EE was function testing shields when the pan line was pulled back coming into contact with a knee. EE did not seek medical attention till 11/7/2019 becoming a RLT on 11/11/2019. Cause of this incident was lack of EE concentration. PPE was not a factor, mining systems was not a factor, compliance with rules/regs was not a factor, miner proficiency may have been a factor.

EE was operating shearer when a piece of rock came from drum area striking EE in the chin resulting in a laceration. Cause of this incident was lack of knowledge. PPE was not a factor, compliance with rules/regs was not a factor, mining systems was not a factor, miner proficiency may have been a factor.

EE was installing cable bolt when expander became caught in screen, while freeing expander EE inadvertently rotated causing bolt to strike wrist. EE sought medical attention 10/13/18 becoming RLT. Cause of this incident was lack of EE concentration. PPE not factor,mining systems/compliance with rules/regulations not a factor, miner proficiency was factor.

Employee was operating the continuous miner when a piece of foreign debris scratched EE's eye resulting in medical eye drops. Cause of this incident was employee knowledge. PPE was not a factor, mining systems was not a factor, compliance with rules/regs was not a factor, miner proficiency may have been a factor.

While employee was relieving pressure on duke jack, jack became dislodged catching employee's thumb between spill tray and jack. Lack of employee concentration was the cause of this incident. While removing a screw jack from underneath a shield canopy, employee placed hand between the crib block capping the jack and the shield canopy. With hand in this position, the shield depressurized, pinning EE's hand between the shield canopy and the crib block, causing a fracture and laceration to the right ring finger requiring stitches.

EE was drilling hole into mine roof to install roof bolt, after drilling ee was removing drill steels when the top drill steel fell out of the roof striking ee on the finger resulting in a laceration, cause of accident was lack of concentration. PPE was not a factor, compliance with rules/regs was not a factor, mining systems was not a factor, miner proficiency was a factor.

Employee was using a bench grinder when his thumb came into contact with the grinding wheel. Employee's lack of awareness was the cause of the incident. PPE was not a factor, mining equipment/systems was not a factor, compliance rules/regs was not a factor, miner proficiency was a factor.

Employee was using a hand grinder when a piece of foreign material entered his eye. Cause of incident was employee's lack of awareness. PPE was not a factor, compliance rules/regs was not a factor, mining equipment/system was not a factor, miner proficiency was a factor

Employee was using a drill when the drill bit caught and spun the drill out of his hand striking the back of his right hand. Cause of incident was employee lacking to hold onto drill. PPE was not a factor, Mining equipment and mining systems was not a factor, compliance rules/regs was not a factor, miner proficiency was a factor.

While drilling a 12' hole for a torque bolt employee was struck in the left ring finger by drill steel. Cause of the injury was employee not holding drill steel in the top when adding additional steel. While doing rehab work employee was running miner when a rock was thrown 5' past miner striking victim in the left foot. Cause of the accident was 8' bolt caught bit lug and threw rock rearward. Miner operator was pinned between standoff cable handler of a continuous miner and coal rib in #1 entry of 5 west development section. Cause of incident was miner operator not paying attention. PPE was not a factor, compliance rules/regs was a factor, mining equipment/system was a factor, miner proficiency was a factor.

Employee was standing in the middle of the offside of roof bolter, during bolter reposition his foot was struck by the front tire. PPE was not a factor, mining equipment was not a factor, mining systems was not a factor, compliance rules/regs was not a factor, miner proficiency may have been a factor.

Employee was cutting belt structure stands when a piece of metal got into his eye. Employee did not seek medical attention until 8/04/2014 where he then received eye drops. PPE was not a factor, mining systems was not a factor, compliance rules/regs was not a factor, miner proficiency may have been a factor.

He was kneeling on the pontoon of shield looking for bolts, heard warning for pan line push went to move foot slipped and was pinched. PPE was not a factor, Mining system s was not followed, compliance rules/regs was not a factor, miner proficiency was a factor.

Employee was watching cm operator cut down stone during stone work when a piece of small metal came off the cutter head striking employee in the right cheek. PPE was not a factor, mining equipment/mining systems was not a factor, compliance rules/regs was not a factor, miner proficiency may be a factor.

