The employee's right pinky finger was caught between the latch and the door of a Rotex machine, resulting in a fracture to the finger.

The employee was removing guard when EE felt a pain in their shoulder. Employee was diagnosed with torn rotator cuff but, continued to work full duty until 2/14/2025 when EE had surgery.

While performing cleanup, the floor plating the employee was standing on fell into the water ditch resulting in a fractured leg.

Employee was descending the ladder of a road grader and felt discomfort in EE's right knee. EE was later diagnosed with a meniscus tear and underwent surgery on 11/7/24 making it a reportable incident.

Employee was setting up bulk bag area to start filling sacks of sand. The employee placed a pallet on the load stations. When EE bent over to start centering pallet to the spout, EE felt pain in lower right side of back.

Mill G classifier #4 on the 6th floor had a bearing that over heated and caught the grease on fire.

A deceased body was discovered in the South Ottawa Pit. The person is not a mine employee. The Sheriff and MSHA were notified immediately upon discovery and the investigation is ongoing. The birthdate of the decedent is unknown.

At approximately 10:30 PM smoke was observed in the B Pit Pump house due to the overheating of a mining pump motor. The local fire department was called and, at 11:15, the smoke was cleared, and a small fire was observed inside the pump and motor. The fire was fully extinguished at approximately 11:40 PM. This occurred in an unoccupied area of the mine.

Employee injured right hand and arm after making contact with a moving conveyor return roller.

Employee experienced back discomfort after moving a metal chute.

Employee chipped a tooth when using a water hose to clean Process Building - first floor.

Miner stumbled on a walkway and experienced left shoulder discomfort.

Death due to non-mining related natural causes.

Employee experienced right shoulder discomfort after removing a screen deck from a mineral separator.

Employee injured right shoulder replacing a filter belt.

Miner fell walking next to haul truck, and landed on right elbow and experienced right shoulder discomfort.

Employee injured back while dismounting a 772G road grader.

Miner experienced right knee discomfort as EE planted EE's right foot on the ground when EE was dismounting a John Deere 460e haul truck.

Miner was descending a ladder when EE slipped off of the rung causing EE to drop to the next rung before EE was able to regain EE's balance causing discomfort to EE's right knee.

Miner injured their right hand and received six sutures to the right thumb when a railcar collided with a platform gangway that was not mechanically secured in place.

Miner injured upper left arm when operating a haul truck and hitting a bump in the travel way.

While exiting a front end loader the employee's boot tread got caught on the bottom step of the ladder causing employee to twist awkwardly resulting in a sharp pain in left knee. Employee went to a doctor for an evaluation on 10/16/2018 and was put on restricted duty.

While pulling a water hose across the floor it became tangled causing it to jerk. The jerking motion resulted in pain to the employees right shoulder. The employee went to the doctor on 10/08/2018 at which time employee was given restrictions and referred for physical therapy.

Employee stood up after opening a rail car gate and felt a "pop" in knee. Employee initially refused medical treatment. Employee requested medical treatment on May 9th at which time surgery was recommended. On June 4th 2018 employee went off work for surgery.

While attempting to line up two pinch tube flanges the spud wrench that was being used slipped and struck the employee above the left eye. The resulting injury was a laceration that required stitches.

While cutting a piece of plastic with a hole saw the circular cutting bit jumped and struck the employee on the left wrist resulting in a laceration. The resulting laceration required stitches.

Employee tripped over sump frame beam and fell, employee broke femur.

While attempting to slide the counterweight of a Rotex machine into place, the weight shift landing on the associates hand. The resulting injuries were lacerations to the left hand that required stitches to close.

The employee lost footing when stepping down from one building to another. EE did not fall to the ground but landed awkwardly on left foot resulting in a sprain to the arch. EE did not seek medical treatment until January 13th, 2017 at which time EE was put on restricted duty.

A shift in hearing was reported during an annual audiometric examination. There have been no significant noise exposures according to our dosimetry records.

Employee was lifting up on an elevator safety gate with right arm. As EE lifted up the gate EE indicated that EE felt pain in right arm between forearm and bicep. X-ray and MRI done. Strain of arm ligament. Restricted duty-right arm and physical therapy. *Referred for surgery 2/9/17. Surgery took place on 2/17/17. Begin lost time on 2/17/17.

