Employee notified supervisor at 6:00 am on 3/25/2024 that they were not going into work due to debris in the employee's in right eye. Employee was unsure of where and when the debris entered eye. Last time operating a grinder was on 3/21/2024.

While kneeling on right knee, went to stand-up. Left knee popped causing pain to outer knee area.

EE was walking from shop building to his pick-up truck, when a gust of wind blew something into his right eye.
Employee had been shoveling; states he first experienced pain at work but is uncertain of the exact time or date. Employee cannot remember an event with the potential to cause an injury. The employee's first report of this to the employer was on 9/11/07.
EMPLOYEE STATES THAT WHILE OPERATING A 50 TON CRANE A GUST OF WIND BLEW DUST INTO HIS EYES CAUSING THE EYE IRRITATION.
EMPLOYEE WAS INSIDE CEMENT COOLER HELPING TO CLEAN IT. HE BECAME OVERHEATED & WAS AFRAID HE WOULD FAINT. IN PANIC HE TWISTED HIS BODY COMING OUT OF A RESTRICTED OPENING. WHEN HE TWISTED HIS BO DY HE TORE LOOSE SOME SUTURES FROM A PREVIOUS HERNIA OPERATION. THIS WAS NOT DISCOVERED UNTIL SOME TIME LATER.
EMPLOYEE WAS OP9RATING A EUCLID BOTTOM DUMP. HE PULLED UP TO DRAG-LINE TO WAIT HIS TURN TO BE LOADED. EMPLOYEE OPENED DOOR TO EUCLID AND WAS IN THE PROCESS OF CRAWLING OUT TO STAND ON FENDER O F EUCLID. A SUD8EN GUST OF WIND BLEW THE EUCLID DOOR SHUT AND CAUGHT THE EMPLOYEE'S FINGER IN DOOR FRAME.
EMP WAS MAKING TRANSFORMER CONNECTIONS ON TOP OF ELECTROSTATIC PRECIPITATOR CONNECTIONS WERE RUSTY PIECE OF RUST WAS CAUGHT BY WIND AND BLEW UNDER SAFETY GLASSES EMPLOYEE RUBBED EYE AND THOUGH T HE GOT PIECE OUT. EYE STARTED BOTHERING HIM LATE THAT NIGHT
EE WAS WALKING FORM SHOP TO THE OFFICE WHEN HE TWISTED HIS LEFT KNEE.