Employee was removing a broken cutter shaft from the shearer; snap ring was removed causing hot oil to come into contact with the employees left cheek, hands, wrist and lower arms causing burns. PPE may be a factor, mining equipment/mining systems was not a factor, compliance rules/regs was not a factor, miner proficiency may be a factor.

Employee standing near #4 shield when a rock laying on the mine floor pinched his foot between the rock and the panline. Cause of the injury was the rock being near the movenment of the shield. PPE was not a factor/mining equip was not a factor/mining systems was not a factor, compliance rules/regs was not a factor, miner proficiency may be a factor.

EE was standing next to panline with his hand resting on top of the spill board. The shearer went by & his hand was pinched between the bretby & the metal spill board.Cause of this injury was employee not paying attention to hand placement. Compliance with PPE rules/regulations/mining equipment systems not a factor. Miner proficency/attitude could have been a factor.

Miner operator was pinned between standoff cable handler of a continuous miner and coal rib in #6 Entry of Productivity Mains. Cause miner operator not paying attention. PPE was not a factor, compliance rules/regs was a factor, mining equipment/sytem was a factor, miner proficiency was a factor.

Injured was standing on the pontoon toes of long wall shield, pan was pushing his foot slipped off and got caught between relay bar and pontoon shields. PPE was not a factor, mining equipment/system was a factor, compliance rules/regs was not a factor, miner proficiency was a factor.

Injured was pulling pan line back and got left foot between relay bar and shield toe. PPE was not a factor, mining equipment was a factor, compliance rules/regs was not a factor, miner knowledge was a factor.

Employee attempting to align a drill steel to the other, he hit the wrong lever on the bolter (tilt lever) and pinched his finger between the two drill steels. Cause of this incident was the inattention of this employee and having the drill steel in the drill pod to align. Compliance with rules/regs protective equip./not a factor, mining equip and miner proficiency a factor.

Drilling hole for roof bolt, he was lining up drill steels, he moved the feed lever rather than the rotation lever. The steels pushed together pinching index finger. PPE was not a factor, mining equip/system was a factor, compliance rules/regs was not a factor, miner proficiency was a factor.

Employee was moving tailgate shields his hand was on top of panline rail. Rock fell when it slid off of top of shield striking middle finger on right hand. PPE was not a factor, compliance rules/regs was not a factor, mining equipment/systems was a factor, miner proficiency was a factor.

Employee adjusted leveling chain on roof bolter, when he picked drill pot something in leveling system broke and hit me in the chin and neck. PPE was not a factor, rules/regs was not a factor, mining equipment/system was not a factor, miner proficiency could be a factor.

Employee was removing a bent roof bolt out of mine roof. The employee reached back to drop drill head and moved the joystick for drill rotation. Bent roof bolt spun striking employee in left wrist. PPE was not a factor, compliance rules/regs was a factor, mining equipment/system was a factor, miner concentration was a factor.

Employee was roof bolting on long wall face, dropped steels from roof the steels got stuck causing a bind causing the drill mast to jump. Put hand on steels while trying to remove steels, they jumped pinching fingers between mast and steel. PPE was not a factor, compliance rules/regs could have been a factor, mining equipment was a factor, miner proficiency was a factor.

Pinched thumb while tightening bolt with impact wrench.

Standing on #270 shield started pushing tailgate drive out, foot slipped off shield toe and instead of pushing the pan it advanced and caught his left foot. PPE was not a factor, compliance rules/regs was not a factor, mining equipment/system was a factor, miner proficiency was a factor.

Mining a sixty to the right, while cutting the roof, the miner bits caught a bacon strap taking out a couple roof bolts allowing the bacon strap to be thrown back towards the miner operator striking him at the bridge of the nose and above the eye. PPE was not a factor, compliance rules/regs was not a factor, mining equipment/system was a factor, miner knowledge was a factor.

During a Part 50 audit, it was determined this injury reportable. Employee was grinding a weld prior to lifting his welding hood, a piece of slag went into his eye. Cause of this injury was knowledge of metal on hood. Compliance/rules, PPE, mining equip/systems not a factor, miner proficiency may be a factor.

