Employee was at work for 4hrs and 20mins when they started feeling a sensation on their left hand and side. They tried to gather themselves and figure out what was happening. Coworker called ambulance to be safe. Paramedics evaluated employee and didn't find any cause. The emergency room didn't find the cause of the pain. Employee was sent home and given a few days to rest.

Mostrando los 6Employee walked out of break room. EE stumbled for unknown reasons and fell. A few seconds later EE started having convulsions. This is when EE struck head on the ground causing EE to bleed. EMS was called and available within 7minutes. The employee was taken to the hospital and received treatment.
Employee came in to start shift. Was seen acting disoriented and 911 was called. Employee was taken to the hospital.

Shortly after starting shift, employee called the scale house operator complaining about chest pains. Scale house operator called 911 and MSHA. Employee was diagnosed with costochondritis, basically sore ribs - usually from stress.
This was not an accident/injury. The employee collapsed while leaning on bagging machine table. Fellow employee found ee on the floor and called 911 and administered CPR. Employee later died at the hospital (see Death Certificate #9 - place of death).
