07pm.

Employee was dragging rock dust hose into #4 entry of East Corridor at 40 crosscut. The rock dust hose snagged on an object causing it to stop suddenly and jerk hose back. Employee felt a strain in his groin.

Replacing angle iron when glove caught sharp angle cutting his hand.

EE was carrying rockdust through a door on the E Mains belt at 15 rm. The door was spring loaded and caused the door to strike his right shoulder. The EE cont. to work and complained of some pain. An MRI revealed degenerative conditions atrophy and a tear in the right rotator cuff. EE had previous rotator cuff surgery for the same shoulder. EE was taken off work as an occ illness.

EE felt pain in his right shoulder after he slipped on wet belting and put his arm out to brace himself.

The #8 elevator would not unlock the bottom hatchway door when called. The shift foreman, MSHA, and DEP were notified. Shindler was notified and arrived at 8:30pm. There was a set screw loose on the coupler between the door lock motor and the door lock. The screw was tightened and adjusted. The car was tested and put back into service at 11:00pm

Employee was struck on the right knee with loose rib on the D-1 section.

Employee stepped on a lump of coal and twisted knee. Employee has a history of degenerative knee conditions including knee joint effusion. This is being submitted as an occupational illness due to the employees existing knee conditions.

The employee was reaching across the top of the miner to retrieve a box of glue when a rock came loose from the face striking the straps that were being stored on top of the miner. One of the straps struck the employee under his right arm. There were no cuts/contusions to the employee and the physician's diagnoses was a strained right shoulder.

The employee was pulling a longwall shield and slipped off the shield pontoon, twisting his right knee. The physician's examination was negative, however the employee was not able to apply weight to his right leg.

The employee was walking to the End of the Track to get supplies off a supply car. Prior to making it to the supply car, the employee tripped over the track, twisting his right knee. The employee was not carrying anything at the time he tripped. The physician's examination was negative, however the employee was not able to apply weight to his right leg.

The employee was walking to a mantrip at the beginning of his shift when he twisted his left ankle. The walkway was cemented and free from material. The physician's examination and x-ray were negative and he was diagnosed with a sprained left ankle.

The employee was removing a piece of belt structure that was in the walkway and felt pain in his abdomen. The physician later diagnosed a right inguinal hernia.

EE was walking from the Headgate to the Mid face on the Longwall. When he stepped over #11 shield relay bar extension, his foot caught the extension's lifting handle. EE attempted to stop his fall by clasping on to the panline handrail. A strain to his right bicep resulted from the clasping. The strain was later diagnosed a muscle tear to the bicep requiring surgery.

A roof fall occurred on the D -1 section in the #1 entry 3 way intersection at 24 crosscut. the fall was approximately 20 feet long and 10 feet high. There were no injuries as a result of the fall. The area was dangered off and posts were set just outby fall area in the #1 entry and also set in the 1 - 2 entry. Ventilation and the escapeways were not effected by the fall.

The #1 Elevator had the safeties set on the car when a test was being conducted and the elevator was called to the bottom. The safeties were reset and the car was tested and returned to service at 12:40 pm.

While changing bits on the tail drum a rock fell from the top striking employee on the neck and left side of his back causing a bruise to the left side of the employee's back.

Employee was standing in crosscut waiting for a scoop operator to return. While waiting a piece of rock measuring 24"x4" fell striking employee and knocking him to ground. When the employee braced himself falling to the ground he jammed/sprained his left wrist.

Employee was struck by a rock 1' long x 1' wide x 3" thick on top of left foot.

Employee had a piece of slate fall loose and strike left hand causing a laceration through his protective gloves.

EE walking a catwalk when rt foot slipped causing some pain in rt knee. Some pain would occasionally surface & MRI was conducted 11/27/12 determined torn meniscus & surgery performed 12/6/12. Surgery was performed on rt knee prior to this accident, repeat trauma to knee from 38 yrs mining & degenerative changes listed in MRI lead to our reporting of OI.

Employee was bolting on the right side of miner #1024 on D1 section when a rock fell from between the outer bolts and the rib striking the ventilation tubes and then the employee knocking him down and landing on his left lower leg.

The #1 elevator was reported to be making a noise at the top landing. EE's found 2 bad rollers and removed the elevator from service at 8:15am. The rollers were changed and the elevator was put back in service at 8:50am. No one was trapped and both agencies were notified.

During a 60 day no load test, the safety catches were not working properly. The elevator was removed from service at 12:20. Shindler made repairs were made to the governor and the safety catches were tested. the elevator was returned to service at 2:50pm. Both agencies were notified and no one was trapped.

Employee received a laceration to finger when reaching for support on a PM4 Box while stepping from panline. Small bur on PM4 box caused laceration.

Employee felt a pop and a burning sensation around the naval area when he bent over to pick up a case of water in the Plant Control room. Accident resulted in a Hernia. Operation was done on 11/12/2012. Employees first day off work was 11/13/2012

Employee received 6 sutures when cutting a band holding a lift of bolts. The band struck the employees left forearm causing a cut 3" long and 1/4" wide.

Employee was loosening bolts to crusher hammer. He removed hammer and attempted to pass to another employee. The hammer was too much weight, pushed his hand down, and struck the next crusher bit with his right ring finger caught between.

Employee was changing bits on longwall shearer using a bit tool when he struck his right hand with a 3 lb hammer causing his hand to be forced down on bit tool. A laceration and fracture to right pinky finger resulted from the force.

Employee caught ring finger on right hand between drill steels while bolting on the right side of a continuous miner. Employee lacerated the finger.