Burned finger while cutting, was using PPE

This injury was to have occurred while this contract labor was supplying a bolter with bolting supplies and bolter operator moved the machine, ran over his foot. Cause of this injury was lack of communication with other workers in area. Compliance with rules/regs not a factor, mining equip/systems, or PPE a factor and job skills and miner proficiency may be a factor.

Had left hand placed on long arm of drill was bringing drill back from the roof to add another steel. Finger got pinched between long arm and rotation hose fitting cutting tip of finger. PPE was not a factor, compliance rules/regs was a factor, mining equipment/systems was not a factor, miner proficiency was a factor.

During a Part 50 audit, it was determined this injury to be reportable. Employee was standing beside someone welding and received a welding flash to his eyes. Cause of this injury was looking at the welding process. Compliance with rules/regs, mining equip/systems not a factor. PPE and miner proficiency may be a factor.

Walking by headgate drive on the long wall a rock flew out the panline chain and struck him lacerating his lip on his face. PPE was not a factor, compliance of rules/regs not a factor, miner proficiency was a factor.

While helping fellow roof bolting partner, trying to bend a roof bolt he dropped the drill pot down and caught my right foot between mine floor and drill pot. PPE was not a factor, compliance rules/regs not a factor, mining systems/equipment was a factor, miner proficiency was a factor. This person worked after the incident and is now off 8/10/2011 last day worked.

While taking drill steel out of roof bacon strap, it became separated from the first inby steel in the roof. When it came out of the hole and swung around striking him in the left ear and head. Lacerating ear and head. PPE was not a factor, compliance rules/regs were not a factor, mining equipment/systems was a factor, miner proficiency was a factor, also.

Yellow wire holding bacon strap on ATRS pad broke causing bacon strap to hit fore head above right eye. Cutting fore head. PPE was not a factor, compliance rules/regs not a factor, mining equipment/system was not a factor, miner proficiency was a factor.

Putting on winch cable on scoop while winding on cable glove got caught in crosby clamp and cable, lacerating left small finger. PPE was not a factor. Compliances rules/regs was not a factor, mining equipment was not a factor, miner profiency was a factor.

Installing roof bolt, roof bolt drill wrench slipped off bolt head pinching finger between wrench and roof bolt plate. Cause of the injury was safety awareness of the employee. Compliance with rules/regs not a factor, ppe was worn so not a factor, miner profiency a factor, mining equip/systems not a factor

The injured was at 114 shield pulling in 115 shield, he heard rock fall between shields behind him. When he turned, the pontoon from 115 shield caught his right foot. It pushed on the side of his boot smashing the meta-tarsal to the top of the foot. PPE was not a factor, compliance rules/regs was not a factor, mining equipment/system was a factor, miner profiency was a factor.

Installing cable bolt, the bolt was bent & the glue had broken. He was holding roof bolt with right hand & reached to lower drill pod. He accidently moved the rotation lever which spun the bolt and the plate came around striking his right forearm. Which created a cut.

Employee was installing a cable bolt in a drilled hole. Cable bolt roof plate slid down the bolt shaft striking the victim lacerating the right wrist area.

Installing a cable bolt in mine roof, accidently hit rotation lever which spun bolt while it was in his hand and hit on right wrist. Cause was concentration. PPE was not a factor, compliance of rules/regs was a factor, mining equipment/system not a factor, miner profiency was a factor. This person worked after the incident and is now is off. 8/30/2010 was last day worked.

Drilling outside roof bolt hole, brought drill head back down after drilling hole and brought it back on top of right upper leg. Cause of the injury was concentration. Mining equipment/system was not a factor, ppe was not a factor, compliance rules/regs was not a factor, miner profiency is a possible factor.

While adjusting a shield, a rock flew from the cutter head, striking him in the lower back. Cause is postioning himself to far out from the cutter head. PPE is not a factor, compliance of rules/regs not a factor, mining equipment was not a factor, miner profiency was a factor. This person worked after the incident and now is off work, 6/13/2010.

Drilling with first drill steel into hole reached down to grab 2nd drill steel, first drill steel fell out of roof striking him in the index finger. PPE was not a factor, compliance rules/regs not a factor, miner proficiency was a factor.
