Employee was standing upright from a stooped position and felt pain in his back.
Employee worked overtime & ate lunch provided by Noranda. Began feeling ill and thought it was a stomach virus. Upon returning home from work, the symptoms worsened and he was admitted to a hospital. He was diagnosed with colitis. The physician stated that whatever he had eaten 8 hours prior to his symptoms starting is what made him ill.
Employee reported to work on 8/29/09 with complaint of a bulge on the side of his abdomen. Employee was sent to his personal physician for diagnosis & treatment. As of this date, employee is alleging the bulge is related to his work activities on 8/28/09. There is no confirmed work relationship at this time.
EE and another EE were changing out 8" Clarkson valve on #3 header south. As EE was working, he bent down & when he stood up, felt a pain in his right knee. EE will be on restricted duty pending a medical evaluation/diagnostic tests. Will see an orthopedist.
NOT WORK RELATED: EE was examined by medical on 9/24 as follow up to his complaint of back pain. EE did not have a specific work event or incident which triggered this back pain; EE reported taking over the counter pain medication prior to the start of his shift which indicates that this pain was not work related.
Employee stated that after returning home from his shift on Sunday evening 8/31/08 he noticed a caustic burn to the back of his leg (ankle area). He did not report it until 9/5/08. GA personnel transported him to an offsite medical facility for treatment.
A section 4 employee alleges that an injury occurred to his right shoulder as a result of a minor altercation with his supervisor. An investigation is continuing, but at this time the company has no definitive evidence that an injury occurred on mine property. The employee is currently being evaluated for a shoulder strain and has not returned to work.
Employee squatted down to retrieve an item from a bottom shelf. Upon rising, experienced severe pain in knee.
While walking to turnstile, employee felt a F/O blow in his right eye.
Not Work Related: Employee had been performing normal shop work and went to get some water and began to feel bad. As he was returning to his work area, he became light headed and dizzy and was unable to stand or talk. He was taken to medical and was checked by the nurse where his condition improved.Was transported to hospital by ambulance for further testing and evaluation.
Upon arriving in the plant at approximately 6:30 A.M., and prior to beginning work, employee reported to safety with complaints of chest pains. He was transported to St. James Hospital where further testing and medical attention were provided. He was then transported to O.L.O.L. in Baton Rouge for additional care.
Employee experienced discomfort which he attributed to indigestion. Symptoms persisted until 3:00p.m. when this was reported to Medical. Employee was transported to local hospital where he was diagnosed with a heart attack. Employee was not involved in any strenuous activity. Condition is believed to be related to personal factors including a strong family history.
