Equipment operator stated EE wasn't feeling good and asked to go home. Operator missed two days the week prior with the flu. After operator badge out and left active mine area to personal vehicle area the operator entered EE's vehicle at approximately 7:51 and was found diseased in EE's vehicle approximately 6:15 p.m.

Employee-EE, first reported to Clinic on 04/02/2024 that on 03/25/2024 they felt something in left eye and has had pain and irritation since. On eval. on 04/02/2024, nothing was visualized and EE was referred to ER for evaluation and treated for eye irritation with prescription eye drops. EE was able to return to duty with no restrictions or lost time to report.

EE reports back pain after standing up straight from a bent over position. EE requested over the counter meds on 4/20 but no other care was given. Clinic was notified on 4/24 that EE is off work due to back pain. EE was sent to Urgent Care and given restrictions the area cannot accommodate; EE is off work until further notice.

Employee reported discomfort in right shoulder on 9/13/22. EE states right shoulder continued to bother EE since incident occurred 6/25. EE was referred to Ortho for additional evaluation. EE continued to work with no restrictions. EE had surgery on 1/27/23.

Employee was en route to EE's job at the time of the vehicle accident. The cause of death was a medical cardiac event.

Employee found unresponsive. Apparent heart attack.