Employee was handling a sand equipment process screen when EE reported a back strain and pain. Employee went to doctor and was referred to the PCP. EE was unable to obtain an appointment with the PCP until 6/22/2016 and returned to full duty work on 6/23/2016.

Employee tripped while walking down the last couple of stair risers, when then the tyvek disposal pant leg cuff caught on a stair riser. Employee's left ring finger was fractured while the employee used left hand to try and break fall.

The employee was opening rail car lids on top of a railcar. The employee was wearing fall protection with a SRL anchored to an approved point on a manlift platform. The SRL strap got wrapped within the manlift railing while the employee was bending to open lid. SRL strap was tighten, employee hurt back.

While using a bobcat to clean sand near a truck scale, an employee injured their rib (closed fracture), when the bobcat struck a protruding concrete footing. The employee was wearing a seatbelt, however employee was forced forward and struck a safety bar in midsection. Medical treatment received 4/18/2016.

Employee was entering the wet process building. EE entered and parked the electric scoot. EE then shut the doors and when ee turned to go back to the cart, ee slipped on ice, but caught themselves before a complete fall. Felt shoulder pain and received treatment for a strain with restrictions.

EMPLOYEE WAS REPLACING A STUFFING BOX FOR A PUMP WHEN HE FELT A PAIN IN HIS BACK. THERE IS AN UNSPECIFIED HISTORY OF PERSONAL BACK ISSUES. NO MEDICAL TREATMENT SOUGHT UNTIL 11-18-2015.

While retrieving a sample ground silica from the process entered the employee's eyes. Safety Glasses and all other PPE was being worn. Eyes were flushed and no further injury noted. A prescription medication was given to prevent infection.

The employee was assisting in the task of lining up the reducer pipe on the #2 in line pump. The process involved 2 other employees on the ground and an employee in the front end loader which lifts the pipe in place. During the process the employee injured his right shoulder. The employee did not seek medical treatment until 8-14-2015 at which time it was deemed reportable

EMPLOYEE WAS WALKING AND PULLING FIRE HOSE TO PUT IT AWAY. EE SLIPPED ON THE WET FLOOR AND LANDED ON RIGHT SIDE STRIKING KNEE AND SHOULDER AGAINST THE CONCRETE AND STEEL GRATING. NO MEDICAL TREATMENT SOUGHT UNTIL 11-12-15.

Flange fell off the vac truck while emptying at site.

Employee was moving plastic bags of non perishable foods to a pallet to be delivered offsite to a charity event. When the employee bent over and came back up they felt a sharp pain in their back. Bags weighed less than 20lbs.

Employee reported that after he slightly bent over he came up and felt a pain in his lower back. He reported he did not lift anything and had pain in the past.

Employee moved the monitor station (using the affixed handle) from the lower to upper position, he injured his left elbow. The force observed to move the monitor was no more than normal to move the handle on the station. Poor positioning and ergonomics was identified as more significant risk factor during the investigation.

Employee was walking and went to make a turn and his knee gave out. The employee reported that no single event occurred to cause the incident, according to the employee other than walking. There is no work relation to the injury other than being on mine property.

Grinding a 2" pipe set vertically on end at waist height on a table in the D-Pit Maintenance Shop. While grinding the sparks were hitting his stomach and somehow made it into the area under the face shield and safety glasses. Employee did not notice until 6pm and did not report this until 6:45am on 6/9/13

Employee manning a tagline while a piece of pipe was removed and lifted out by a sky track boom lift. Pipe moved slightly and the rope slipped through the left hand and index finger causing a laceration. 15 stitches were administered.

Employee was attempting to open a gate on the bottom of the rail car. The gate was stuck from residual materials from external sources, in addition the whether was below the freezing mark. The EE attempted to put more than normal force on the bar that opens the gate, and when the gate opened the employee lunged forward and to the left injuring causing pain in his left shoulder.

While conducting a 2 person lift of an office desk, the employee experienced pain in his lower back.

The employee was injured while operating the Volvo A40F Haul truck. He was diagnosed with a shoulder strain. The employee reported the pain was an accumulation over a period of 3 to 4 weeks.

