(MT) : Employee was picking up acetylene bottle. The cap came loose, striking chin causing a laceration requiring sutures. MEDICAL TREATMENT ONLY. NO LOST TIME.

ROOF FALL (7000-1-022-06) : LOCATION - Main E #5 Belt, Crosscut #5-#6. DIMENSIONS - H:8', W:19', L:60'. GEOLOGY - Soft, medium to dark gray shale with prominent roof jointing or slips in roof. ROOF SUPPORT - 5' Fully grouted bolts.

(LTA) : Unit # 1 - Employee was lifting rub rails on #55 miner. Strained right chest muscle. Did not start losing time until 12/20/06.

ROOF FALL (7000-1-021-06) : LOCATION - Incline Switch, Slope Bottom Between Road and Belt. DIMENSIONS - H:5', W:19', L:30'. GEOLOGY - Soft, medium to dark gray shale. ROOF SUPPORT - 6' Conventional bolts. The area was mined in April 1973. Cutters and draw slate were present in the roof.

ROOF FALL (7000-1-020-06) : LOCATION - MW#1 Roadway, between Crosscut #9 and #10. DIMENSIONS - H:6', W:19', L:80'. GEOLOGY - Soft, medium to dark gray shale. ROOF SUPPORT - 6' Conventional bolts. The area was mined in March 1983. Cutters and draw slate were present in the roof.

ROOF FALL (7000-1-019-06) : LOCATION - Main D, Face Area, #5 Entry, 80' North of Spad #57429. DIMENSIONS - H:6.5'-7', W:18', L:18'. GEOLOGY - Soft, medium to dark gray shale. ROOF SUPPORT - 5' Fully grouted bolts. The area was mined in May 2001. Cutter and draw slate were present in the roof.

ROOF FALL (7000-1-018-06) : LOCATION - Main E Return, Crosscut #103. DIMENSIONS - H:6', W:19', L:30'. GEOLOGY - Soft, medium to dark gray shale with prominent roof jointing or slips in roof. ROOF SUPPORT - 5' Fully grouted bolts. The area was mined in June 2004. Prominent roof jointing in the roof were the apparent cause of the fall.

HOISTING INCIDENT : Cowling Portal Elevator - On Wednesday, 12/06/06 at 09:53 am, the cage was taken down and was inoperable for more than 30 minutes. The elevator was removed from service. The bearings on the idler roller were changed. MSHA was notified on 12/06/06 at 09:55 am. The elevator was operable and running on Wedneday, 12/06/06 at 02:00 pm.

HOISTING INCIDENT : Cowling Portal Elevator - On Thursday, 11/30/06 at 12:40 pm the cage was taken down & was inoperable for more than 30 minutes. The elevator was not properly hoisting. The door switch on bottom was worked on to repair the problem. MSHA was notified at 01:20 pm on 11/30/06. The elevator was operable & running on Thursday, 11/30/2006 at 02:00 pm.

1W#1 Belt, 12 x-cut - Employee was dusting belt. Employee attempted to go under water line and did not duck low enough. Jammed neck into water line causing a neck sprain.

ME3 Roadway - Employee and co-worker were on their way to ME5 and 6 to check splices. They met the scoop and drag at 78 x-cut. Co-worker stated a piece of bridge plank approximately 5' long by about 4" wide was kicked up from under the front wheel and hit employee in the right ribs causing multiple fractures.

Unit #2, #3 Face - Employee was bolting face of #3 entry. A piece of rock approximately 30" square, with thickness of 2 1/2" tapering off to 1/2" fell from between bolts. The rock bounced off ATRS, striking left side of lower back and left hip causing abrasions. RTW 11/04/06. Lost 2 Work Days.

Unit # 1 - Employee was hanging miner cable with high-line hooks when the cable came down. Caught the inside part of right hand between thumb and index finger on hook causing injury. Laceration required 6 sutures.

HOISTING INCIDENT : Cowling Portal Elevator - On Wednesday, 10/18/06 at 06:05 pm the Cowling Portal Elevator was taken down and was inoperatable for more than 30 minutes. MSHA was notified at 06:35 pm by Tamara Betancourt. The door switch fuse in the control panel had to be replaced. The elevator was operable and running on Wednesday, 10/18/06 at 10:05 pm.

