A supervisor was working on writing an employee review in the office. As another individual walked past the office, they heard a loud noise and went into the office to see what it was. They found the supervisor lying on the floor actively having a seizure. Sometime during the event the left ear and left head suffered lacerations that required stitches.
Particle blew into left eye, behind safety goggles, while walking around machine. Reported to employer on 6/4/11.

HIS FIRST DAY MISSED WAS MARCH 11, 1997. WE WERE FIRST NOTIFIED OF HIS BELIEF THAT THIS IS WORK RELATED ON 3-27-97. WE WILL CONTEST THIS WITH WORK COMP. & NOTIFIY YOU OF THEIR DETERMINATION.