The employee was turning a chuck key with a T-Bar on a lathe in the machine shop. As the chuck and the chuck key moved downward his left hand ring finger was caught between the chuck key and the lathe table. The employee instinctively pulled his hand back, the chuck key caused a laceration which required 5 sutures.

After grinding on a piece of metal the employee noticed something in his eye. The employee was wearing the appropriate PPE and apparently received the metal contamination after wiping his brow following the work being completed.

The employee was lifting into place an upper idler roll on the facility dozer the employee did not seek assistance from 2 other staff within the department to complete the task or did not wait for the forklift to complete the lift instead of conducting the lift manually.

The employee was originally injured on 2-11-12 while descending a ladder in a fluid bed dryer the employee tripped on the bottom rung and fell 12 to 18 inches backward. The original diagnosis was bruised ribs, 1 week later the employee, due to continued pain, went to the doctor again and was diagnosis with non-displaced fractured ribs 3,4,5 which then made this injury reportable.

Employee was cleaning a railcar when the car cleaner bar popped out of the hub and hit his right knee on the inside. No medical treatment was sought at the time of injury. On 12/21/11 the employee asked to seek medical treatment and received it. The employee was given restrictions and a required follow up appointment. This was reportable as of 12/21/11.

Injured employee was assisting pipefitter crew during pipe installation. Pipe was in a bind and employee attempted to push it free. Pipe released from bind pinching fifth digit on left hand between pipe and angle iron resulting in laceration and fracture. Employee received medical examination, sutures and finger splint.

The employee was looking up while inspecting a motor on a portable crusher machine. A small rock fell from the crusher and struck him in the top left side of the head, prior to the injury the employee was looking up at the motor and his hard hat slipped backward exposing his head allowing the rock to hit me.

Employee was loosening packing gland when wrench slipped off nut and cut right pinky finger on a piece of steel. Employee taken to doctor and received stitches. Did not miss any work.

Employee was carrying heavy tools and a chain hoist. When he stepped up on the pump platform, his left foot hooked the pull-eye. This caused employee to lose balance and twist, driving his left knee into the pump frame. Was driven to the doctor and then came back to work. No kneeling for two weeks. Accident was 6/1/11 but employee did not go to the doctor until 6/3/11.

EE was operating a HDPE fusion machine fusing two 16" HDPE pipes. While loading the machine, the section of pipe jammed on the saddle not allowing the pipe to feed properly. The ee attempted to reposition the pipe by hand leaving his left hand exposed in a pinch point. The pipe came loose and went forward pinching his #2 and #3 digits on his left hand.

EE was reposition a structural steel member so a forklift could move the material. EE stated that he thought he could push the member onto the forks. The structural member was approximately 300lbs and when the ee pushed, he felt a pop in his neck. He returned to work where he continued for several additional minutes before his neck began to hurt. He then told site management.

EE was performing routine clean up when he fell and struck his leg against a piece of metal grating. The ensuing injury was a cut to his left leg requiring six stitches to close.

While working in a flange overhead a piece of foreign material struck his eye. The object was removed by a physician and drops were prescribed. The injury wasn't reported until Monday 05/03/2010 at 1315 at which time the material was removed.

Employee was struck in the eye by debris while working either on a door in the shop or while working on packing for a pump, both jobs were on separate days, the employee is not sure when it happened exactly. Employee reported incident to his supervisor Sunday February 28th and stated that he wasn't sure if it happened Friday February 26th or Saturday February 27th.

Employee was attempting to remove a loose flap covering a splice on a filter belt with a utility knife, when his hand slipped and came in contact with the blade, resulting in a minor cut on his hand that required three stitches.

While connecting two sections of 16" plastic pipe together in the pit the pipe slipped off 6x6" wooden blocks on which it was resting and landed on the employees left shin. The weight of the pipe caused both his fibula and tibia to fracture.

While climbing into the trackmobile the employee's hand slipped causing him to have a pain in his back. The employee reported the accident to the company on 9/2/2009.

While closing the bottom hopper gate on a rail road car the employee didn't notice a piece of metal sticking out next to the latch and struck his finger. The resulting cut required 3 stitches.

While walking to his monitor in the pit he slipped and twisted his knee. The incident just became a reportable/lost time yesterday (5/4/2009) when it was found out that his injury requires surgery.

While grinding in the shop area, the employee got a sliver of metal in his eye that needed to be removed by a doctor.

