EE saw a loose piece of stone stack placed on the flatbed trailer. The EE tried to dislodge the broken piece of stone so it would not fall off onto the roadway during transport. Upon loosening the stone, the piece separated quickly, falling and pinching left pinky finger between stone and flatbed surface. The EE had gloves, but retracted, causing a laceration.

Employee was operating Splitter #5. There was a piece of stone stuck in the machine and belt. Employee went to remove the stone piece, and EE's finger on EE's right hand was pinched, cut.

While loading material onto the conveyor of TXS 3 in the NTV Flats dept, the stone tipped over, pinching EE's finger between the stone and the conveyor.

Employee in the NTV area was moving a stone off a conveyor belt when it fell on EE's foot.

While trying to move a stone that was jammed on the feed conveyor of splitter #9 inside the CMF, the employee's hand slipped on some ice that was on the stone, and the employee's left middle finger was pinched between that stone and another stone that was on the conveyor. The finger was bruised, numb, and swollen after making contact between the two stones.

Employee was working inside of the CMF building on splitter machine #7. While removing a stone from the splitter, the scrap guard between the discharge conveyor and the bottom row of teeth popped up, then came down on finger, pinching it between the scrap guard and bottom row of teeth. The left index finger was cut and fractured.

Employee was working in the CMF on the splitter nearest the office. Was helping the operator move a rock standing on the machine leg. The employee's foot slipped and went into a slit. The employees right foot then got stuck between two pieces of iron when a waste tub was slid into place by the material handler.

Employee reporting pain/injury in upper left shoulder from repetitive use over time operating forklift steering wheel in the CMF.

I lifted a stone from the conveyor belt that weighed about 100 pounds to place it on the pallet, and I immediately felt a crack in my back. I told the material handler about the accident & asked the material handler to let the supervisor know. I continued working slowly and after I took my lunch break and went back to the work area, the pain in my back was worse.

I took a stone from the splitter table that was 10" to change its position, and when I held it in my hands, the thumb of my right-hand bent.

The injured employee was cutting rocks and they smashed their left ring finger with a rock. IMPACT/BLUNT FORCE INJURIES.

Rock rolled down the Sherpa onto the splitter table; employee was not able to move fast enough to get hand out of the way.

I was picking up and moving a stone piece when it slipped out of hand and slid down my arm. It cut my arm.

EE had split a piece of stone in half, slid the top half back off of the bottom half, and pinched their finger in the process.

When moving a stone from one pallet to the finished pallet for packaging with a suction cup tool employee reached down to move some packing material and at the same time the stone fell from the suction cup onto EE's hand

Employee was taking a stone from the pallet to cut it on the machine and another stone from the pallet fell on their right thumb resulting in a nondisplaced fracture of distal phalanx of the right thumb.

EE had to crouch down to pick up multiple rocks that fell over, causing pain to EE's left lower back.

Employee was getting out of the cab of the JCB and missed the step. EE didn't grab the hand rail first and then fell backwards landing on EE's lower back and butt.

Improper hand/finger placement.

EE was working on machine #2 and a rock piece stuck under the lower belt guard and the belt that carries the split stone away. There is a metal plate that acts as a belt guard which protects the belt from having rocks get under it. It appears as the belt was running/advancing the rock piece was dislodged and the guard came down against the belt which pinched EE's finger.

Employee was stacking rock near a drain, employee tripped on slope and twisted foot.

Another employee threw a snowball in the production facility and struck employee in the ear.

Employee was work on splitter numbers 8 and 7 when EE picked a rock off the line and EE was opening a seam on the rock and a piece fell off and struck EE's right foot.

Employee works inside our Chilton Manufacturing Building in the Splitter Department. Employee operates the splitter and moves stone before it is split open.

EE is a Splitter Operator in our Chilton Manufacturing Facility at our Chilton Quarry. EE was leaving the shift early at 5:00pm as EE was feeling ill. As EE was walking out to the car in the parking lot, EE slipped landing on right side. A salt/sand mixture and salt is used regularly in winter to limit slippery spots in our parking lot and employee walkways.

Employee is a Quarry Laborer at the Sabel Quarry. As they were helping another employee, who was driving at skidsteer at the time, place the stone on the pallet, their right ring finger was pinched between that stone and another stone already on the pallet.

Employee is a Splitter Operator at our indoor Chilton Manufacturing Facility. Employee was handling two stones when EE pinched EE's left pinky/5th finger between the stones. The pinch caused an laceration to EE's left pinky/5th finger.

