Unit #1 - Employee was rib bolting off of the scoop with a rib drill gun. The auger hit a rib bolt on the other side, causing the gun to twist resulting in a strain to the left hand, wrist, and forearm area.

Mostrando los 23Unit #2-While roof bolting, cable reel came on too fast & caught legs when bolter was started. Employee's legs went out from under, forcing legs into bumper of bolter. Incident resulted in injuries to right & left legs, knees, & hips. Employee continued to work. In 2007, Dr recommended left knee surgery. DID NOT LOSE ANY TIME UNTIL DATE OF SURGERY ON 04/04/2007.
Unit #2 - Employee was operating miner, rerouting back across the section. After backing out, the cable was in the roadway. Employee picked up cable and walked across to the rib line. Right lower leg muscle gave out.
Unit # 2 - Employee was installing a roof bolt. Pinned finger between bolt plate and the mine roof. Injury resulted in fractures and a laceration to left middle finger. Returned to Work 03/02/2006. Lost 4 Work Days.
Employee was cable bolting when co-worker lowered the foot on employee's left foot causing a facture to 3rd toe and contusions to 4th and 5th toe.
Employee was swinging boom over when a pinner steel flew from the tray, striking employee on right side of cheek resulting in bloody nose and fractures in the cheek area.
1W#3 Belt - Dust hose came apart and hit employee in the face causing a contusion and laceration to upper center lip and inside of lip. Laceration was approximately 1/2" to 3/4" long requiring 4 sutures, 3 on the outside, 1 on the inside. MEDICAL TREATMENT ONLY.
Employee heard a noise coming from refuse screen. Put hand between springs and side panel causing abrasions to right wrist and hand. Amputation of right middle finger and fingernail requiring skin grafts. Strain to back occurred as employee pulled hand back from machine. Delay in reporting due to waiting on medical reports. MEDICAL TREATMENT ONLY. NO LOST TIME.
Employee was bolting brow of roof fall. On left side of bolter using two pieces of steel. Upper section of the roof steel hung up in the hole when EE dropped the pod. Steel dropped out after it cooled & struck rt. thumb mashing it between upper steel & drill pot causing a fracture and laceration. NOTE: Amended Narrative per misc. mail 1/25/05 #1.
EE was installing outside bolts next to ribline. While in drilling procedure, a piece of top (6" thick 36" long 31" wide struck backside, pushing ee into bolter boom, injuries included contusions to back, nose (fractured) and mouth.
Employee was cutting swivel pin out of miner. A piece of hot slag came back and burnt the oxygen hose in two right behind the torch causing 2nd degree burns to right hand and fingers.
EE WAS INSTALLING 2ND RIB HOLE WHEN A PIECE OF COAL, 7' LONG, 3" THICK RASHED FROM THE RIB STRIKING HARD HAT KNOCKING EE 20'AWAY PIECE WAS IN 2 SECTIONS. CONTUSIONS/SOFT TISSUE INJURIES TO HEAD, BACK, NECK, CHEST & RIBS
EE WAS INSTALLING PULL THRU BOARD, DRILL STEEL GOT STUCK IN BOARD. THE ROTATION OF THE STEEL WAS ENGAGED AND CAUGHT GLOVE. FINGERS OF LEFT HAND WERE WRAPPED AROUND THE STEEL TO THE POINT THAT THEY BECAME OVEREXTENDED. DISLOCATION TO RING FINGER, CONTUSION TO MIDDLE FINGER.
MAIN D UNIT - EE WAS INSTALLING OUTSIDE BOLT IN #1 ENTRY AND INADVERTENTLY PUSHED THE BOOM LEVER THE WRONG WAY. SWING BOOM STRUCK CANOPY CAUSING IT TO HIT LEFT UPPER LIP & UNDER LEFT EYE AREA CAUSING FACIAL LACERATIONS. REQUIRED EXTENSIVE SUTURING. PLANS TO HAVE LEFT EYE CHECKED BY EYE DOCTOR. NO LOST TIME.