While using a walk behind forklift, the employee was performing a turn when she felt a pain in her shoulder, initially refused medical treatment. Recently her shoulder was bothering her, so she sought medical treatment and was told she needed physical therapy and there is a possibility of surgery at some point.

While driving out of the pit and going up a hill the vehicle started spinning tires on the ice so the employee got out of the truck to see if the 4-wheel drive was engaged. When the employee exited the vehicle he slipped and fell on the ice.

Slipped on bottom step of short stairway while walking out of maintenance shop to start work truck. The slip resulted in the employee falling and striking his head the resulting cut required nine staples to close.

The employee knelt down to pick up a piece of metal, when he stood back up, a lump on his knee appeared. The employee went to the doctor, where blood was drained from the lump. Incident was not reported until 11/26/2008.

While working to bolt a sandline together the chain holding the pipe off the ground came loose causing the pipe to come loose and hit the miner in the chest. The force of the pipe hitting him in the chest knocked him on the ground causing him to strike his head on a piece of metal used to hold conduit off the ground.

While lining up an adaptor piece on a valve the ring adapter slide down and struck his right thumb resulting in two stitches and a small bone fracture.

While opening a RR car gate the bar slipped smashing his finger against car hopper.

Maintenance man was grinding overhead.

Employee was breaking RR cars apart by pulling on lock pin when his finger was pinched requiring 5 stitches to close cut.

Slipped on ice and twisted knee

Employee slipped on an icy surface and fell, causing a hip contusion.

While rebuilding a rotating assembly, employee's finger was pinched between the shaft and the bearing press, resulting in a cut that required five stitches.

Employee was dilling a hole in sheet metal when the drill bit caught, causing the drill to turn. The turning drill caused his finger to be smashed, resulting in a hairline fracture to his right ring finger.

IE was working on a pick up truck when he got a small metal sliver in his left index finger. He removed the sliver and continued to work. He developed granuloma at the spot where the sliver was removed. He went to the doctor a few days later and the doctor said that surgery was needed to fix the problem. He had surgery and began missing work on 09/27/2007.

Employee was checking railcar supply in the rail yard. He started to climb up onto the brake platform of the car. He received a laceration on the tip of his middle finger on his left hand from a sharp edge in the back side of the ladder rung. The laceration required stitches.

As employee broke for lunch he started to feel pain slowly come on in his neck and left arm. The employee has a serious personal illness and has not been to work since 7/18/2007. The company received paperwork from the employee's doctor that said he is off of work for this injury not his personal illness on 8/2/2007.

The employee was installing a new MCC panel. He and his work partners were lifting the panel into place when it slipped and struck him just above his hair line. He was taken to the ER where he received several stitches to close the cut.

EE was walking around a coiled water hose and tripped over it. He then fell to the floor landing on his left hip, resulting in a ruptured disc in his lower back. This injury required surgery on 3/9/07.

The IE said on 3/6/2007 that he believed that he hurt his shoulder on 3/1/2007, but was not sure. He said he felt a pain in his shoulder when closing the outer doors of an elevator. Employee has started physical therapy which caused this injury to become a medical.

IE was cutting plastic seals on railcar lids when he cut the tip of his right middle finger. He received three stitches.

The IE was holding a door open when he felt a pain in his right hand. He was diagnosed by his doctor to have arthritis. We received new information in the past week that has changed this injury from a FA to a Med. The IE had 8 occupational therapy visits after the incident.

Employee was getting onto bagging machine when a foot rail broke. He fell and caught himself on the bagging machine platform, twisting his back.

While removing a vacuum pump the employee pinched his r. wrist and hand between a pump shaft and a cement wall. He didn't miss any work days and didn't require extended medical care. The injury was reported as a first aid. He returned to the doctor two months ago due to irritation in his wrist. The employee had surgery on his right wrist Feb. 2nd and returned to work Feb. 6th.

HIGH WINDS ON TRACK CAUSING AIRBORNE MATERIAL (SAND). EE'S EYE VERY SCRATCHY. EE SAW DOCTOR AND RECEIVED DROPS.

EE WAS OPENING LID ON RAILCAR - HURT LOWER BACK INTO LEFT LEG. EE WAS BENT OVER THE LID AND THE LID WAS HALFWAY OPEN. TREATED AS FIRST AID UNTIL 12/13/04.