ROOF FALL (7000-1-011-06) : LOCATION-Main E, crosscut #115, entry #2. DIMENSIONS-H:6', W:19', L:50'. GEOLOGY-Soft, medium to dark gray shale with prominent roof jointing or slips in roof. ROOF SUPPORT-5' Fully grouted bolts.

ROOF FALL (7000-1-010-06) : LOCATION-Main E #5, crosscut #1 belt entry. DIMENSIONS-H:8', W:19', L:60'. GEOLOGY-Soft, medium to dark gray shale with prominent roof jointing or slips in roof. ROOF SUPPORT-5' Fully grouted bolts.

ROOF FALL (7000-1-009-06) : LOCATION - Main E #5, crosscut #1 between road and belt. DIMENSIONS - H:6', W:19', L:50'. GEOLOGY - Soft, medium to dark gray shale with prominent roof jointing or slips in roof. ROOF SUPPORT - 5' Fully grouted bolts.

Unit #2-While roof bolting, cable reel came on too fast & caught legs when bolter was started. Employee's legs went out from under, forcing legs into bumper of bolter. Incident resulted in injuries to right & left legs, knees, & hips. Employee continued to work. In 2007, Dr recommended left knee surgery. DID NOT LOSE ANY TIME UNTIL DATE OF SURGERY ON 04/04/2007.

: MW#4 - Employee was bolting on roof fall. Rock fell from between bolts, hitting the ATRS and sliding off. Struck employee's right lower leg. Rock was 29" x 31" x 1".

Employee was hanging a block curtain. Stepped in a hole, twisting left knee. Employee did not start losing time until 10/30/06.

ROOF FALL (7000-1-008-06) - LOCATION - Main #4 Belt, Crosscut #102. DIMENSIONS - H:6', W:19', L:40'. GEOLOGY - Soft, medium to dark gray shale with prominent roof jointing or slips in roof. ROOF SUPPORT - 5' fully grouted bolts.

Unit #3 - Employee was installing 8' roof bolt. The bolt did not tighten up. Employee was trying to pull the bolt back out of the hole and off pod. Felt a pop in right shoulder.

Unit #1 - Employee was hanging miner cable. A piece of tie wire went behind safety glasses and left eye was injured, resulting in a scratched eye. RTW 08/11/06. Lost 3 Work Days.

(LTA) : Unit #3 - Employee stepped off front of bolter into hole, twisting left knee. Heard a popping sound. Dr diagnosed a hyper-extension of left knee. RTW 08/03/06. Lost 2 Work Days.

ROOF FALL (7000-1-007-06) : LOCATION - Main E #3 - 1st Panel, Near Spad #65012. DIMENSIONS - H:6', W:19', L:40'. GEOLOGY - Soft, medium to dark gray shale with prominent roof jointing or slips in roof. ROOF SUPPORT - 5' Fully grouted bolts.

(LTA) : Unit # 2 - Employee was taking down miner cable. Cable struck employee, knocking employee to the ground. Right knee landed on pile of coal, bending and twisting resulting in injury to right knee.

ROOF FALL (7000-1-006-06) : LOCATION - 1W#3 - Crosscut #105 between the roadway and the belt. DIMENSIONS - H:10', W:20', L:20'. GEOLOGY - Soft, medium to dark gray shale with prominent roof jointing of slip in roof. ROOF SUPPORT - 6' Instal bolts.

Unit # 3 - Employee set-up miner in the face of the #4 entry. After set-up, the employee was throwing cable along ribline. Experienced pain in lower back radiating down left leg.

Employee was climbing on scaffold, right knee hit runner. Sprained right knee while trying to keep from falling because of muddy conditions. Started losing time 09/05/06. Filed late due to employee not losing time until 09/05/06. We were also waiting on Dr's reports.

HOISTING INCIDENT : Cowling Portal Elevator - On Friday, 07/07/06 at 11:30 am the Cowling Portal Elevator was taken down and was inoperable for more than 30 minutes. MSHA was notified at 12:30 pm on 07/07/06. The elevator was operable and running on Friday, 07/07/06 at 01:05 pm.

HOISTING INCIDENT : Cowling Portal Elevator - On Thursday, 07/06/06 at 01:25 pm the Cowling Portal was taken down for preventative maintenance work by two electricians and was inoperable for more than 30 minutes. The elevator was running on Thursday, 07/06/06 at 03:00 pm.

