A mechanic failed to wear appropriate eye and face protection while assisting a welder during welding operations. The mechanic suffered from eye discomfort with no permanent damage.
While observing the installation of the master link in the highwall miner belly chain, a small piece of metal broke loose and struck EE in the right forearm. Resulting in a small laceration and the embedding of the foreign object. EE treated at the Hospital with a tetanus shot and a prescription anti-biotic.
Due to the buildup of material in the catchment bench, a Komatsu PC300 excavator was being used to remove material and buildup the outer berm. The PC300 lost control of a large rock that traveled approximately 600' downhill and passed through a mobile home. The home was damaged beyond repair. The occupant was bruised up after falling through the floor.
Operating a dozer and felt pain in back & neck.
Questionable incident - Employee was operating a D10R dozer and ran over what appears to be a primer or large bolter causing the dozer to bounce straining back, neck, & shoulder. Employee continued to work. Lost time began 09-06-11 possibly related to this incident.
EE was pushing in a shot when he backed into a rock causing a quick jerk to his lower back. EE is off work pending the results of an MRI.
Employee was pushing rock with a Cat. D11 dozer when the blade of the dozer slipped on a rock, causing a quick jerk to the operator. Employee complained of lower back pain. He drove himself to the Mary Breckingridge Hospital in Hyden, Ky where an MRI is pending.
EE was in the process of cutting a beam with an acetylene torch when the oxygen hose came apart at the connection to the torch, causing a burn to his left hand. Diagnosis is pending.