Ee stepped in a hole between tracks 4 and 5 south end, twisting his right knee.

Employee was over valve, starting to open the 10' valve when victalic coupling blew apart due to the mainline being under presser 200 psi the force of the uncoupled pipe hit employee on right shoulder and ribs, knocking him to the ground.

EE WAS STEPPING DOWN FROM A PLATFORM WHEN HE TWISTED HIS LEFT LEG/KNEE. FURTHER DIAGNOSIS REVEALED TORN LIGAMENTS AND EE UNDERWENT SURGERY 7/7/04.

EMPLOYEE WAS LOWERING AN EIGHT INCH PIPE DOWN A LADDER WHEN THE PIPE SLIPPED MASHING HIS FINGER BETWEEN THE PIPE AND A FEED BOX.

EE WAS WALKING ALONGSIDE THE RAILROAD TRACK TO THE NEXT CAR WHEN HE STUMBLED IN THE SAND. EE TRIED TO STOP HIS FALL BY STRETCHING OUT HIS ARMS.WHEN HE LANDED HE FRACTURED HIS WRIST.

EE HIT HIS WRIST ON AN ANGLE IRON BRACE WHILE CLEANING UNDER GKC'S. HE RECEIVED SURGERY ON HIS WRIST ON 11/19/03 AS A RESULT OF THIS INJURY. HE LOST WORK DUE TO THE SURGERY.

EE WAS USING A CUTTING TORCH TO CUT OFF BOLTS OF A CONNECTING CLAMP ON A 10" WATER LINE APPROX. 40' LONG. THE BOLTS BROKE LOOSE ALLOWING THE PIPE TO DROP ABOUT 2', STRIKING HIS LEFT SHOULDER & ANKLE RESULTING IN A FRACTURE TO HIS ANKLE.

EE WAS UNROLLING A FIRE HOSE SECTION AND GRABBED IT ABOUT A FOOT FROM THE END TO TOSS IT OUT OF THE WAY. WHEN HE DID THE END OF THE HOSE FLIPPED BACK AND HIT HIS CHIN, CUTTING IT. (ONE SUTURE)

AFTER SPOTTING TO POSITION A RAILCAR, THE EMPLOYEE WAS WALKING BACK TO THE PICKUP TRUCK. AS HE STEPPED OVER THE RAIL TRACK HIS TOE CAUGHT ON THE RAIL AND HE FELL FACE FIRST INTO THE REAR WHEEL WELL OF THE PICKUP. UNABLE TO CATCH HIMSELF HIS ARM HIT THE WHEEL WHELL AND HIS FACE HIT THE HUB OF THE TIRE.

WHILE CUTTING AN AIR SIZER HOSE WITH A UTILITY KNIFE, THE KNIFE SLIPPED AND CUT THE BACK OF LEFT HAND.

EMPLOYEE'S HEEL CAUGHT ON SECOND FROM BOTTOM STAIR CAUSING HIM TO FALL. HE STRUCK HIS KNEE ON THE METAL GRATING OF THE WALKWAY. HE HAD 7 SUTURES TO HIS KNEE.

EMPLOYEE WAS CARRYING A PIECE OF GRATING (METAL) WITH ANOTHER EMPLOYEE WHEN HIS FOOT SLIPPED BETWEEN TWO OTHER SECTIONS OF GRATING. THE EMPLOYEE SLIPPED TWISTING HIS BACK. THE CONDITION OF THE EMPLOYEES BACK CONTINUED TO CAUSE HIM PAIN, SO HE HAD BACK SURGERY ON FEBRUARY 18, 2003. FEBRUARY 18TH WAS THE FIRST DAY THE EMPLOYEE LOST ANY TIME FOR THIS ACCIDENT.

WHILE CLEANING A RAILCAR, HE MASHED HIS FINGER BETWEEN THE VIBRATOR BRACKET & THE CLEANING BAR.

EE WAS OPENING HATCH ON TOP OF A RAILCAR AND FELT PAIN IN HIS LOWER BACK. (THE INCIDENT DID NOT BECOME REPORTABLE UNTIL 2/23/00 WHEN HE STARTED LOSING TIME FROM WORK.)
