At 14:50, IP had been clearing brush and felt a sting on their neck. IP told ERT/EMT that they were feeling numbness and tingling in extremities. IP stated they threw up after the sting. IP vitals were elevated. EMT transferred IP into ERT ambulance and transported IP to meet with UFA at Notch gate at 15:35.

The death was caused by a heart attack, as confirmed on the death certificate. This was a personal medical event unrelated to work. No workplace factors contributed, and no controls could have prevented a condition not linked to occupational exposure.

Employee was found deceased in their office. Initial conversion with the coroner stated that the cause of death was not work related and most likely a personal medical issue.

At the end of shift, individual was cleaning the back of their pickup bed transferring trash into a large roll-off waste bin. This is when some unknown dust or debris was caught by the wind and got into their left eye.

A metal fragment blew into millwrights eye

IE was looking for printer paper while bent at the waist felt a sharp pain in lower back. IE contacted health services company for advice, due to symptoms described OC meds were prescribed. After experiencing discomfort while at rest the IE setup a clinic visit and received prescription medication seven days after initial injury.

A production supervisor was walking up a flight of stairs and felt a pop in the calf muscle

Employee was in an open field watching a valve to check for leaks. A bee stung EE in the back of the neck. There were Bees noticed in the area on wild flowers but were not agitated or angry. The employee is allergic to stings and was taken to ER for treatment. EE received shots and released for normal work duties. EE's shift had ended so EE returned the following morning.

While repairing a seat in a drill, a craftsman's hand was caught between the door and frame when a high gust of wind caught it and slammed shut, impacting fingers on the left hand. EE's left, middle and ring finger had the pads on fingers above the first knuckle tore off, and small laceration to tip of index finger.

Employee suffered a fatal cardiac arrest on 10/4/2017 due to a personal illness. Full description obtained by MSHA Inspector - Rocky Mountain District.

The death was caused by a personal medical condition. The employee was found unconscious and non-responsive. The incident is currently being investigated by MSHA as a non-chargeable incident.

The employee became confused and said that he wanted to find his father. He also claims to hear voices.

Employee was adjusting a new 'Self-Closing' door mechanism using a ladder. While the employee's helper opened the door to assist with the adjustment, a gust of wind forced the door open quickly, causing the tracking arm to slide out from the control arm and strike the employee.

Employee experienced pain/discomfort in his left eye as a result of contact with an insect/spider while operating equipment. Employee was evaluated by a medical professional and treated with prescription antibiotic for possible infection and was returned to work without restrictions.

Employee was standing near forklift talking with the operator. The door was latched in the open position, a strong gust of wind unlodged the door and pushed the injured employee into the frame of the operators compartment from the back. His forehead struck the grab bar resulting in a laceration that required stitches despite the hard hat and safety glasses the worker was wearing.

Employee passed away on site at 11:34 a.m. on 06/13/2010. Cause of death appears to be due to a personal medical problem.

The employee was eating lunch outside at a picnic table and felt a bite on the backside of his right knee. The employee dismissed the incident as an ant bite. The next day, the employee sought treatment at a local hospital. The physician diagonosed the injury as a black widow spider bite. The employee is scheduled to return to work on Friday, July 11th.

EE stated that on the 8th or 9th of April he was working in the shop when an unknown event strained his left elbow. He reported injury on 4/10/08, took EE to DR and could not find any damage, but gave EE restricted duty.

Employee had bent over to write down purchase order numbers on a small note pad as part of receiving process. As employee stood up, the individual felt a pain in lower back. Once employee was able to stand the individual could not stand up straight as they normally would. Employee felt that they could just work it out and with a good nights sleep and feel better in the morning.

Poor decision making to rotate 14" diameter sch. 20 pipe on pipe stands rather than utilizing proper lifting equipment conveniently available at jobsite. Pipe fell from stand, employee injured when moving away from pipe. Pressure on right ankle caused by reactionary movement resulted in ankle fracture.

The EE passed away while working along the conveyor belt below the crusher. The worker was positioned on the catwalk/walkway, when he lost consciousness. A co-worker caught him and laid him on the walkway. The death is believed to be caused by natural causes.

Employee bent down and felt pain in left ankle. Resulted in a sprain/strain to ankle which will require Physical Therapy.

While loading drill blast holes with anful (ammonia nitrate and diesel fuel) a wind gust blew dust into employee's right eye. The employee sustained a chemical burn from ammonia nitrate dust on cornia of eye.

EMPLOYEE WAS MOVING EMPTY BAGS AND FELT A SHARP PAIN FROM AN INSECT BITE.

Employee reported to work. Before receiving his work assignment, he experienced symptoms of heart attack. CPR was administered and medical personnel called to the scene. Employee was transported to Jordan Valley Hospital.

EE STATES HE FELT A FOREIGN OBJECT IN HIS LEFT EYE WHILE WALKING IN BORIC ACID PLANT. PARTICLE REMOVED (WITH Q-TIP) FROM UPPER LID BY PHYSICIAN. USE OF PRESCRIPTION MEDICATION.

EMPLOYEE APPEARS TO HAVE EXPERIENCED A CHANGE IN HEARING SENSITIVITY RELATIVE TO THE BASELINE AUDIOGRAM OF AN AVERAGE OF 25 DB OR MORE AT 2000, 3000 AND 4000 HZ IN EITHER EAR.

