EE was working in the beltline and stated that EE was not feeling good, after sitting down EE collapsed EE's Co-workers checked EE and found no pulse at which time they began CPR, an AED was retrieved. EE was brought outside with CPR in progress. Around 3:50 AM EE was transferred to Sevier County EMS and taken to the Medical Examiner.

Mostrando los 7Employee suffered a stroke while walking in the beltline. They later suffered a severe stroke at the hospital. Mine investigation determined it was a personal medical incident.
Employee was standing by foreman. Employee fell over forward and died. Miners tried CPR and Defibrillator but could not revive them. We believe the autopsy will reveal a heart attack. This fatal is not mine related.
Crew pulled belt out of LW belt bank. They were in process of pinning the belt back together. EE got under elevated belt on knees to help line up splice. Once they were done, EE scooted out from under belt and when they went to stand up, they strained their back. EE did not return to work on the next scheduled shift.
Witnesses stated employee was on a ladder grinding. EE came down to the platform and stated they felt ill. EE sat down and then laid down. Witness went to summon assistance. Sufco EMS transported EE to the Richfield area and then met with Sevier County EMS. Then Sevier County EMS took EE to the Hospital, Richfield, Utah.
EE was preparing to go home after shift ended, when he experienced chest pain. He was transported to Sevier Valley Hospital by ambulance, where treatment continued. Life flight helicopter transported EE to Utah Valley Hospital. EE passed away either in helicoper or at hospital, due to heart condition. EE's shift started at 2:30pm on 12/12/06.
EE HAD JUST STARTED EATING LUNCH AND NOTICED AN UNUSUAL SMELL AND TASTE IN HIS SOUP AFTER TWO ORTHREE SPOONFULS. LATER THAT DAY EE WENT TO THE HOSPITAL FOR OBSERVATION. IT IS UNKNOWN AT THIS TIME HOW THIS INCIDENT OCCURRED. THIS INCIDENT IS UNDER INVESTIGATION BY THE SHERIFF'S DEPT. EE HAS NO ILL-EFFECTS FROM THIS INCIDENT.