SURFACE, NORTH STACKER TUBE - EMPLOYEE WAS OPERATING D-9 DOZER, BACKING AWAY FROM FEEDER. BACKED OFF A LEDGE OF COAL. EMPLOYEE WAS LOOKING BACK TO THE RIGHT WHILE BACKING UP. DOZER HIT HARD, J AMMING NECK. EXPERIENCING SEVERE PAIN IN NECK/HEAD.
MAIN D - EE WAS RIB BOLTING W/CO-WORKER. RIB ROLLED OFF PINNING EE. ANOTHER CO-WORKER HAD TO REMOVE RIB FROM EE. RIB LANDED ON THE BACK OF FOOT. FXS TO THREE TOES ON LEFT FOOT, CONTUSIONS/SWEL LING TO LEFT FOOT.
B1 SECTION #1 ENTRY- EE WAS RIB BOLTING. REACHED OUT TO ADJUST STEEL, ROCK (33"X 53"X 2")FELL. STRUCK BACK OF RT. ARM, WRIST & HAND. FX'S TO ARM & HAND. TAKEN TO HOSPITAL THEN TRANSFERRED AND ADMITTED FOR SURGERY.
(LTA) B1#3(ROOF FALL @ 2 X-CUT). EMPLOYEE WAS INSTALLING CABLE BOLTS ON INBY SIDE OF FALL W/#72 BOLTER. PIECE OF RIB, 5'L X 2 1/2 H X 2-3"THICK FELL. HIT BACK & PUSHED CHEST INTO BOLTER, INJUR ED LT HAND ALSO. CONTUSIONS/ABRASIONS.
EE WAS WORKING ON MINER. PIECE OF ROCK FELL FROMTOP STRIKING MIDDLE OF BACK, FELL ON BAR WITH LOWER LEG & HE WENT DOWN. ROCK SIZE WAS 3' X 14" X4". ROCK FELL ~5' TO 6' BEFORE STRIKING EE.
EE WORKING RIB BOLTING W/OTHER WHEN THEY WERE PUTTING RIB GUN DOWN. OTHER DROPPED HIS END BEFORE EE DID. EE WAS HOLDING GUN W/LEFT HAND. HE JERKED IT HAND AND ARM. THOUGHT ARM WOULD BE OK, THE N ON 7-21 NOTICED SWELLING AND BRUISING DX-LT ARM BICEP TEAR. TURNED IN LATE DUE TO WAITING ON MEDICAL RPTS AND INFO TO CONFIRM INJURY.
MW #3 INSTALLING CHAIN HANGERS IN FALL AREA. STANDING AGAINST RIB HOLDING 4' BOLT & CHAIN HANGERIN HAND. DRILL STEEL CAME IN CONTACT WITH ROOF. ROCK FELL & STRUCK CANOPY. CANOPY BROKE UP. EMPL OYEE TRIED TO GET AWAY & HIS HAND & FINGERS WERESTRUCK. CRUSHING INJURIES TO INDEX & MIDDLE FINGERS OF RIGHT HAND. TAKEN TO HOSPITAL.
EE WAS OPERATING MINER CLEANING UP WHEN HE INADVERTLY BACKED INTO THE RIB. RIB FELL OFF AND HIT THE BACK OF HIS ANKLE. RIB THAT FELL ON ANKLE WAS 11' LONG, 10" WIDE AND 11" THICK. TAKEN TO WGH , TRANSFERRED TO ST. MARY'S. FRACTURE TO ANKLE. FIRST DAY MISSED 8-9-00.
(LTA): B:1 UNIT - EE WAS ROOF BOLTING WHEN A PIECE OF ROCK FLEW INTO HIS RIGHT EYE CAUSING A CORNEAL LACERATION. DID NOT RECEIVE ANY DR REPORTS UNTIL AFTER VACATION PERIOD 6/23 THROUGH 7/10. R ELEASED FROM W/C 6/24/00.
