In order to dump a large rock from the bed of the Caterpillar haul truck, the Miner stopped short of the dump to unload on the top of the dump. The dump spotter misjudged the distance to berm causing the rock to rest on the berm and the tail of the truck when dumped. This caused the front of the truck to be jarred, injuring the Miner.

Mostrando los 6While working from a scissor lift stationed on the plant concrete floor, the Miner reached from the basket to access and replace a rubber boot. The shift of the Miner's weight caused the basket to shift and pinched the right hand of the Miner between the basket and the structure causing a fracture of the 5th metacarpal.
While attending the feeder station at the raw coal hopper, the Miner was positioning a timber for removal from the feeder. As the loader was filling the hoper with ROM, two pieces of rock or coal toppled the swing gate and struck the Miners right hand.