(MT) : Unit #3 - Employee was working in the battery charging station, putting batteries on charge. Employee was prying the lid on battery. Slipped, falling on the metal corner of battery. Injury resulted in a laceration to the right side of face around the jaw bone approximately 1/2" long and 1/2" deep. NO LOST TIME. MEDICAL TREATMENT ONLY.

HOISTING INCIDENT : Cowling Portal Elevator - On Monday, 06/19/2006 at 03:00 pm, the Cowling Portal Elevator went down due to an electrical problem. David Rinehart, Electrical Engineer worked on the issue. The elevator was repaired and running on Monday, 06/19/2006 at 03:45 pm. MSHA was on-site. Hoisting incident was called in to MSHA at 03:40 pm.

ROOF FALL (7000-1-005-06) : LOCATION - Old Main B, #7 Entry, South of Spad #51614. DIMENSIONS - H:6-7', W:19.5', L: 35'. GEOLOGY - Soft, medium to dark gray shale with prominent roof jointing or slips in the roof. ROOF SUPPORT - 6' Instal bolts.

Unit #2 - Employee was operating miner, rerouting back across the section. After backing out, the cable was in the roadway. Employee picked up cable and walked across to the rib line. Right lower leg muscle gave out.

Unit #1 - Employee was putting down cable crossers to take the bolter into #2 entry. When employee was lifting miner cable to move it against the corner of the rib, he experienced pain in lower back and a sharp pain going down left leg.

MDR Return - Employee was walking weekly examination. Stated stepped off of 2" or 3" thick piece of rock. Felt a pop in left foot and pain shot up through left leg and left hip.

Unit #2 - Employee was bolting #8 entry. The height was 5' 9". Experienced pain in lower back going down left side and left leg when moving a bundle of roof plates. Pain got worse as employee tried to resume bolting.

HOISTING INCIDENT : Cowling Portal Elevator - On Saturday, 03/11/2006 at 12:38 pm the Cowling Portal Elevator went down due to a door switch on the bottom being out of adjustment. The elevator was repaired in Saturday, 03/11/2006 at 01:06 pm, the door switch was readjusted.

ROOF FALL (7000-1-004-06) : LOCATION - Old Main West Left Return, #89 Crosscut, Entry #5. DIMENSIONS - H:6-7', W:23', L:22'. GEOLOGY - Soft, medium to dark gray shale with prominent roof jointing or slips in roof. ROOF SUPPORT : 6' Conventional bolts. The area was mined in November 1987. Prominent roof jointing in the roof were the apparent cause for the fall.

ROOF FALL (7000-1-003-06) : LOCATION - 1 West #4 Roadway, #7 Crosscut. DIMENSIONS - H:6-7', W:20', L:20'. GEOLOGY - Soft, medium to dark gray shale with prominent roof jointing or slips in roof. ROOF SUPPORT - 6' Instal bolts. The area was mined in November 1993. Prominent roof jointing in the roof were the apparent cause for the fall.

HOISTING INCIDENT : Cowling Portal Elevator - On Tuesday, 03/07/2006 at 06:45 am the Cowling Portal Elevator went down due to a door lock switch on the bottom outside door being inoperative. The elevator was repaired on Tuesday, 03/07/2006 at 09:00 am, the door lock switch was replaced.

#4 Entry @ 45 X-cut - Employee was a passenger in mantrip driven by co-worker, sitting in middle seat on right side of mantrip. Operator backed up and hit the rib on the right side causing employee to go back into frame of seat. Injury to neck area and upper back. Released to RTW 03/09/06. Lost 3 Work Days.

ROOF FALL (7000-1-002-06) : Roof Fall was found at the following LOCATION-Outby Main East Seals, #4 Seal. DIMENSIONS-H:6-8', W:19', L: Approximately 30'. GEOLOGY-Soft, medium to dark gray shale with prominent roof jointing or slips in roof. ROOF SUPPORT : 6' Conventional bolts.

Unit # 2 - Employee was installing a roof bolt. Pinned finger between bolt plate and the mine roof. Injury resulted in fractures and a laceration to left middle finger. Returned to Work 03/02/2006. Lost 4 Work Days.

ROOF FALL (7000-1-001-06) : LOCATION : Outby 1 North 3 West Seals - #4 Seal. DIMENSIONS : H:6-10', W:19', L:280'. GEOLOGY : Soft, medium to dark gray shale with prominent roof jointing or slips in roof. ROOF SUPPORT : 6' Hytech point anchor bolts. The area was mined in June 1993. Prominent roof jointing or slips in the roof were the apparent cause for the fall.

