Employee was guiding an equipment operator relocating the weight and guy wire for a telephone pole. The pole cracked and bees flew out stinging the employee multiple times.

Injured miner received bee stings when a nearby dozer ran over a box containing a bee hive. The hive swarmed around the injured miner and stung EE on head and neck. EE was treated at the company clinic and released to full duty.

Employee complained of angina after shoveling and believed he was experiencing a heart attack. He was taken for med treatment & admitted to hospital. Tests indicated he did not suffer a heart attack. It was described as an "episode" related to childhood heart surgery. Employee related that he has these episodes periodically, not work related. MSHA was notified as a precaution.

While the employee was grinding slag off a piece of angle iron a gust of wind blew a small piece of grinding under the employee's face shield and safety glasses into his upper eye lid.

The employee felt/heard a popping sound in his right knee after rising from a sitting position from working on the brakes of a water truck.

While walking the water conveyor line, the employee's knee felt like it slipped out of joint.

Employee received a spider bite while driving a haul truck.

Employee developed an allergic reaction to a bee sting he suffered while exiting the office.

EE STUNG BY A BEE.

THE EE STOPPED HIS DOZER. NEVER GOT OUT OF EQUIPMENT. NAOTHER EE SAW HIM SLUMPED OVER STEERING WHEEL. CALLED FOR HELP. 2 EES GAVE PRY UNTIK PARAMEDICS ARRIVED. WAS PRONOUNCED DEAD AT TRI CITY HOSPITAL AT 200 PM 12-11-95. NO ACCIDENT NATURALCAUSES.

EMPLOYEE WAS BENDING OVER WHEN HIS KNEE POPPED. EMPLOYEE HAS HAD AN ON GOING PROBLEM WITH HIS LEFT KNEE.
