On 2-19-26, a teammate reported eye pain in BD 10. After using an eye wash, they requested a medical evaluation on 2-23-26 for persistent discomfort. While no debris was found, a corneal abrasion was diagnosed, and prescription drops were issued for recovery.

While removing a jam up at the Case Packer teammate stood up form a kneeling position and felt a sharp pop in their right knee.

While attempting to return to the standing position after being bent over under the railcar, the teammate felt back pain in the middle of their back.

Team Lead turned leg the wrong way while working in the Baghouse, but stated was okay and no medical attention needed. Teammate wanted medical attention the next day and was driven to medical facility to get checked. Doctor released teammate to restrictive light duty and given a knee brace.

Employee reported something blew into EE's eye and notified company 4 days later of the incident. Prescription medicine given for treatment.

Bent over to tie EE's shoe and lower back started hurting.

EE noticed soreness/pain around pre-existing injury, torn right Achilles tendon, 11-20-18. EE reports inflammation of said injury due to using stairs to access a platform. EE reported injury 11-26-18 and medical treatment at ER & Orthopedic doctor. Diagnosis - tendon not torn, no surgery needed, tendon simply needs to heal. Cast/walking boot & crutches provided.

EE was on a forklift getting a roll of poly bags down to take to the packaging line. Prior to doing this the teammate must take the plastic wrap and cardboard off of roll and lay to the side. When EE came back to clean up the trash, got off the lift and squatted to pick up the plastic and felt a pop or something pull in lower back.

The incident is a hearing loss over time and was found during the bi annual health assessment given to all employees during the 2010 year.

EE was riding on golf car when wind blew dust into face and right eye. Washed face and right eye, but still irritated. Sent to doctor diagnosis was right eye corneal abrasion. Given eye drops and pain medication. No lost time just restricted.

Operator was adjusting grippers and while bending he started experiencing minor lower back pain.

EMPLOYEE WENT UNDER A CONVEYOR AND WHEN HE STOOD UP, HE FELT A PAIN IN HIS BACK.

EE WALKED OUTSIDE TO GET A TRUCK DRIVER WHEN SOMETHING FLEW INTO HIS EYE. HE TRIED TO FLUSH THE OBJECT OUT BUT COULD NOT. EE WENT TO THE HOSPITAL AND WAS GIVEN 2 DAYS OFF. ON 4/19/01, THE SUPE RVISOR TURNED IN THE DR'S EXCUSE, HE STATED THE INJURY WAS NOT WORK RELATED. 4-23-01 EE SAID HE NEEDED A STATEMENT FROM WORK REGARDING HIS LEFT EYE INJURY SO HE COULD GO TO THE EYE SPECIALIST

HE REPORTED THAT HE FELT HIS BACK BEGIN TO HURT AFTER HE WENT HOME; IT GOT WORSE OVERNIGHT & HE COULD BARELY MOVE DUE TO PAIN. HE WAS EXAMINED BY COMPANY DR. ON 6/16/00 & IT WAS DETERMINED TH AT HIS CURRENT PROBLEM STEMMED FROM A PREVIOUS BACK INJURY IN 1992. THERE WAS NO CURRENT INCIDENT TO SUGGEST THE INJURY OCCURRED ON THE DAY IT WAS REPORTED TO HIS SUPERVISOR.

EMPLOYEE WAS WALKING NORTH BETWEEN LINE 5 AND EAST WALL. THE SURFACE WAS LEVEL AND DRY. TURNED 180 DEGREES TO THE RIGHT. HE EXPERIENCED A SHARP APIN IN HIS RIGHT LEG.

WHILE WALKING UPSTAIRS AT WORK HE FELT A PAIN IN HIS BACK PAIN RADIATED INTO HIS LEFT BUTTOCK & DOWN THE UPPER THIGH TRETED BY A CHIROPRATOR & LATER BY OUR CO DR HE WAS GIVEN LIFTING RESTRICTI ONS OF NO MORE THAN 25 POUNDS RELEASED TO NORMALDUTIES ON 06-29-92

WIND BLEW FOREIGN OBJECT INTO EE EYE EE WAS WEARING SAFETY GLASSES.

OPERA TOR WAS WORKING OUTSIDE & THE WIND BLEW A PIECE OF CLAY INTO HIS EYE THE CLAY SCRATCHED HIS RT EYEBALL

EMPL WAS SITTING IN CONTROL ROOM OF MILL BUIDLING WHEN HE NOTICED RIGHT EYE FELT IRRITATED. HE WENT TO CO EYE DR ON 3/16 & WAS TREATED FOR CORNEAL ABRASION TO RIGHT EYE WITH RX OINTMENT. NO FO REIGN BODY WAS FOUND HE RETURNED TO NORMAL DUTY 3/16/90.

EE WAS WALKING THRU THE PLANT TO THE BREAK ROOM WHEN HE FELT SOMETHING ENTER HIS EYE. HE WORKED THE REST OF THE SHIFT WITHOUT PROBLEM. LATER HE DECIDED HE NEEDED TO HAVE IT CHECKED AND HE WENT TO HIS OWN DR. AN EMBEDDED FOREIGN BODY WAS REMOVED AND RX MEDICATION GIVEN. NO LOST TIME OR RESTRICTED DUTY WAS REQUIRED.

EMPLOYEE WAS PLACING A PALLET IN THE STACKING STATION. A WASP NEST WAS IN THE PALLET. AS EMPLOYEE LIFTED PALLET, THE WASP STUNG TWICE ON THE UNDERSIDE OF THE RIGHT BICEP.

EMPLOYEE WAS PREPARING TO LEAVE FOR THE DAY WHEN HE FELT A FOREIGN BODY ENTER HIS LEFT EYE.HE WASHED THE EYE OUT WITH TAP WATER.THE NEXT MORNING HE AWOKE WITH HIS EYE DRAINING AND IRRITATED.HE WAS REFERRED TO THE OPTHAMOLOGIST FOR REMOVAL OF AN EMBEDDED FOREIGN BODY IN THE CORNEA.RX OINTMENT WAS ORDERED.HE RETURNED TO WORK WITH NO RESTRICTION.

EMPL WAS WALKING THROUGH SOUTH PLANT ON WINDY DAY. DUST PARTICLE ENTERED RIGHT EYE. HE WAS ENT OT COMPANY DR WHO REMOVED PIECE OF CLAY FROM EYE WITH EYE SPUD. HE WAS TREATED WITH RX MEDICATION . NO RESTRICTIONS.

EMPL WALKING THROUGH WAREHOUSE FELT BURNING IN RIGHT EYE.WASHED IT OUT WITH EYE WASH.CONDITION CONT TO WORSEN AND 3 HRS LATER REPORTED TO NURSE,REFERRED TO OMPANY DR.TREATED FOR CORNEAL ABRASI ON WITH EYE PATCH AND RX MED.RET TO REGULAR JOB AS SUPERVISOR.CONDITION RESOLVED.

EMPLOYEE WAS WA5KING THROUGH THE PLANT, UNDER THE ELEVATOR WHEN H E FELT A BITE ON HIS RIGHT LOWER LEG, A FEW HOURS LATER, HE NOTED THAT IT WAS RED, PAINFUL, AND ULCERATING. HE WAS SENT TO THE CO. MD FOR EVA5UATION, AND WAS TREATED FOR AN INSECT BITE. IT SUBSEQUENTLY BECAME INFECTED, AND HE WAS TREATED WITH ANTIBIOTICS.