Slope Belt - Employee was resetting pull cord. In the process of getting down off the belt, employee slipped, resulting in a strain/sprain to right leg above and below knee.

Roadway, Past Cowling Barn - Employee was driving mantrip, just before crossing under belt, the top fell out from between bolts, striking employee on the left forearm. The piece of rock was approximately 19" x 19" x 2 1/2" thick causing a severe hematoma. Possible injuries to left upper arm and shoulder also.

MW#3 - Employee was resetting timbers when a 6 x 6 timber slipped out of hands. Struck tip of nose on left side, causing a laceration requiring 3 sutures. MEDICAL TREATMENT ONLY.

HOISTING INCIDENT : Cowling Portal Elevator - On Thursday, 02/02/2006 at 08:45 pm the Cowling Portal Elevator went down due to the door switch at the bottom of the shaft being out of adjustment. The elevator was repaired on Thursday, 02/02/2006 at 10:45 pm when the door switch was adjusted after locating the problem.

Supply Yard - Employee was helping truck driver untangle cables. Employee was lifting, pulling and tugging on cable, felt a pop in pelvis/lower back area. Pain going down right leg also. Reported incident. On way back to work, collapsed and began to have a sick feelingin stomach.

Employee was cable bolting when co-worker lowered the foot on employee's left foot causing a facture to 3rd toe and contusions to 4th and 5th toe.

Employee was swinging boom over when a pinner steel flew from the tray, striking employee on right side of cheek resulting in bloody nose and fractures in the cheek area.

Employee was standing by AL Lee machine when co-worker tripped over some metal. Co-worker knocked into employee. Employee fell against AL Lee, striking head against the boom and scraping left arm on the forks. Complaining of numbness in neck, arm, hand and leg on left side. RTW 01/20/06. Lost 4 Work Days.

EE: Incline Belt Tail, Mother Belt Transfer - A long piece of rock was wedged in discharge. Employee was jerking on the rock (4 1/2' long, x 1 1/2' wide x 8" thick) trying to dislodge. After getting rock loose, employee picked up one end to remove from chute. Alledges feeling popping sensation and pain in right shoulder.

1W # 3 Take-Up - Employee was cleaning 1W # 2 tail. hit head, causing a burning sensation in middle of back.

SLOPE HOIST INCIDENT - On Monday, 12/19/2005 at approximately 11:30 am, ice in the shaft broke loose and hit the buffer switch. The cage was fully operational at approximately 01:25 pm on Monday, 12/19/2005.

SLOPE HOIST INCIDENT - On Tuesday, 12/06/2005 at approximately 06:30 strands were discovered broken in the slope rope. A new slope rope was put in and the slope rope was operational at approximately 01:40 pm on Tuesday, 12/06/2005.

(LTA) : Kersey Barn - Employee was loading a nitrogen bottle in back of mantrip. Strained lower back.

11/23/05 @ 12:55 pm-Gas was reported outby in the LOC @ the B1 Seals-18.1% O2, 3.7% CH4. 01:26 pm-MSHA-K. Roberts was informed. 02:00 pm-spoke with MSHA-K. Roberts, D. Whitcomb, & J. Oakes. 02:10 pm-called MSHA, High CH4 @ ME Seals. All power off in mine. 2:35 pm-the ME Seals showed-20% O2, 2.4 CH4. 02:45 pm-all miners were out of the mine, informed MSHA-D. Whitcomb.

HOISTING INCIDENT : Cowling Portal Elevator - On Monday, 11/21/2005 at approximately 12:40 pm, the Cowling Portal Elevator experienced a mechanical component that went bad. The coupler between the motor and reducer. The reducer side was stripped out. The elevator was back up and the mechanical component was functioning properly at approximately 07:00 pm.

1W#3 Belt - Dust hose came apart and hit employee in the face causing a contusion and laceration to upper center lip and inside of lip. Laceration was approximately 1/2" to 3/4" long requiring 4 sutures, 3 on the outside, 1 on the inside. MEDICAL TREATMENT ONLY.

ROOF FALL : (7000-1-014-05) LOCATION - Main E Roadway, Crosscut #104, Entry #4. DIMENSIONS - H:6-8', W:25', L:25'. GEOLOGY - soft, medium to dark gray shale with prominent roof jointing or slips in roof. ROOF SUPPORT - 6' fully grouted bolts.

