The employee was cleaning pond over flow drain. The pipe inlet has an expanded metal barrier to catch large debris from entering the inlet. The employee was reaching down to remove large debris from barrier and was bit on EE's right index finger by a venomous snake hiding under the debris. The worker was wearing rubber style gloves when bit.

Mostrando los 13Employee was unloading cam frame bolts (approx. 45 lbs. each) from a plastic box supplied by the mine. He stepped into the box to reach the bolts and noticed a bite/sting on his leg. He keep working. Later the bite site became painful, swollen and abscessed. He reported this to his boss 9/3/14 and was instructed to go the occupational medial clinic 9/4/14.
At approximately 2:00 am, a Kress coal hauler was observed off the road in an MSHA pond. Upon investigating the truck was found to be stuck in the mud and the operator was unresponsive. The operator was later pronounced deceased by the Justice of the Peace.
Employee was sitting on a trailer and felt something sting him, but did not see what it was. The bite got infected and had to go to hospital.
Employee died of natural causes - myocardial infarction (Heart Attack). See MSHA's correspondence: CMH&H Memo No. HQ-06-112-A (Sec 103), Memorandum for Allyn C. Davis. Classified Degree 9.
EE WAS CUTTING MATTING USING A MACHETE, WHEN THE WIND CAUGHT THE MATERIAL AND PULLED HIS HAND INTO THE PATCH OF THE MACHETE, CUTTING HIS LEFT INDEX FINGER REQUIRING STITCHES.

EMPLOYEE GRADING HAULROAD AND WAS STUNG BY A RED WASP ON LOWER LEG RESULTING IN AN ALLERGIC REACTION.
