Employee was removing equipment extension to facilitate welding repairs when he propped his left leg against truck and felt pain in hamstring area.
Employee had irritation of the eye on 04/25/2015 and sought medical treatment on 04/25/2015 leading to a foreign metal sliver removed from surface of eye. Employee is unaware of when or how the injury occurred.
The miner stood up after squatting to look at a pipe and felt pain in right knee.
On 5-29-13 EE returned a Disability form to the Co in which his Dr stated that his hand condition was work related. Co had no reason to suspect his condition was work related until the Dr's form was returned. He has work restrictions that the Co cannot accommodate and is missing work. Date of inj unknown. Investigation is ongoing as the Co's W/C carrier has denied this claim.
Employee contacted Personal/Payroll Admin on 2/1/12 requesting information concerning short term disability due to his back hurting. The employee stated that the injury was not work related at the time. The work restrictions placed on him by the treating physician of his choice, we are unable to meet. We were informed on 2/17/12 that a workers comp claim had been filed.
Employee was walking through the Hydrate Baghouse at the end of his shift and was stung by a bee near his right eye.
Employee was at the back of the preheater taking measurements. Lime dust blew into his right eye. Did not see Dr until 8/16/2005. NO LOST TIME, HOWEVER HE GOT PRESCRIPTION EYE MEDICATION.
Employee was cleaning with shovel & bobcat and thinks he got bit by a spider. He asked to see a Dr. on 6/24/2005 when he reported the incident. NO LOST TIME, NO WORK RESTRICTIONS AND NO MEDICAL TREATMENT OTHER THAN A RX FOR ANTIBIOTICS.