HOISTING INCIDENT : Cowling Portal Elevator - On Monday, 11/14/2005 at approximately 07:00 pm, the Cowling Portal Elevator experienced an electrical component part that went bad. The field power supply had to be replaced. The elevator was back up and the electrical component was functioning correctly at approximately 08:50 pm

ROOF FALL (7000-1-013-05) : LOCATION - Old Main E Beltline, Crosscut #199, Entry #5. DIMENSIONS - H:5-7', W:30', L:25'. GEOLOGY - soft, medium to dark gray shale with prominent roof jointing or slips in roof. ROOF SUPPORT - 5' fully grounted bolts.

ROOF FALL (7000-1-012-05) LOCATION - Top of Fault Hill, East of Spad #166. DIMENSIONS - H:6-6.5', W:19', L:120'. GEOLOGY - Soft, medium to dark gray shale with prominent roof jointing or slips in roof. ROOF SUPPORT - 6' conventional bolts. The area was mined in March 1974. Prominent roof jointing or slips in the roof were the apparent cause for the fall.

(LTA) : Main E Unit - Employee was walking toward left transformer. Tripped over a piece of curtain board next to hi-line skid causing employee to fall toward end of the transformer. As employee fell, tried to break fall with left hand. Alledges injury to left shoulder and upper left arm.

Employee: Cowling Bottom Tool Room - Employee was getting tool box and come-a-long out of belt crew tool box. The lid of the box (30" x 36") fell and closed on right hand. No fractures. Contusion/bruise type injuries to right hand and right index finger.

ROOF FALL (7000-1-009-05) : LOCATION - 2 West, Crosscut #18, Entry #8. DIMENSIONS - H:7', W:18', L:18'. GEOLOGY - soft, medium to dark gray shale with carbonatious laminations and prominent roof jointing or slips in roof. ROOF SUPPORT - 5' fully grouted supertwist bolts.

ROOF FALL (7000-1-008-05) : Main E Panel - Crosscut #4 Entry #6 Spad #63898. DIMENSIONS : H:7', W:19', L:30'. GEOLOGY : Soft, medium to dark gray shale with prominent roof jointing or slips in roof. ROOF SUPPORT : 5' fully grouted bolts.

ROOF FALL (7000-1-007-05) : 2 West Return - Crosscut 19, Entry # 1. DIMENSIONS - H:6-7', W:19', L:160'. GEOLOGY - Soft, medium to dark gray shale with prominent roof jointing or slips in roof. ROOF SUPPORT - 5' fully grouted bolts.

Main E Unit - Employee was cutting belt with belt knife. The knife slipped and went into right knee, requiring 3 sutures. MEDICAL TREATMENT ONLY, NO LOST TIME.

HOISTING INCIDENT : COWLING PORTAL ELEVATOR - On 09/21/2005 at approximately 04:10 pm, the Cowling Portal Elevator experienced a braking system malfunction which interfered with the use of the hoist for approximately 70 minutes. The elevator was back up and the brakes were functioning properly at approximately 05:00 pm.

Main E Unit - Employee was attempting to install top right rear cover on miner. Pushed cover, injured/twisted right knee.

Lost Time Accident: ME #4 Belt - Draw rock fell out of head hole while beltmen were setting a drive. Employee got on top of belt to examine the roof. While in the process of getting down, stepped off belt into scoop bucket. Then, when getting down off scoop, stepped on a rock (baseball size), twisting left ankle and falling to the ground.

MW3 Roadway - Employee was spotting bolts at #28 crosscut when a piece of rock fell out of top, causing a laceration above left eye (5 sutures required). Employee returned to work after ER visit. MEDICAL TREATMENT ONLY. NO LOST TIME.

Main E Unit - Employee was helping tip up a large rock to get miner hose out from beneath it with 2 other employees. Employee alledges pain down left leg, hip, and near tailbone. Report states this area has been injured once before.

ROOF FALL (7000-1-006-05) : LOCATION - Old 3 West Roadway, Crosscut 52 Entry. DIMENSIONS - H:6-7', W:19', L:110'. GEOLOGY - soft, medium to dark gray shale with prominent roof jointing or slips in roof. ROOF SUPPORT - 6' Hytech point anchor bolts.

LOST TIME ACCIDENT. Main E Unit - Employee was trying to untangle miner cable. As employee was bent over, he stepped backwards while standing up. There was a rock on the ground and his foot (back of heel) caught rock; he fell down backwards. Contusion and swelling to left lower leg. Started missing 08/09/05.

