Mostrando los 17Employee was working indoors in the Chilton Manufacturing Center. EE was stretching during the morning required stretching. EE was bending forward stretching towards EE's toes and felt a pain in EE's lower right back.
Employee was doing stretches. He twisted and felt a pain in his back. Employee did not seek treatment until 01/17/2011. Restricted duty assigned.

Employee felt a twinge of discomfort in his back at work on 8/24/06. On 8/28/06 he had severe back pain and left work. His back had also hurt over the weekend while doing housework. Employee was unsure whether injury was work related. Doctor was also unsure. On 9/6/06 doctor determined injury to be work related. Employee did not report an injury until 8/28/06.
Employee's right wrist has been hurting for the last month so he went to the doctor. Tendonitus of the right wrist. Restricted duty.
Employee doesn't know what happened. He had no pain during work or when he left work. He started feeling pain about 7 pm at home.
EMPLOYEE WAS IN THE OFFICE HORSEPLAYING WITH ANOTHER EMPLOYEE. THE OTHER EE KICKED HIM IN THE KNEE. EMPLOYEE DID NOT SEEK MEDICAL TREATMENT UNTIL 11/15/05. DIAGNOSIS: KNEE CONTUSION, MEDICINE PRESCRIBED.
EMPLOYEE AWOKE MORNING OF 11/10/03 W/PAIN IN CHEST AREA. FIRST DOCTOR RULED THAT IT WAS NOT WORK RELATED. LATER A SECOND DOCTOR DECIDED THAT IT COULD BE WORK RELATED. SHE HAS FRACTURES IN THE 6TH AND 8TH RIBS.
EE WAS SHOVELING STONE DEBRIS INTO TUBS, THE WIND BLEW THE DEBRIS BACK IN HIS FACE. DIAGNOSIS. METAL FOREIGN BODY IN LEFT CORNEA. EE SAW DR ON 12-4-02, WAS ABLE TO RETURN TO WORK ON 12-5-02 BU T TOOK A VACATION DAY & RETURNED ON 12-6-02.
EMPLOYEE WAS IN THE MUD ROOM. HE HAD SWATTED AT A BEE. IT THEN STUNG HIM IN THE RIGHT HAND.
EE WAS DRIVING BOBCAT ON QUARRY WHEN SHE WAS BITTEN FROM A BUG, SHE NEVER HAD A REACTIN TO A BUG BITE BEFORE.
EE HAS A HISTORY OF MEDICAL PROBLEMS. HE WAS EXPERIENCING NUMBNESS OF THE LEFT SIDE OF HIS FACE, AND A SLIGHT LOSS OF STRENGTH. EMERGENCY PERSONNEL WAS CALLED.
EE WAS BENDING OVER AND MOVING, HIS KNEE POPED AND STARTED TO HURT. RESULT A KNEE SPRAIN. EE DID MENTION HE INJURED IT SLIGHTLY OVER THE WEEKEND.