Employee is a Splitter Operator at our indoor Chilton Manufacturing Facility. Employee was making a cut of the stone. When the cut of the stone was complete, the stone bounced back causing a finger to be caught between the two stones as the left hand was up on the rollers.

Employee is a Splitter Operator at the Chilton indoor within the Chilton Manufacturing Facility. Employee pulled a stone back on the rollers in order to reposition it for further splitting. In this process, EE pinched EE's finger between that particular stone and a stone that was behind EE on the rollers resulting a laceration and abrasion to EE's left ring finger.

Employee is a Saw Operator at the Chilton Quarry indoor within the Chilton Manufacturing Facility. Employee lifted a thirty pound stone from the Saw. In the process of setting it on the table next to EE, EE caught EE's finger between the rock and the corner of table resulting in a laceration and break to the right middle finger.

EE is a Splitter Operator at the Chilton Quarry indoor within the Chilton Manufacturing Facility. EE was assembling boxes. EE put a nail in one section followed with a second nail. The nail gun then jumped and landed on top of EE's left hand. When the gun hit EE's hand, a nail fired into the top of EE's left hand as EE's finger was still on the trigger.

Employee is a Splitter Operator at the Chilton Quarry indoor within the Chilton Manufacturing Facility. Employee was pulling a stone towards self when the stone slipped causing employee to punch finger between two stones. The pinch resulted in a laceration to left pinky finger.

Employee works at the Oakfield Quarry as a loader operator. While banding a pallet in the quarry, the employee cut the right thumb and index finger. The banding did not spool as freely as the employee was expecting and when they pulled harder on the banding, it slipped through their hand cutting it.

Employee was working indoors in the Chilton Manufacturing Center. EE was stretching during the morning required stretching. EE was bending forward stretching towards EE's toes and felt a pain in EE's lower right back.

Employee works indoors in the Chilton Manufacturing Center as a Splitter Operator. The employee was operating a splitter and a piece of rock fell off the table and struck foot. The employee was wearing steel toed boots with metatarsal protections.

Employee is a material handler working indoors in the Chilton Manufacturing Center. While putting a stone on a pallet, EE twisted and felt a pain in EE's lower back.

While working indoors in the Chilton Manufacturing Center, the employee was positioning stone and pinched finger in between two stones. The pinching in between the stones resulted in a right ring finger laceration.

While working in the Chilton Manufacturing Center (indoors), an employee on a forklift moved material, causing another employee's foot to become wedged in between a splitter and a bin of stone.

Employee was working indoors in the Chilton Manufacturing Center as a material handler. While lifting a stone off of the floor, finger struck another stone and was lacerated.

Employee works outdoors in an above ground quarry operating a loader. While banding a water pump hose, the banding cut EE's hand. Employee sought treatment for the laceration.

Employee works indoors in the Chilton Manufacturing Facility where ee packages stone weighing 15-20 lbs on average. While putting stone in a box, ee reached for a stone and right wrist was struck by a stone that was traveling down a conveyor.

Employee works indoors in the Chilton Manufacturing Facility as a Splitter Operator. While pushing a large piece of stone into the splitter, the stone got stuck and the employee pushed hard against the stone. The employee noticed pain in left wrist following these actions.

Employee was working indoors in the Chilton Manufacturing Facility in the Natural Thin Veneer Department making boxes. While assembling a box ee punctured finger on a nail hanging out.

Employee works indoors in the Saw Shop. Employee was taking a piece of stone from a table when the end broke off. The employee stated, "juggled the stone and my left ring finger got stuck between the stone and table." The employee went into see the doctor and the doctor diagnosed injury as crush injury to the right ring finger.

Employee works as a Material Handler in shipping and receiving. While ee was grabbing a box from a pallet, feet slipped forward on the ice. EE said ee, "jarred neck." Employee did seek medical treatment and the doctor diagnosed it has a neck strain.

Employee had pulled into the quarry and parked semi in yard to hook up flatbed trailer. Fell of top step of truck & landed on gravel lot. Broke left wrist, right thumb & needed stitches in forehead.

Employee was working indoors on a NTV Corner Machine cutting stone. Employee loaded stone and had his hands place incorrectly and ended up lacerating is right index finger. Employee ended up getting sutures.

Employee was operating a stone splitter and while pulling a stone towards him the stone hit the right side of his torso. The stone was roughly 6' by 5' wide.

Employee is working indoors in the splitting department. While putting a stone on a pallet, the employee pinched his right 1st distal tuft in between 2 stones.

Employee was moving a piece of flagstone and while moving the stone, he felt a pain on his right abdomen area which occurred on 10/26/2015. The pain did not go away after a feel days, employee sought treatment on 11/06/2015, where he was given work restrictions.