The worker was preparing to go home at the end of shift. He had showered and changed into his street clothes. He was sitting on a bench in the mine office. He got up from a sitting position and turned his head to the left. He felt a "pop" in his neck and pain and discomfort ensued.

WHILE LAYING ON BACK INSPECTING THE REPLACEMENT OF A ACM MILL BELT, TAIL DUST FELL ONTO HIS FACEFROM ABOVE. WHEN THE EMPLOYEE STOOD UP TALC DUSTON SAFETY GLASSES FELL INTO THE EMPLOYEE'S LEFT EYE. REQUIRING MEDICAL ATTENTION TO REMOVE THE EMBEDDED FOREIGN OBJECT, FROM LEFT EYE.

THE EMPLOYEE STATES HE WAS REACHING UP WHEN HE SUDDENLY FELT A SHARP PAIN AND WAS NO LONGER ABLE TO BREATHE. HE WAS REACHING UP TO LOOSEN A TURNBUCKLE. THE EMPLOYEE HAD A COLLAPSED LUNG & IT DOES NOT APPEAR TO BE JOB RELATED. THE EMPLOYEE WAS HOSPITALIZED.

THE EMPLOYEE STATES THERE WAS BLOWING DUST AND DIRT IN THE PIT AND HE GOT SOMETHING IN HIS EYE. HE RECEIVED A CORNEAL ABRASION AND WAS GIVEN EYE ANTIBIOTICS. AN EYELASH WAS REMOVED FROM HIS EY E.

EMPLOYEE WAS OPERATING SHOVEL AT THE O7 RELOAD WHEN A BEE STUNG HIM ON THE RIGHT FOREARM. EMPLOYEE WAS TREATED TWO DAYS LATER FOR INFECTION.

EE WAS STANDING IN READY AREA WHEN HE WAS STRUCK ON TOP OF THE HEAD BY ANOTHER EE.

WHILE OPERATING AN ELECTRIC SHOVEL, A GUST OF WIND BLEW DUST THROUGH OPEN WINDOW, AND INTO EYE.

EE WAS WALKING OUT OF #3 DRIVE HOUSE WHEN DUST FELL DOWN ON FACE AND HEAD AREA. EE WAS EVALUATED AT CLINIC AND PRESCRIPTION MEDICATION WAS GIVEN FOR IRRITATION.

WHILE DRESSING FOR WORK-WORKER COMPLAINED OF SHARP RADIATING CHEST PAIN AND NAUSEA. HE WAS TREATED FOR HEART ATTACK SYMPTONS, STABILZED AND TRANSPORTED TO HOSPITAL. ON ROUTE TO HOSPITAL OFF TH E MINE PROPERTY, EE SUFFERED A HEART ATTACK DUE TO 100% BLOCKAGE OF MAIN ARTERY

GUST OF WIND BLEW LINER 10'-15' IN THE AIR. INJURED PARTY WAS LIFTED UP WITH THE LINER AND FELL TO THE GROUND, LANDING ON HIS BACK. CONTUSIONS TO HEAD, CHEST, RIGHT LEG, MUSCLE STRAIN NECK.

WIND BLEW BORAX & GRINDING DUST INTO EYE. A SMALL PARTICLE WAS REMOVED FROM HIS EYE BY A DR. THERE WAS NO LOST TIME.

EE WAS CARRYING SCRAP SIDING MATERIAL 30"X5' LONG FROM THE E CYCOLOONE BLDG. TO TRASH CONTAINER LOCATED 100 YARDS. FROM BLDGE S/W WHEN THE EE REACHED UP TO PUT IN THE TRASH CONTAINER. THE WIND BLEW THE SIDING MATERIAL FROM HIS RT HAND LACERATING THE RING FINGER PALM SIDE, FIRST JOIN 8 STUTURES.

WORKER WAS ACCIDENTLY CUT ON FINGER BY ANOTHER PERSON IN THE MILL LUNCHROOM.

EMPLOYEE WAS GUIDING 40" BLASTER PIPE INTO THE FUSION MACHINE BY HAND. A GUST OF WIND CAUGHT THE PIPE THE EMPLOYEE WAS HOLDING, CAUSING HIM TO TWIST AND STRAIN HIS BACK.

RECD MEDICAL REPORTS VIA FAX FROM THE ST PAUL INS CO ON 5/22/97 REGARDING AN EYE INJURY DATED 3/8/97. THE EE WENT TO THE DR ALONE AFTER WORK AND HAD MULTIPLE FOREIGN BODIES REMOVED FROM BOTH OF HIS EYES. NO INJURY WAS EVER REPORTED TOT HE SITE SUPERVISOR OR SITE SAFETY

A TRESSPASSER'S BODY WAS FOUND FLOATING IN A POND ON U.S. SILICA PROPERTY. AN AUTOPSY SHOWS DEATH DUE TO DROWNING. WOMAN WAS BELIEVED TO HAVE ENTERD THE POND VIA A PUBLIC ACCESS ROAD WHICH BOR DERS THE POND.

THE MINER EE WAS GETTING TIMBER FOR THE MINE. HE WAS ROLLING A TIMBER OVER TO PICK IT UP WHEN HE WAS BITTEN BY AN ALLEGED BALCK RECLUSE SPIDER. IT BIT HIM ON THE RIGHT THUMB.
