Senior Employee who wears a pacemaker experienced shortness of breath and felt heartbeat was abnormal

Workers were finishing up crusher inspections in plant when a worker was stung one to two times in upper right shoulder by hornet. Other workers in the area observed the sting. Worker stated EE was not allergic (that EE knew of) but after 10 minutes EE experienced anaphylaxis.

Helping load a shot, sitting in the truck operating the controls, started to feel nauseous. (Diagnosed with vertigo)

**Medical Stroke** The miner became dizzy and weak in the morning of his shift. It was decided by the mine superintendent to transport the miner to the local hospital. The miner was transported via the superintendent. While in the ER the miner suffered a stroke. THIS IS UNDER PROTEST AS A MEDICAL INJURY NOT RELATED TO MINING ACTIVITIES.

HE TOLD SUPERINTENDANT THAT HIS BACK WAS BOTHERING HIM ON 6-3-97.

INJURED WAS DRIVING EUCLID TO SHOP WHEN BEE GOT IN CAB AND STUNG HIM ON THE RIGHT TEMPLE.PROCEEDED FORWARD RAN INTO REAR RIGHT SIDE OF AIR COMPRESSOR.HAD AN ALLERGIC REACTION TO BEE STING.REQU IRED MEDICAL TREATMENT.

EMPLOYEE CALLED INTO OFFICE THURSDAY AUG 18 1983 AT APPROX 0700AM AND ALLEDGED THAT HE HAD HURT BACK AND ALSO STATED THAT HE DID NOT KNOW HOW HE WAS ABSENT FROM WORK 8/18,19,22,23 WHEN HE RET URNED ON AUG 24TH HE HAD DOCTOR STATEMENTS AND ALLEDGED HE HAD INJURED BACK ON JOB