ROOF FALL (7000-1-005-05). LOCATION - 1W#3, Roadway Crosscut 92, Entry 7. DIMENSIONS - H:15', W:19.5', L:20'. GEOLOGY - Soft, medium to dark gray shale with prominent roof jointing or slips in roof. ROOF SUPPORT - 6' Hytech point anchor bolts.

EE: 2W Unit - A piece of rock 2" thick by 2' by 2' fell out of top and hit employee on top of the head, neck, left shoulder, and left thumb. Complaining of lower back pain a day later also.

EE 2W Unit - Employee was helping a co-worker put cable bolt in hole. The glue was starting to set up. The bolt sprung and the roof plate struck employee on the right forearm causing a puncture/laceration wound requiring 9 sutures.

SLOPE HOIST INCIDENT - A kink was discovered in the slope rope on Wednesday, 07/20/05 at 06:00 pm. The slope was taken out of service. Repairs were made on 07/22/05 and the slope was put back in service.

GOOCH, ORA (LTA) : 2W Unit - Employee was trying to remove rock, between telebody and push plate jack. It was too large to move, so employee used a sledge hammer to break the rock. While striking the rock with the hammer, employee hit the rock with the handle where hands were located before hammer head hit the rock. Fracture to left 5th digit.

LOST TIME ACCIDENT. Main E Unit - Employee was pulling oxygen bottle out of truck when right thumb was smashed between bottle and truck bed, causing a contusion. Continued working until surgery (tendon repair) on 08/11/05.

Employee alledges he was walking back to dinner hole. Stumbled on a lump of coal, fell backwards, pulling hamstring in right leg.

5th wheel trailer hitch was lower than tractor hitch. Employee continued to push trailer trying to get under it to hook up. In the process, employee knocked the leg down that was supporting heinzman beam. The beam fell down about 2 feet, striking employee in the head, neck, and right shoulder.

Employee heard a noise coming from refuse screen. Put hand between springs and side panel causing abrasions to right wrist and hand. Amputation of right middle finger and fingernail requiring skin grafts. Strain to back occurred as employee pulled hand back from machine. Delay in reporting due to waiting on medical reports. MEDICAL TREATMENT ONLY. NO LOST TIME.

Employee was straightening frame of transformer. Prying frame with a slate bar, using a lot of force. The bar slipped free, pulling employee's body forward. Experienced pain in right shoulder immediately. RTW 04/19/2005. Lost 4 Days.

Employee was putting in a 14' cable bolt. As employee was pulling and pushing it in the hole, experienced pain, starting in left side of neck, going down into left upper back and left shoulder. Continued to work. Disabled on 03/08/05.

Employee was allegedly lifting oxygen and acetylene bottles into empty trailer. Complained of strained back or ribs. The next morning, experiencing severe lower back pain.

Employee was bolting brow of roof fall. On left side of bolter using two pieces of steel. Upper section of the roof steel hung up in the hole when EE dropped the pod. Steel dropped out after it cooled & struck rt. thumb mashing it between upper steel & drill pot causing a fracture and laceration. NOTE: Amended Narrative per misc. mail 1/25/05 #1.

EMPLOYEE WAS SITTING ON A BUCKET, COOLING OFF AFTER HANGING CABLE FOR REMOTE. HE TOOK OFF HIS HAT TO SWIPE OFF HIS FOREHEAD. A ROCK 1' SQUARE AND 1" THICK FELL FROM THE ROOF AND STRUCK HIS HEAD. THE LACERATION TO HIS SCALP REQUIRED STAPLES.

EE was installing outside bolts next to ribline. While in drilling procedure, a piece of top (6" thick 36" long 31" wide struck backside, pushing ee into bolter boom, injuries included contusions to back, nose (fractured) and mouth.

Main E Unit-While screwing a bolt into resin, ee slipped and fell down on drill pod. knocked co worker over also. resulting in a strain to right knee.

Mantrip was struck in belt entry near feeder. as coal hauler approached feeder to discharge load of coal, hauler operator did not see mantrip. Tailgate of hauler made contact with mantrip causing injury to rt foot/toes when they were caught between two pieces of equipment. Injured was a passenger in mantrip.

Employee crossed belt tail to check fire suprression box. as he stepped down with hand on water line, arm gave way. EE landed on left ankle causing it to fold under his own weight resulting in a fracture.
