Employee was cleaning the C-3 Conveyor Deck with a shovel while the plant was running. EE's Left Hand got caught by a trough Idler and was pinched between the belt & idler.

Employee was attempting to clean a return roller on conveyor under the HP 300 crusher, against company policy, without utilizing lock out tag out training. EE was using a bar to scrape mud off the return roller. The bar was caught between the roller and moving conveyor belt, pulling the EE's left arm into the pinch point and resulting in broken bones and a dislocation.
